ACTIVITIES. Jigsaw. Walking. WEATHER: 31° sunny day, partly cloudy. NOTES: Sachin was at his residence when I commenced shift, he was seated at the dining room table drinking the last of his cup of tea. Sachin appeared in a good mood, I got a fist bump straight up from him then encouraged him to use the toilet. Sachin produced a medium amount of urine then flushed the toilet and turned the light off by himself. I then assisted Sachin with brushing his teeth, using his electric tooth brush I brushed his teeth then handed the brush to Sachin so he could brush a few teeth himself. Sachin and I worked as a team to make our cups of tea, Sachin got the required ingredients and I made the cups up and poured the hot water. Sachin and I took our cups to the programs room and commenced work, I turned on the lavender diffuser, the calming music, made sure the lighting was just right and not too bright. Sachin was stuck on the same place as Friday, he has However started working from the opposite side coming back to it, we found the elusive missing piece that’s been plaguing us for 4 days today in another box of pieces on another table, winning but feeling silly. Once Sachin found the piece he powered on at a good speed filling in the gaps. He finished off completing a line then we left the house to bowna boat ramp. I had intended to go fishing with Sachin, I forgot my rods so we ended up walking along the weir bank. At 8 we returned home to a toilet break, small urine output, then a fresh cuppa while we watched Mobius movie with mum. Sachin appeared to have a good night. Shift ceased at 2100 hours.

Arrived at approximately 17:00 Sachin was at the table eating his dinner while Sachin was doing this I was having a chat with prev about Sachin. He had a seizure on the Sunday afternoon. Sachin does not have services on the weekend. Sachin has a small cut on his bottom inner lip from the seizure. Sachin is now in recovery mode. Once Sachin was finished his dinner he walked his plate to the kitchen sink and we headed to brush teeth at approximately 17:15. Sachin has a noticeable amount of blood in his spit after brushing his teeth I informed prev on this so she was aware. We then grabbed out cups of tea and set up the puzzles room. Sachin was struggling to find the right puzzle pieces but remained focused. At approximately 1800 he had his medication. Sachin appears to be in a good mood he is laughing and talking. At approximately 19:30 we packed down the puzzle room and removed our cups placing the tea bags in the bin and the cups on the sink. We then headed to try for the toilet to no output Sachin showed no interest or need for the toilet and instead tried to unravel the toilet paper I redirected Sachin to not do that. We then went to the couch to have relax time I handed Sachin his iPad and he sat next to prev on the lounge Sachin is more interested in the movie on the tv than his ipad tonight. At approximately 20:15 Sachin was giving a small serving of corn chips and was handed a fresh cup of tea. At approximately 20:45 I prompted Sachin to use the toilet to a small output we then came back to sitting on the lounge for the remainder of my shift. I finished at approximately 21:00. ACTIVITIES. Jigsaw. Drive to Aldi and petrol station. WEATHER: 30° sunny day. NOTES: Sachin was at home in the lounge room when I arrived for shift, we quickly had a toilet break, he produced a small amount of urine. I then assisted Sachin brush his teeth, I did the main big clean with his electric toothbrush then let him have a turn. Sachin and I made up cups of tea together, Sachin got the ingredients and I poured the hot water, we took these to the programs room for puzzle time. Sachin appears to be in a great mood tonight, he is not displaying any seizure warning signs, he is coherent and processing instructions fairly fast, although he is struggling with his puzzle, half hour into it and he has only found one piece. He’s more interested in taking the boxes of pieces off me than finding it himself, he’s all up in my business to worried what I’m doing. At 1900 hrs we went for a drive with his father to ALDI, while he shopped Sachin and I walked laps of the carpark. We dropped into the fuel station then went home. At home Sachin and I made cups and sat watching TV with mum. Sachin appeared to have a good night apart from doing next to nil on his puzzle. Support worker Steve
9hrs 41 minutes sleep 60bpm rhr Weather 29°Sunny Activities: Dinner and tea , toilet moderate output, teeth brushed, program room to work on puzzle. Progress Notes: Arrived at 4.50pm to commence shift with Sachin who was just about to have some dinner, he was extremely happy to see me and gave me a good fist bump and continued eating had a good catch up with Saj and Preveena whilst waiting for Sachin to finish then prompted him to use the toilet using sign and was repeated back to me moderate output then into the bathroom to brush his teeth then got our cuppas and made our way into the program room difuzer and music put on and applied some Lavender oil inside his nostrils and got to work on his puzzle and straight away making good progress meds given at 6pm via Saj ,Sachin at times quite often vocal and very focused on what he was doing hr was a little elevated at 85bm and a couple of times hitting 90 but the longer he went the quicker it started to drop staff giving Sachin plenty of praise which he was lapping up lots of smiling and laughing through out and Sachin interacting very well with myself leaving pieces for me to pop in hr remained steady at 83 bpm we eventually finished up at about 7.30pm and got ready to go for his walk heading along Cambridge drive to the park and back started off abit slow then began building some momentum Sachin really enjoyed the walk as he was quite vocal as well as laughing ,straight into the lounge to relax when we arrived back hr never dipped below 80 bpm throughout taken to the toilet just before shift end at 9pm with only small output. ACTIVITIES. Jigsaw. Walk. WEATHER: 31° sunny day. NOTES: Sachin was at home in the lounge room when I arrived for shift, we quickly went and attempted to use the toilet to no success, then I assisted brushing sachins teeth with him. Sachin and I made up cups of tea together then went to the programs room for puzzle time. Sachin managed ¾ of a line tonight, not his best but he did focus well. At 1930 hours Sachin and I went for a walk along Tabletop road, we made it a abort walk, 20 minutes but ling enough for Sachin to get out of the house and exercise. At 2000 hours we made sleep teas together and rested on the couch watching TV with mum. Sachin has had a good night, no incidents. Shift ceased at 2100 hours. Brayden Ballard
Sleep 11hr 1min 77BPM 59 resting BPM Sunny 30deg Risk low. Greeted Sachin in lounge relaxed. Sachin seemed to be in a good mood full of smiles. Teas were being made, Sachin got up to help he put the milk away for mum. We grabbed our tea and headed for puzzle room around 1745. We prepped room and applied lavender around 1750. At approx 1800 Saj delivered meds and I observed Sachin take them. More lavender applied around 1820 68 BPM and 4 puzzle pieces snapped into place. 1850 lavender applied. Sachin had a couple of tricky pieces to find but he stuck at it and eventually we snapped them in to, Sachin was happy to take a fist bump. Few more pieces found, Sachin seems well being a little vocal and giggling, Sachin’s even reached out to me for a fist bump after a piece was placed. BPM 70. Around 1935 Sachin is up and making his way to the toilet large urinal output made, Sachin continued to the bathroom to wash hands and brush teeth I physically assisted with the task. Sachin moved back to puzzle room I suggested we go for a walk a few times but Sachin didn’t respond. Saj came into room to get Sachin to go for a walk a stern voice got Sachin moving. Around 1950 we began to prep for a walk I physically assisted putting brace, shoes, hat and sunnies on. We headed down the street around the bend weather had cooled off and got windy the pace was slow and steady. Sachin was quite verbal we got around the bend and the wind blew Sachin’s hat off and he let out a good laugh. Roughly 100m past the bend we turned around to head back home. Sachin still pretty vocal letting out a couple of giggles but the paced had increased a fair bit but still stable. We got back inside around 2005 Sachin sat down in room for me to take his shoes off I did so and placed Sachin’s slides in front of him and he put them on unassisted. Shortly after Sachin sat in the lounge with his iPad and night time tea sharing a snack with mum. BPM 76. This is where Sachin remained when I left around 2100. Support worker:- James

ACTIVITIES. Jigsaw. WEATHER: 38° hot sunny day. NOTES: Sachin arrived home from a car ride with family at 1930 hours, Sachin and family went to collect Doner kebabs for tea, Sachin had his tea then I assisted him brush his teeth. Sachin didn’t want to go to the toilet so we will try later. Sachin and I then made up our cups of tea, as always Sachin and I worked together to achieve this. Sachin did display a lot of ‘side tracking’ and some ‘confusion on procedures’ tonight, for example Sachin would be on the way to collect suger from the pantry and divert to putting dishes away that mum had just washed up, he wanted to make teas up before using the toilet and brushing teeth, this is two example of strange things happening tonight so I had to keep on top of Sachin to keep him on track and focused, I put it down to having “Meat Brains” from the Kebab for dinner. We have a routine every night and it’s- toilet break, brush teeth, make teas, programs room, walk, couch time TV time. Sachin is a bot confused tonight and breaking routines. Sachin flowed along well with his jigsaw tonight, he was putting in the pieces himself, and at a reasonable pace for him. He struggled at times with his putting in, pulling out, putting in, pulling out of jigsaw pieces business but over all he overcame it by himself. The last hour of puzzle time is where he fell down, his “Meat Brains” caught up with him, the unfocussed moments, the side tracked moments all caught up and cost him possibly finishing the whole puzzle. 32° Celsius at 7:30pm so the evening walk was cancelled, plus Sachin is in no state to walk anyway so win win. We reluctantly stayed in the programs room till 8pm, and not just because we had the added bonus of two pedestal fans to ourselves, # spoiled! At 8pm we packed up and exited the programs room, Sachin had a toilet break ( medium urine output ) we then made new cups of tea and sat in the lounge room with mum watching TV. Sachin had a better bpm during TV time, average of 75 bpm. Sachin appeared to have a good evening, he was in a good happy mood. Shift ceased at 2100 hours. Support worker:- James
Seizure risk level:- Risk level- Minimum. Previous night Sleep:- 7:44 hours of sleep. Restingart rate:- 61 resting bpm. Shift Times, Date :- 1700 hrs to 2100 hrs. Wednesday the 15th of February 2023. ACTIVITIES. Jigsaw. WEATHER: 30° sunny day. NOTES: Sachin was inside the house when I arrived for shift, he was polishing off his dinner and pot of tea. Sachin acknowledged me with a fist bump and a smile. Once Sachin had finished his meal we went to the toilet where he didn’t produce urine, we will get back to this later, we moved on to brushing teeth. Sachin relieved a good clean tonight, approx 5 minutes worth, he had a go himself after I did the bulk clean. Sachin and I made up our cups of tea, working as a team Sachin acquired the ingredients as I pointed them out to him. We then went to the programs room for jigsaw time. Sachin appeared to go well for the first few minutes, he then displayed a dazed look at times, staring off into nothing and shakey hands. Sachin has had a high bpm since entering the room, 100 to 110 at times. With Sachin showing warning signs I applied lavender oil to his chin to breath up his nose and sprayed lavender into the pedestal fan to disperse into the room. At 1820 I called a break and encouraged sachin to use the toilet, he failed to produce again. When we returned into the programs room I applied more lavender oil to his chin to breath up and sprayed some more into the pedestal fan. At approximately 1830 hours Sachins bpm lowered to 80’s 90’s, a good start. Despite his dazing moments, fluctuating heart rate and shakes Sachin managed to complete an astonishing amount of the puzzle tonight. At 1945 hours Sachin and I went for a walk, a short walk to the end of the street and back, approx 300 meters. Sachin was shakey on his feet. Once we got home it was straight to the couch with a cup of sleep tea and a snack of doritos. Sachin has appeared to have a good night despite the shakes, bpm and dazing moments. ACTIVITIES. Jigsaw. Fishing. WEATHER: 29° sunny day. NOTES: Sachin was inside the house when I arrived for shift, he was couching it up. Sachin had split sleep, but enough sleep so I kept a closer eye on him anyway. Sachin was encouraged to use the toilet, he produced a large urine. I assisted Sachin brush his teeth then we acquired our cups of tea and entered the programs room. Sachin has completed a bit more since my last shift with him, he has come to a point where he is stuck, there is a few similar pieces that fit into the wrong spots. Sachin appeared to be taking the challenge of trial and error with the pieces to find where they fit correctly well, his bpm stayed constant and mid 70’s throughout. At 1930 hours we packed up and rinsed our cups out, we then left his house to Bowna boat ramp. Sachin and I spent a good half hour casting and reeling, I cast the lures out then pass over the rod to Sachin who in turn reels in. Sachin is becoming much more fluent every time we come fishing, he is able to take hold of the rod in the correct hand, reel in at a decent pace to avoid snags and hands to me when he is done. At 2030 we returned home to another toilet break for Sachin , large output, then a sleep tea and snack on the couch. 10hr 43min sleep Sunny top of 28• Today I arrived at Sachins house at 17:06. Sachin was in the kitchen seated at the table drinking some tea. After Sachin had finished up he helped prev put away some cutlery into the draws once Sachin had completed this we set off to the bathroom to brush his teeth. Once we had done this task we grabbed our teas made by prev and then headed our way into the puzzle room at approximately 17:30. Once we were in there I sprayed the room with lavender and applied oils to the inner walls of sachins nose. I switched on the lights and defuser and music. Sachin was doing his puzzles and not gesturing for me to help him. During this time Sachin was exploring new ways to place the pieces together during this Sachin was showing excitement by closely analysing his pieces and laughing followed by flicking it down. Sachins heart rate is sitting quite high (95-100bpm) at approximately 17:55 Sachin began to stare off into the distance becoming very disoriented I called out to Sachin to no signs of response or acknowledgment to hearing me I did this about 3 times sachins eyes began to flicker and roll to the back of his head I again called out to Sachin to no response I then lightly touched Sachin on the right forearm and said his name once more in a firmer voice Sachin came to it with a body jolt he started to respond. At approximately 18:40 Sachin was encouraged to use the toilet he had a very small urine output. At 18:44 we entered back into the puzzle room then approximately 18:50 Sachin is visibly off. Gazing into the open again he also began to rock back and forth and being once again unresponsive. After several attempts for Sachin to come back into focus he suddenly became very active but continued to rock back and forth with occasionally appearing disoriented. His heart rate is sitting at 82bpm at 19:10 Sachin has begun to appear his normal self with complete focus and completing a large portion of his puzzle he also became vocal and happy. We got stuck on a few pieces that were placed in the wrong place saj came In and adjusted this. At approximately 19:54 Sachin and I packed down the room and headed to the kitchen to clean our tea cups then came to find our place on the couch to watch his iPad. At 20:20 Sachin had a small snack some potato chips. At approximately 20:37 sachins heart rate is quite high for sitting and resting 83bpm this is unusual for Sachin as it usually sits between 60-65 bpm. At 20:44 Sachin hopped up to play with the toilet roll and then moved into the laundry to adjust Sajs clothing and observed holes in the teeshirt. 20:46 Sachin has started using a new sound the sound closely resembles the word “dog” in Hindi. He then came to sit back on the couch and was handed a tea Sachin drank this within a matter of seconds about “40 seconds “. 9pm clicked around and I said my fairwell to Sachin and the gang |
