Support worker:- James.

Seizure risk level:- Risk level, minimal.

Previous night Sleep:- 8.05 hrs.

Resting heart rate:- 62 bpm.

Activities:

Jigsaw puzzles in programs room.

Walking.

Wobble board.

Mood:

Sachin appeared to be well rested, despite having a seizure last night.

Sachin did not display any after affects of his seizure, unlike last time.

Sachin appeared to be having a normal day.

Weather:

32 degrees, sunny.

Notes:

Sachin had a seizure last night, 2239 hours, toilet bathroom room area, 9.51 hr sleep the night before.

Sachin appeared normal and well rested today, he showed no difficulty with completing pieces of his jigsaw, he wanted to keep a normal walking speed during our walk(2823steps/30mins), instead of his slow stroll, he actually pulled me along to hurry up on many occasions.

Sachin was very observant tonight, I was watched while touching puzzle pieces, if I moved a box full of pieces around the room he’d move it back.

Sachin had a large urine at 1740 and 2010 hours,

Medications at 1830 hours.

Wobble board for 15 mins after walk, then rested on couch with a cup of tea.

Shift ceased at 2030 hours.
