Toileted before shower. Medium urine output. Toileted after puzzle time. Medium urine output. Toileted after head massage. Medium urine output. Teeth brushed before puzzle time. 58 BPM RHR. 
Activities: 
Sorted weekly medications, 35minutes. Puzzle time. Head massage. Watched Tv.


Progress notes: 
Greeted Sachin’s family at 1730. Sachin was in the shower when I arrived this afternoon. Minimal sleep for previous night meaning extra level of caution taken. Lavender oil applied every half an hour, cava drink, 2 cups of double camomile tea as well as extra meds. Sachin was very on edge this afternoon. His eyes were darting back and fourth, very vocal and was concentrating on what everyone was doing around him at all times. At one stage while we were doing our weekly medications routine Sachin teared up but reason is unknown. When Sachin was doing the medication he wasn’t being his usual particular self with the placement of the tablets in the containers. We headed into the puzzle room with our cups of tea after I assisted Sachin with his teeth brushing. Sachin was back and fourth from doing his puzzle. 1830 meds taken. Saj mentioned that we needed to be hauling the flash cards to assist Sachin with his routines. Saj also mentioned that Sachin didn’t eat his dinner tonight and that was very unusual for him. I also found it very hard to prompt Sachin to drink his cups of tea even when using the flash cards. Heart rate between the 80’s-90’s. Sachin Toileted again at 1845 with a medium urine output. Sachin made good progress with his puzzle despite the lack of sleep. We went into the family room at 1930 and Sachin a grandmother gave him a head massage for 1 and a half hours. This usually relaxes Sachin but his heart rate was still remaining high 80’s. Saj prepared the cava tea for Sachin and also gave him a bowl of corn chips which are his favourite. Sachin didn’t eat many chips at all usually he finishes the bowl in 2 minutes. Sachin was very emotional tonight and Saj said maybe it was to do with the Indian tv show we were watching. Sachin was very restless while we were sitting on the couch watching tv. His heart rate dropped right down to 65 and then jumped back up to the mid 80’s. After 10mins of Sachin being vocal his heart rate dropped back down to 65. Sachin was making his belly jiggle to hear the liquid in his tummy move. He kept picking at the chips he had but wasn’t eating them all at once like usual. Sachin was sitting on the couch and his heart rate was in the 70's when my shift ended at 2130. ​
