Today I arrived and was greeted by saj. Sachin was inside at the kitchen table we fist pumped hello and headed straight to the toilet for a mid output. We then swung round to the bathroom and brushed teeth after this Sachin and I grabbed our teas prepared by mum and set off with a mission straight to the puzzle room at approximately 6pm Sachin had his medication. At about 6:15 I began to show Sachin a new word in auslan also known as sign language the word we focused on is “sit” this involves keeping one hand flat and placing the other arched over onto the palm of the hand. 7:30 then came along and we packed down and got ready for a small walk. Sachin showed enjoyment in the work by walking at a fast pace while being verbal. We got back home and Sachin chilled on the couch at approximately 8:20 Sachin had a small snack. 9pm came and I headed off.

Today I arrived at met with Sachin on the couch I headed with sach to go brush his teeth Sachin indicated to be that he needed to used the toilet he did this by hand signal Sachin had a kid output this was at approximately 5:30pm we then continued to the bathroom to brush his teeth. After this Sachin and I grabbed our teas and he added to the puzzle room we did our usual set up with lights music and oils at approximately 6pm Sachin had his medications. At 7:38pm we packed down the puzzle room and headed for the toilet once again to a mid output. At 8pm Sachin went and hung out on the couch watching his iPad. Sachin was full of giggles today. 8:20pm Sachin was given a small serving of cake and home made icecream from mum. 9pm came round and I finished my shift

9hrs 8 minutes sleep 64 rhr Weather 31° Sunny Activities Straight into program room to work on puzzle walk later in the evening ,toilet ,brushed teeth Progress Notes Arrived at 5pm to commence shift with Sachin who was standing in the kitchen then disappeared into the program room and was eager to get into his puzzle hr steady at 70bpm ,Sachin was quite vocal tonight ,got a visit from Olivia who was here for a get together and Sachin was at first a little standoffish but quickly came around and was interacting well with her ,toilet just on 6pm with very large output then took his medication from Saj at 6.05pm and 5hen headed to the bathroom to wash his hands with some hand over hand assistance then happy to brush his teeth once done Sachin had his dinner at the dining table and was happy to sit there with everyone until just after 7pm then made our way back to program room until about 8pm then decided on going for a walk up Cambridge drive to the Table top rd and back very vocal on walk and was powering through the walk completing 1815 steps in total now just relaxing in the lounge having some birthday cake and chilling for remainder of night hr a little high probably due to all the people and noise . ACTIVITIES. Jigsaw puzzle. Evening walk. House chores in patio area. WEATHER: 31° sunny day. NOTES: Sachin was inside the house when I arrived for shift. Sachin has had a late start again and so had dinner late, he had bangers and mash with steamed veggies. Sachin was encouraged to use the toilet, he produced a large amount of urine. Sachin was assisted with brushing his teeth, Sachin was in a cheeky mood and at times pursed his lips or fought the brush with his tongue. Next on the list is making cups of tea, Sachin followed instructions well and gathered the ingredients from the pantry, I then made the cups up and together we went to the programs room. We made good progress on his puzzle tonight, Sachin was interested and put his own pieces in. At 1930 hours we packed up and headed outside to clean some patio chairs with a dust pan brush, Sachin helped me carry the chairs out to the lawn, he had a go at brushing some cob webs off then got bored quickly. We then left the house and went for a walk up the street and back, approximately 20 minutes walk. Sachin and myself then assisted his father carry some groceries into the house then rested on the lounge watching ipad cartoon network. Sachin had a good low to medium heart rate tonight, he appeared to just go with the flow and not be bothered by anything, all up he had a good night. Shift ceased at 2100 hours. ACTIVITIES. Jigsaw. Fishing. WEATHER: 29° sunny day. NOTES: Sachin was inside the house when I arrived for shift. Sachin’s mother arrived home with KFC for dinner, Sachin appeared very impressed and demolished his share, with no worries of stopping. Sachin and I then made our cups of tea and went to the programs room for more jigsaw puzzle edging. It took Sachin a few minutes to get going he wanted to be lazy and wait for me to slide the pieces he found in the spots, I encouraged sachin to slide them in himself, he did so and started flowing along well. Sachins heart rate was quite high at the start, low to mid nineties, vigilant observation of Sachin and calming lavender was used regularly. At 1915 hours we packed up and left the house, we went to Bowna boat ramp for fishing. I cast out for Sachin and he’d reel in, once again we had no luck but Sachin appeared to enjoy himself, evident by his laughs and giggles. At 2020 hours we headed home for a cup of tea and movie time with mum. Sachin had a good night with no issues or behaviours. Shift ceased at 2100 hours. Support worker:- James

ACTIVITIES. Jigsaw. Fishing. WEATHER: 30° sunny day. NOTES: Sachin was inside the house when I arrived for shift, he had just finished his shower, though was dried and dressed. Saj served Sachin up his dinner which he devoured tonight, after that Sachin was encouraged to use the toilet, he didn’t go so we brushed teeth. Sachin and I made cups of tea then went into the programs room to start a brand new jigsaw puzzle. Sachin put roughly half of the edge pieces together before we packed up to go. Sachin and I headed to bowna water boat ramp and set up for fishing. Sachin is getting good at reeling and so was tried with a lure, I cast out and sachin reeled in without a single snag. We kept this routine up for an hour with no success of landing a single fish, Sachin and I headed home at 2030 hours. Sachin had a toilet break when we returned home, medium urine, then had a sleep tea while watching TV with mum. Shift ceased at 2100 hours. Arrived at 17:00 greeted and went to Sach he was served dinner and then after he finished he went to the toilet 17:30 very small output. We then brushed teeth grabbed tea and headed to puzzles to finish the puzzle. 18:00 Sachin had his meds 18:23 we finished the puzzle!! We then went with saj to the bottle shop and returned home by 19:00 sach then sat on the couch as guests are over. At 19:30 Sachin went to the toilet to a mid output. We then went back to the couch with the iPad and chilled. At 20:20 Sachin started a bottle of cider. Every 7 minutes I pored a small glass for Sachin. Sachin finished off his bottle of cider at about 20:50 he definitely enjoyed this beverage. 20:55 saj got a cider ready for Sachin. 21:00 came around and I headed home.

Arrived at 1700 Sachin was on the couch. At about 17:20 Sachin was served dinner after Sachin was finished we went to the bathroom and brushed teeth. 17:40 we grabbed tea and went to do puzzles 18:00 Sachin had meds. Sachin appears to be in a good mood today very vocal 20:00 Sachin went to the toilet to a very large output. 20:20 Sachin had a small snack and sat with his iPad 21:00 came around and I went home Support worker:- James

ACTIVITIES. Jigsaw. WEATHER: 24° rainy day. NOTES: Sachin is of the medium risk level tonight, he had minimal split sleep but appeared fine, no jitters or shaky movements. He was a lot faster with his comprehension of requests tonight. Big improvement from last night. Sachin was at his residence inside when I arrived for shift, he was seated in the lounge with his mother watching TV. Sachin had a toilet break, nil urine produced, then I assisted him brush his teeth. Sachin and I made cups of tea, Sachin put all the ingredients away properly and promptly. Sachin had a good go at his puzzle tonight, he completed a full line, Sachin got me to put the pieces in, he is still going with this tactic. At 1945 hours Sachin had a toilet break he did produce large urine. Sachin had a sleep tea at 2000 hours while we watched TV with mum. Shift ceased at 2100 hours. Support worker:- James

ACTIVITIES. Jigsaw. WEATHER: 36° sunny day. NOTES: Sachin was at his residence inside when I arrived for shift, he was finishing off his dinner. Sachin and I had cups of tea made for us tonight, we acquired them and headed straight into the programs room for jigsaw puzzle time. Sachin was on a mission tonight, he found pieces very fast and also was comfortable with my assistance to put the pieces in. Sachins BPM was abnormally high, sitting around 100 so we tried a few things to promote the calming environment. We did have a pedestal fan blowing in the room which was blowing away the lavender diffuser, I put Lavender oil on his top lip to go straight up his nose and occasionally a few sprays of lavender into the fan to disperse it around the room, this appeared to work and his BPM slowly crept down. Sachins behaviour and attitude was on point tonight, he was well behaved and had no grumpy assertive moments. Sachin appeared happy to go with the flow of the routine. At 8pm Sachin and I took up seats in the lounge and settled in to watch a movie with mum. Sachins BPM at this stage slowed to mid 70’s to mid 80’s, tha calmest he has been all night. Sachin had a great night and no issues. Shift ceased at 2100 hours. |
