Movie Sachin was in the shower, being assisted by Praveena when I arrived. I spoke with Praveena for short while, and then helped guide Sachin to the lounge room, as he was very unsteady on his feet. I sat with him while he drank his tea and ate his breakfast. His heart rate was very high (100-110 bpm), so I kept a very close eye on him while Praveena had a shower and then prepared dinner. He was trying very hard to stay awake. Sachin had a special dinner of KFC chicken. He ate much slower than usual, but still clearly enjoyed his favourite meal. I cleaned him up and guided him back to the couch so he could finish his tea. Sachin appeared very tired and looked as if he was struggling to stay awake. I made us a couple teas and we sat on the couch and watched TV with Praveena. At 1900, Praveena got us all some homemade icecream. Sachin perked up at this and appeared very happy to be eating it. I prompted Sachin to the toilet after, he did not go. Sachin sat back in the couch, appearing tired again. We prompted him to lay down and he rested and we watched Tarzan for the rest of the evening. Sachin’s heart rate settled into the 60s and 70s for the evening. Overall, Sach was quiet and very tired this evening due to him being sick. No major issues. Sent from my iPhone

ACTIVITIES. Drive. Movie time. Jigsaw. WEATHER: 34° sunny. NOTES: Sachin was fine today, he has had plenty of sleep and presented no complications. Sachin followed nightly routines well with minimal setbacks. Sachin and I started out by fixing up his tea pot ready for tomorrow. Sachin had dinner then a toilet break. I assisted Sachin to brush his teeth. Sachin and I made cups of tea and done his jigsaw puzzle. At 7:10 we went for a drive through lavington l, Sachin likes his drives. Sachin and I made another cup of tea and had a toilet break when we got home. We then sat with his mother watching hypnotic movie. Sachin has been good today, his bpm was a little high but no worries. Sachin was a delight. Shift ceased at 2130 hours. Support worker:- Katy

Puzzle Walk Movie Sachin was in the couch finishing a bottle of water when I arrived. I chatted with Praveena for a short while, before following Sachin into the programmes room. He put a few pieces in before being called to dinner. I sat with him while he ate, and then assisted him with washing g his hands and cleaning the dishes. I then prompted him to the toilet (medium amount), and assisted brushing teeth. We then made a teas together, Sachin was alert and helpful. Once in programmes room, Sach was immediately engrossed in the puzzle. His heart rate was in the 90’s for the start, but eventually settled into the 70’s. We finished up at 1930 and I prompted Sach to toilet again (very large output). I then put Sach’s shoes on and we went for a walk. Sach was powering along, and communicated that he wanted to walk further than the second bridge, I decided not to push it too far, and Sachin eventually turned around. He did not appear to be upset at this. We got back and I put Sachin’s slippers back on. I got him a cup of water and he went and relaxed on the couch while I made his tea and got him a snack. I sat down next to him, he refused to eat his corn chips himself, so I had to hand each one to him. He did, however, take out bowls to the sink once I finished. Sach came and sat back down, he was calm and relaxed for the rest of the evening, with a bpm in the 70’s and low 80’s. Support worker:- James

Shift Times, Date :- 1600 hrs to 2130 hrs, Friday the 19 th of January 2024. ACTIVITIES. Gardening. Movie time. Jigsaw. WEATHER: 28° sunny. NOTES: Sachin has had very little sleep last, we stuck to our nightly routine well. Sachin did take extra time to complete his tasks, not from physical tiredness but mentally he was fatigued. Sachin would get side tracked frequently. Once Sachin had his dinner I encouraged him to use the toilet, no go. I assisted Sachin with brushing his teeth then we made cups of tea. Sachin took extra time to collect his ingredients, and a encouraging prompt. Sachin and I put some time into his puzzle, he appeared to focus well on this task, no distractions. At 1900 hours Sachin used the toilet, large urine. Sachin put an extra 15 minutes into his jigsaw then at 1945 hours we went outside and harvested the egg plants. Sachin and I made another cup of tea and sat with his mother in the lounge to watch a film, Fast Charlie. Sachin received a head massage from his mother. Sachin surprisingly had a great heart beat today, between 60 and 84. Sachin has been very vocal, but making happy noises. Shift ceased at 2130 hours. Support worker:- Katy

Puzzle Walk Movie WEATHER: 25 degrees - mostly clear Sach was in the programme’s room with praveena, his OT and his speech therapist when I arrived today. We all greeted each other, Sachin appeared to be happy and calm, he was completing his puzzle. There was a mistake that he made, and it took approx. 10 mins to rectify and convince him that it was the wrong piece. Sachin remained in a calm state with a bpm in mid 70’s during this, which was excellent. After the OT and ST left, we went out to the kitchen to have dinner, Sachin quickly ate his and appeared to enjoy it a lot. We then cleaned up and the family sand me happy birthday over a cake Praveena had made for me. Sachin was all smiles during this and seemed exited for it. However, before cake, we decided to go for a walk. Sachin was smiling and laughing for the whole walk, and when I went to turn around at the first bridge - he very clearly wanted to keep going. So we walked another couple hundred metres before turning back. He sat an excellent pace. Once back home, I prompted Sach to have a shower. He went to the toilet beforehand (large output) and was very cooperative with the shower routine. We then cleaned teeth and made teas and headed back to programme’s room. Sachs heart rate was great to begin with, but then spiked into the 90’s and didn’t get much lower. Despite this, he was calm and visibly was having no issues with the puzzle. We finished up at approx. 1945. Sach went and sat on the couch with Praveena while I prepared hot drinks and slices of cake for us all. Sachs heart rate was in the 90’s and higher while on couch and we couldn’t settle him. I prompted him to toilet (small output) with no improvement in heart rate. ACTIVITIES. Movie time. Jigsaw. WEATHER: 26° Overcast. NOTES: Sachin had just finished his dinner when I arrived, we started our routine. I encouraged sachin to use the toilet, he didnt go. I assisted Sachin brush his teeth, we then made cups of tea. Sachin and I done some jigsaw puzzle work, Sachin was very good tonight. Sachin found pieces fast, he focused well, and displayed minimal ocd traits of perfection. At 1930 hours Sachin used the toilet, medium urine passed. Sachin and I went for a drive to lavington and back through residential streets of Thurgoona. Sachin appeared to enjoy himself. We returned home to make cups of tea and a snack of corn chips. Sachin and I watched a movie with his mother. Sachin has been very good tonight. Shift ceased at 2130 hours. Support worker:- James

ACTIVITIES. Water garden. Movie time. Jigsaw. WEATHER: 27° partly cloudy. NOTES: Today Sachin was inside when I arrived, he was drinking tea. Sachin appeared happy and rested. Sachin had dinner then used the toilet for a medium urine. I assisted Sachin brush his teeth then we made cups of tea. Sachin and I spent some time on his jigsaw, I assisted when he required it, he managed to complete an impressive amount. At 1940 hours we exited and Sachin used the toilet once more for a large urine. I assisted Sachin water the vegetables then we made a cup of tea. We watched x files with his mother in the lounge. Sachin has been quiet, by choice, Sachin has had a good amount of sleep and has a good bpm to match. Shift ceased at 2130 hours. Support worker:- Katy

Puzzle Movie WEATHER: 30degrees - partly cloudy Sach was in the shower when I first walked in. I checked in with Saj and Sachin was out shortly after. Sach appeared a bit tired today. We sat on the couch and played with some fiddle toys while he finished off his tea from earlier and once done, we prepared the pot for tomorrow. I helped prepare dinner after this, and Sachin and I sat at the table and ate dinner together. We then cleaned up, brushed teeth, and prepped teas. Sach was a little slower with his routine this evening, but was otherwise ok. We then headed to programme’s room, his heart rate was low-mid 80’s while doing the puzzle, and he completed almost 3 lines. We packed up at normal time. Sach came out without hassle but did take a while to settle. It wasn’t until he has done some laundry and gone to the toilet (large) that he sat down and enjoyed a piece of cake and his sleep tea. He sat with Praveena and I and watched a thriller movie. His heart rate was in the 60’s until approx. 2100, when it spiked to 90’s. I prompted him to toilet again but he only went a small amount. He came and sat back down with us. His heart rate didn’t fully settle for the rest of the evening, be he appeared alert and happy. Support worker:- James

ACTIVITIES. Movie time. Jigsaw. WEATHER: 36° muggy and hot. NOTES: Today Sachin had two support personnel supporting him, he appeared to take delight in switching between us. Myself and the other support worker assisted sachins parents by doing a lot of cleaning and preparation work in the kitchen and patio area for a party they are having tomorrow night. Sachin had a good bpm during shift and even managed to get in his jigsaw time. Sachin appeared happy and vocal, his mother had music playing in the lounge area and he sung along to a few. Sachin and I had some couch time after 2000 hours, he watched a bit of TV and listened to music. Shift ceased at 2130 hours. Support worker:- Katy

Puzzle TV Show Folding Laundry WEATHER: 35 degrees - sunny Sachin was drinking his afternoon tea when I arrived, he appeared calm and happy. I tried to turn the dining room light on, but he got up and turned it off and then sat in the dim light and continued to drink his tea. I left it this way while I assisted Praveena in the kitchen with dinner. I turned the light back on while we ate - Sachin didn’t appear to be bothered by this. After dinner, we cleaned up together and I then prompted Sach to the toilet. He did his routine fiddle with the toilet paper and went to toilet (small output). I then assisted with cleaning his teeth and then made teas together. Sachin was very particular when picking out teabags today. We then went to programme’s room. He was a bit fussy with the way that pieces were being placed, initially, but he was focused and smashing out the puzzle as soon as he sat down. BPM was in the 70’s and 80’s. We finished up at 1930 and Sach sat on the couch while I helped around the kitchen. I made him a tea and got him a snack at 2000. Once he was finished, he got up and folded clothes in the laundry and then moved onto folding clothes in the walk in. While he was up, I printed him to toilet (small output). It wasn’t until approx. 2045 that he sat back down and settled for the evening while watching TV. His heart rate was in the 70’s from 2000 onwards. He was smiling and giggling for the whole evening. |
