• Notes:
1530, Chat with Saj about how Sachin's week had been and any changes that need to be made.
1540, Cleaned and organised the activities room while Sachin finished his morning tea.
1630, Sachin and I started the new Lego soccer stadium.
1710, Sachin had his dinner.
1730, back to activities room to continue on Lego.
1740, Transitioned to puzzle and he had his camomile and green tea
1830, medication administered.
1832, Took Sachin to the toilet, large output.
1836, Sachin organised his tea for the morning.
1840, Back to activities room for more puzzle.
1910, moved to the dining to get ready for walk.
1917, walk up and back table top rd, 3138 steps in 35 minutes.
1955, Family room for tea and videos on the iPad.
1745, Took Sachin to the toilet, medium urine.
1738, Assisted in brushing Sachin's teeth.
1751, Moved to activities room and continued new puzzle and has made good progress. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every half an hour to help relaxation and reduce heart rate.
1830, Regular medication given to Sachin by Sajit.
1928, Took Sachin to the toilet.
1933, Short walk up Cambridge dr, 5-10 minutes on the swings, then short walk up Table top rd. for variety, 2101 steps in 27 minutes.
2005, Family room for sleep tea and videos on the iPad with a bowl of corn chips.

Well Rested

ACTIVITIES.
Jigsaw.
Walk
 
WEATHER:
31 degrees, sunny day, passing showers evening.
 
Mood:
Sachin appeared well rested today, he was easy going and went with the flow of our night routines well.
No arguments from Sachin.
NOTES:
Sachin was already in the programs room doing his jigsaw puzzle when I arrived, Sachin was encouraged to use the toilet, he produced a large urine.
6:30, 7:30 and 8:30 hours Sachin was encouraged to use the toilet, he did go at these times.
Sachin was assisted with brushing his teeth.
We then made our cups of tea and started work in the programs room.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
At 1930 hours we packed up in the programs room and got ready for a evening walk.
Sachin collected his shoes and I assisted by putting them on and tying his laces.
Sachin and I walked to the park down the road and had a short swing in the park.
When we returned home we had a cuppa and watched TV with his mother and nan in the lounge.

ACTIVITIES.
Jigsaw.

WEATHER:
31 degrees, sunny day.
 
Mood:
Sachin appeared well rested, he appeared dazed and or confused at times but during all other he was normal Sachin dedicated to his routine.
NOTES:
Sachin was already in the programs room doing his jigsaw puzzle when I arrived, Sachin was encouraged to use the toilet, it was observed he had wet his pants on the way to the toilet, Sachin still produced a large quantity of urine and opened his bowel.
Sachin was assisted with brushing his teeth.
We then made our cups of tea and started work in the programs room.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
Sachin put a few lines of his puzzle together, he is still working away one piece by one piece left to right bottom to top.
Sachin had enough puzzle at 1920 hours, he stood up and checked my coffee, this is a good indication from Sachin he’s had enough.
Sachin was encouraged to use the toilet, large urine output.
We set off for a walk to the park and had a swing when we got there, Sachin had enough after 10 minutes and got up and started walking.
We returned home to use the toilet again ( large urine ) , then got new cuppas and sat on the couch to watch Married at first sight.
At approximately 2003 hours Sachins BPM jumped to 116 and he was dazing off unresponsive, he was assisted into recovery position and quickly came back.
Sachin did not seizure but did appear to black out.
Sachin remained seated and calm afterwards, he watched MAF and he man on his ipad.

Good ol Sachin

Walk

WEATHER:
26 degrees, sunny day.
 
Mood:
Sachin was well behaved and no trouble at all tonight. Sachin has been displaying a perfectionist behaviour lately, any job or task he does will be done at his own pace and correctly, he will take his time and make sure he is satisfied he done it the way he wanted.
 
NOTES:
Sachin was sorting out his weekly medications when o arrived , I observed until he was done then started our nightly routine.
Sachin was encouraged to use the toilet, large quantity produced.
Sachin was assisted with brushing his teeth.
We then made our cups of tea and started work in the programs room.
Sachin displayed good attention to his puzzle, he struggled with a repetitive behaviour with every piece he picked up and placed but he was coaxed through putting one down and straight away moving on to the next.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
Sachin completed his Jigsaw tonight, once he placed the last piece he laid back in his chair and appeared satisfied.
Sachin and I finished in the programs room and set out for a short walk to the swings down the road, Sachin had his 10 minutes play then walked home with me.
Sachin was encouraged to use the toilet, he produced large amount of urine.
We made our tea’s and got a snack then sat in the lounge watching Dave Attinburough Animal Planet.
After that show Sachin and I watched The Bourne Ultimatum movie.
With a new bowl of vegie snacks.
At 2150 hours Sachin was encouraged to use the toilet again, he produced a large quantity of urine.
 
Shift ceased at 2200 hours.

Relaxed

WEATHER:
33 degrees, sunny day.
 
Mood:
Sachin appeared well rested, quiet and behaved.
He did bust out a few vocal noises every now and then but for the majority he was quiet. He appeared content with his puzzle and nightly routine.
 
NOTES:
Sachin was already in the programs room when I arrived, I encouraged him to use the toilet first up, large urine output then I assisted him brush his teeth.
We made cups of tea and went back to jigsaw puzzle.
Sachin appears to have stalled at the present time, I think it might be because he has flooded his work space with pieces and not enough room to work. Recently Sachin has taken to working line by line starting at the bottom and moving up to complete a puzzle.
Sachins BPM was fluctuating just like last night but stayed in the 80’s mainly, for the majority of the time he was a few units lower than last night.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
Sachin struggled at times being satisfied he had put the piece in exactly the way he wanted, he would repeat the action of removing and placing the piece a few or a dozen times till he was satisfied or I would assist him to move on to the next.
Sachin and I finished in the programs room at 1930 hours, Sachin was encouraged to use the toilet again, he produced a medium amount of urine.
Sachin and I then went for a walk along Tabletop road, for the 26 minute walk at a slow pace Sachin did 2852 steps with a average bpm of 111, hence the slow pace.
When we returned home we made new cups of tea and settled down on the couch to watch a Indian movie from the 70’s.
Sachin stood up at approximately 2110 hours and insisted on a head massage from his nan, he was assisted down to sit and got a head massage before bed.
Shift ceased at 2130 hours.

WEATHER:
33 degrees, sunny day.
 
Mood:
Sachin was a little vocal tonight, every now and then busting out little giggles, sighs or blowing air out. He struggled with a high BPM most of the night high 80's to high 90's, nothing I tried lowered it, but all calming methods were utilised, we kept a calm cool quiet environment for Sachin, Lavender diffuser, lavender oil under nose, chamomile teas, calming music, no loud noises, appropriate room temperature.
He showed good focus and dedication to his jigsaw.
 
NOTES:
Sachin was encouraged to the toilet at 1735 hours, he gave a large urine output, Sachin was then was assisted with brushing his teeth, he had a go himself after I got the bulk work done.
We picked up our cups of tea, and went to the programs room and started working on the Jigsaw puzzle.
Sachin was focused and dedicated to it tonight, I wasn’t allowed to help, evident by raised BPM when ever I got to close to pieces.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
Sachin finished his cup and looked distracted from his puzzle so we made another cup and went back to puzzles.
Sachin got a fresh surge of interest in his puzzle then at 1950 hours we finished in the programs room and anpther toilet break, large urine out put.
A short walk tonight to the end of the street to the swings, 10 minutes play then back home.
Sachin had a bowl of chips and a seat on the couch till 2130 hours watching TV, then it was bed time

Beautiful sunset at Lake Hume

Weather 29 degrees- sunny
Toileted before shower. Large urine output. Toileted after puzzle time. Large urine output. 58 BPM RHR. 
Activities: 
Sorted weekly medications, 35minutes. Puzzle time. Drive to lake Hume for our walk. 


Progress notes: 
Greeted Sachin’s family at 1730. Sachin was in the puzzle room when I arrived. He came out and greeted me in the kitchen and appeared to be very well rested. We hooked into our weekly medication sorting routine. Usually Sachin is pretty good with remembering where the tablets go in the sections but tonight he needed some prompting. Large urine output after medication sorting. We then got our cups of tea and headed into the puzzle room. 1830 meds given and taken. Sachin was very engaged tonight and focused on his puzzle. Sachins heart rate remained in the 60’s and he appeared to be content. He tipped 2x lids worth of puzzle pieces into the middle of the board and to me it looked like a chaotic mess but to Sachin it was perfect. He made really good progress on his puzzle tonight. Teeth brushed at 1930 and Toileted again with large urine output and we hopped into the car to drive to lake Hume. Sachin enjoyed his drive but enjoyed the walk more. Completing 1,767 steps. Sachin had a big smile on his face the entire time and enjoyed seeing the water rush from the lake gates. On the way back to the car I felt Sachin’s arm get heavy and he appeared to become tired. We arrived home at approx. 2020 and Sachin enjoyed another cup of tea in the family room. Shift ended at 2030

Solid sleep has returned

Back To Normal

Sachin was quiet and well behaved , he appeared well rested and calm, very much the same as yesterday.
 
NOTES:
Sachin went to the toilet at 1740 hours , large urine,  and then was assisted  brushing teeth.
We picked up our cups of tea, and went to the programs room and started a new jigsaw, finally finished the Marvel super hero one, Sachin wasn’t to interested in that one.
Sachins bpm was steady throughout the evening, hovering around 74, plus or minus 5.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
A bit more puzzle till 730 then off to the toilet, medium urine and open bowel.
Sachin and I went for a easy short walk to the park for a swing then returned home.
When we arrived back at the house I noticed Sachin had a few moments of staring off into space, he remained alert and would return my fist bumps, saj and Praveena notified.
 
Shift ceased at 2030 hours.

Productive Sachin.

ACTIVITIES.

WEATHER:
27 degrees, sunny day.
 
Mood:
Sachin was quiet and well behaved , he appeared well rested and calm.
 
NOTES:
Sachin Did not produce urine in the toilet at 1740 hours so we continued with routine and brushed teeth.
We picked up our cups of tea, and went to the programs room and got to work on this Marvel jigsaw.
Sachin current BPM was high, sitting stable around 90 , he climbed a few digits when I would help him so I backed off a bit and gave him space.
Medication at 1830 hours, dispensed by his father and handed to Sachin by me.
Sachin was then encouraged to use the toilet, large urine quantity produced.
Sachins BPM dropped into mid 80’s after toilet break and I think also he realised he could finish the puzzle tonight.
Sachin picked up the pace and hammered the pieces in and did finish.
Sachin and I then went for a quick 20 minute walk around the neighbourhood and stopped off at the park for a congratulatory 5 minute swing for a productive night.
Sachin finished the night strong.
 
Shift ceased at 2030 hours.
