High hr but calm

Weather 31° mostly sunny 
Had already gone to the toilet when I arrived ,teeth brushed at 5.45pm

Activities:
Brushed teeth , work on current jigsaw puzzle ,walk ,short walk up Cambridge drive to the park and back

Progress Notes:
Arrived to start work with Sachin at 5.30pm Sachin was relaxing on the couch in the lounge room and appeared quite calm and relaxed ,Sachin had already been to the toilet so just brushed his teeth then grabbed our teas and headed into the Activities room to do some work on his puzzle, Sachin seemed more interested in stacking the boxes rather than working on his puzzle his hr was quite high sitting at 98 bpm and didn't drop below 90 bpm for quite some time, every now and then hed add some pieces to his puzzle but didn't really show alot of interest and every so often I'd grab a couple of pieces and give him a hand which he didn't seem to mind, staff continued to monitor his hr as it was constantly fluctuating up and down between 92 bpm and 95 bpm but still appears calm his medication was given at 6.30pm and continued to plod away with the puzzle until 7.30pm when we packed up and got ready for a walk and due to his high hr we only went up Cambridge drive to the park where Sachin sat on the swing before we made our way back home,Sachin was quite heavy on the arm and vocalising quite a bit and completed 1042 steps at a very moderate pace toileted again by Saj when we got back and relaxed in the lounge till end of shift again his hr not dropping below 90 bpm .

Toileted after medication sorting. Large urine output. Teeth brushed before puzzle time. 56 BPM RHR. 
Activities: 
Sorted weekly medications, 35minutes. Puzzle time. Short walk. Watched Tv.


Progress notes: 
Greeted Sachin’s family at 1730. Sachin was in the puzzle room this evening when I arrived. I organised the medications ready for Sachin to put in the next weeks days containers. Sachin had to make sure everything was perfect tonight while sorting the meds into their containers for the week. The lids were perfectly lined up and all tablets in the right spots. This took approx. 35 minutes. Sachin was very vocal tonight. Sachin was walking around the house after we did the meds. Sorted out his mothers clothes after her shower and then wandered into his own bathroom. Turns out he needed to use the toilet. Large urine output at this time. We got our tea and headed into the puzzle room. Sachin was extremely focused on organising the boxes again and stacking them right. Sachins heart rate went up to the 100’s and eventually went down to the 80’s and that’s where it stayed for the duration of shift. Sachin kept blocking his ears while we were doing the puzzle but he would fist bump me to ensure me he was okay. 1830 meds administered and taken. Saj and I were trying to help Sachin by putting some pieces in but Sach would just pull them back out and then go back to stacking the boxes perfectly. 1930 rolled around and we headed out to go for a walk. We went all the way down around Cambridge and got half way back when Saj picked us up due to the rain that was about to come. Sachin was very excited and smiling the entire time while we were walking. He was also very vocal. He set the pace fast at times and then slowed right back down. 1,544 steps complete. Upon arrival home I assisted Sachin with brushing his teeth. Sachin wouldn’t sit still in the family room. His heart rate went right back up to the high 90’s. Sachin returned from the toilet and sat down with his sleep tea. His heart rate dropped back to the 70’s again. Praveena gave Sachin a bowl of corn chips and he was very content. Sachin took his bowl up to the counter but then returned back to the couch with the rest of the corn chip packet asking for another serve. His HR went to 90 while he was eating his second serving of corn chips. I said goodbye. 
Shift ended at 2100. ​

ACTIVITIES.
Jigsaw.
Watering plants.
 
WEATHER:
32 degrees, sunny.
 
Mood:
Sachin appeared to be in his own world tonight and I was just along for the ride he appeared more focused on his puzzle work, I did manage to get one moment out of him and a fist bump.
 
NOTES:
Sachin was already in his programs room getting a few sneaky jigsaw pieces in before I arrived.
I stayed with Sachin in his room and attempted to help him, I noticed his BPM climbing so I backed off, taking that as a sign he didn’t need my help.
His bpm fluctuated over minutes high into the 90’s 100’s and Down to mid 60’s, I took him to the toilet, and the very second he got in there he let go a large urine, then we moved on to brushing teeth, after that we went back to programs room and jigsaws.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
At 1930 hours Sachin and I tried something different, we went outside and sat with his father in the garage.
Sachin watered his plants then we went inside and returned to our normal routine, a snack bowl and a Indian movie with Nan and mum.
Sachin was more open to my presence the later the evening got, he gave me numerous fist bumps, even double handers.
Shift ceased at 2100 hours.

Quiet day due to seizure

Toileted at 5.40pm large urine output and also moved bowels ,teeth brushed

Activities :
Brushed his teeth
Finished off the model plane we were making ,short walk up Cambridge to the park played on swing then back

Progress notes :
Sachin was relaxing in the lounge room when I arrived tonight and was straight away toileted passing a large amount of urine and also moved his bowel but wasn't a large amount ,hands washed then brushed his teeth then grabbed our cups of tea and made our way into the activity room to finish the model we made last week ,Sachin was showing quite a bit of interest but wasn't smiling and was more content watching what was going on Sachins hr at the start was a little high in the mid 80s music was put on and diffuser put on just to help him settle ,was required to keep a close eye on him due to having a seizure the day before ,Sachin was pretty calm and relaxed throughout 6.30pm medication was given and Sachins hr was starting to come down and was sitting around 70 bpm to 75 bpm we finished up at 7.30pm and got ready for a short walk going up Cambridge Dr to the park and let Sachin play on the swing for a while then we slowly made our way back with Sachin feeling quite heavy on the arm and was very vocal, when we arrived back Dachin headed back into the toilet and passed another large amount of urine and again moving his bowel and seemed to be struggling he then made his way into the lounge room and sat down on the couch with Preveena with some doritos and was monitored until ending shift at 8.30pm his hr was sitting at 75bpm .

Puzzle troubles

Toileted after medication sorting. Then again after head massage- medium urine output. Teeth brushed before puzzle time.
Activities: 
Sorted weekly medications, 33 minutes. No walk tonight due to high risk. Puzzle time. Watched Tv and head massage. 


Progress notes: 
Greeted Sachin’s family at 1730. Sachin was in the puzzle room this evening when I arrived. I organised the medications ready for Sachin to put in the next weeks days containers. Sachin required a lot of prompting to come out of the puzzle room to organise the meds. He also required a lot of prompting to put the tablets in the right spots. Usually Sachin is on the ball with this. After we sorted the meds Sachin went to the toilet and produced a medium urine output. I assisted him in brushing his teeth. We got our cups of tea and headed into the puzzle room. 1830 meds given and extra Epilim. All extra precautions taken due to lack of sleep the night before. Sachin’s heart rate was sitting in the high 70’s to 80’s and sometimes reaching 90. Sachin and I completing a good chunk of the puzzle and he allowed me to help him. He was very focused on sorting out the boxes though. At 1930 we headed into the family room and Sachin received a head massage from his grandmother for approx 40 mins. Sachins heart rate dropped into the 70’s whilst receiving the massage and stayed there for the rest of shift duration. Sachin went to the toilet again after the message and produced another medium output of urine. Sachin was drinking his sleep tea and sitting on the couch with Praveena when I left. Shift ended at 2100. ​

ACTIVITIES.
Walk

WEATHER:
30 degrees, sunny.
 
Mood:
Sachin appeared to be in a similar mood to yesterday’s shift, with slight differences of; he was more interactive with me, he didn’t sink into his own world as much plus he appeared more focused on his puzzle work.
 
NOTES:
Sachin was in the kitchen when I arrived, he was cleaning his tea pot and getting it ready with tea bags and water.
Sachin was encouraged to use the toilet, he didn’t go so we moved on to brushing teeth.
We picked up cups of tea and went to puzzle room.
Sachin was showing good attention to his puzzle at first, then he paid more attention to stacking old jigsaw boxes one on top the other and making sure the edges were perfectly aligned with each other.
Sachin regained his interest in his puzzle and was not interested in letting me touch any pieces tonight, and when I did push my luck and touch some he’d drop what he was doing and take it off me straight away. Sachin was very much in control with how he had his boxes aligned and where they were positioned right through to being in charge of touching pieces and putting them where they go, no James help allowed.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
He did more puzzle till 1720 hours, he  was encouraged to use the toilet once more, this time he did produce a large urine.
At 1930 hours we stayed local and walked around the streets, Sachin had a easy walk tonight and a good play on the swings at the park, Sachin did 1649 steps.
We returned home and rested up in the lounge room with mum and nan, Sachin also had Kava drinks to help him sleep well over night.
 
Shift ceased at 2100 hours.

WEATHER:
28 degrees, sunny.
 
Mood:
Sachin appeared to be in his own world this evening, he had his fingers in his ears for the majority of the shift.
He was wild eyed and alert, but would zone out quite often, staring into space.
 
NOTES:
Sachin was in the kitchen when I arrived, he was seated at the kitchen table with his fingers in his ears.
Sachin was encouraged to use the toilet, he didn’t go so we moved on to brushing teeth.
We picked up cups of tea and went to puzzle room, sachin had a very high heart rate all shift, I paid particular interest to Sachin when he would spike for no reason,
Medication at 1830 hours, dispensed by his father.
Sachin was encouraged to use the toilet once more, this time he did produce a large urine.
We done a bit more puzzle, Sachin let me help him tonight, he didn’t argue when I touched pieces and his heart rate didn’t rise when I did.
At 1930 hours we drove to Brooklyn fields and walked around the streets, Sachin did 2716 steps in 27 minutes.
We returned home and rested up in the lounge room with mum and nan.
Shift ceased at 2030 hours.

Activities:
Brushed teeth, put together a wooden model jet plane ,walk up Table top road to first bridge and back

Progress notes:
Sachin was pretty calm and relaxed when I arrived tonight after getting a bit of a run down on how Sachin had been going I toileted him.at 5.50pm large output then grabbed our teas and headed into the activities room, this time we did something different at Sajits request putting together a wooden model plane which Sachin thoroughly enjoyed and was extremely excited and focussed on what we were doing and very eager to help putting all the pieces together then helping paint it ,Sachin didn't even give the puzzle a second thought tonight and was smiling the whole time and engaged well ,we finished up at 7.30pm and headed into the lounge and sat down for a minute then got Sachin ready for his walk only going up Table Top road to the first bridge and back, Sachin was actually dragging myself tonight and was full of energy and completed 1714 steps and taking 17 minutes was also very difficult to get a good read on his hr due to the internet being down but when it was working it averaged between 70 and 80 bpm for most of the shift ,when Sachin was in the lounge relaxing I noted that Sachin at times was quite fidgety and finding it difficult to sit still ,Saj has me stay back til 9pm as an extra set of eyes to monitor Sachin .

Toileted before shower time. Then again before meds sorting- large urine output. Toileted after teeth brushed after puzzle time with large urine output. 
Activities: 
Sorted weekly medications, 20 minutes. Went for a short walk along Cambridge, 1000 steps completed. Puzzle time. Watched Tv and ate snacks. 


Progress notes: 
Greeted Sachin’s family at 1730. Sachin appeared to be very distracted this afternoon. Toileted before sorting weekly medications with a large urine output. Sachin assisted me in sorting out the week aheads medications. This room approx. 20 minutes. We got organised for our usual walk along Cambridge but had to make it a quick one due to the weather forecast and Sachin not being 100% focused. Sachin was very vocal in our walk and heavy on my arm so it was a slower paced walk tonight then usual. We still completed 1000 steps. 1830 meds given when we arrived home. We got our cups of tea and headed into the programs room. Sachin was very focused on his puzzle tonight. He spent a lot of time going through each box of pieces. Sach’s heart rate remained in the mid 70’s to 80’s. Once finished in the programs room I assisted Sachin in brushing his teeth. Sachin also went to the toilet on his own terms and produced a large urine output. We then headed into the family room for some iPad time and snacks at 1945. Sachin was very up and down from the couch tonight, watching over Praveena meal prep her lunch. A few times blocking his ears, and then jumping around being vocal. Sachin was enjoying his sleep tea and a snack Praveena got for him when I was leaving. 
I said goodbye. Shift ended at 2030. ​

Full focus day.

Continued to work on current puzzle. Head massage from Grandmother.

Reserved and quiet, very particular and possessive at times.

• Notes:
1733, Took Sachin to the toilet.
1735, Assisted in brushing Sachin's teeth.
1738, Moved to activities room and continued new puzzle and has made good progress. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every half an hour to help relaxation and reduce heart rate. 
1830, Regular And extra medication given to Sachin by Sajit.
1843, Light blackout whilst doing puzzle, Saj and I got him to the ground and in the recovery position, came to and was up and back to his puzzle in 6 minutes.
1932, Took Sachin to the toilet.
1936, Head massage from Grandmother to reduce stress and encourage sleep.
2030, Family room for sleep tea and videos on the iPad.
