Sachin was on the couch when I arrived, he appeared calm and content. I helped Praveena with preparing dinner and then Sach and I ate together, he appeared to enjoy his chicken a lot. We then cleaned up, I prompted Sach to toilet (small output) and cleaned his teeth. We then made teas and headed to programmes room. Sach immediately settled with a bpm between 70-80. He did appear to be struggling to find pieces as we needed to have all the sorted boxes available for him to use. At approx. 1806 Sachin became zoned out. I attempted to gain his attempting to no avail. I called to Praveena, her and Saj came and assisted with keeping him from falling from his chair. He seized for approx. 25 seconds before we lifted him to ground and placed him in the recovery position. Sachin was unconscious for only 30-45 seconds before becoming aware again. We sat with him for 10 mins before he motioned that he wanted to get up. We assisted him to couch - he appeared quite alert and his bom was mid 80’s. We relaxed for an hour before going and sitting in the garage with the family. Sachin appeared to be acting normally, and was snacking and looked fully alert. His bom was between 70-90.

WEATHER:
36° Sunny.
 
NOTES:
Sachin and his mother were waiting inside their car when I arrived, I accompanied them to a Dr appointment for Sachin. Sachin appeared nervous at the Dr but he behaved. After that we went to spc warehouse and Chinese restaurant. When we returned home Sachin and I started our nightly routine. Sachin used the toilet for a large urine output them he had his dinner. After dinner I assisted Sachin brush his teeth. Together we made cups of tea and went to the programs room. Sachin a little while to get in the flow but once he did he had no trouble finding pieces. At 1930 hours Sachin and I exited the room, Sachin used the toilet again, medium urine output. Sachin and I once again made a cup of tea and sat on the couch with his mother and watching TV, x files.
Sachin has had a good night with no issues or concerns to report, he has had plenty of sleep and has had a good current and resting bpm all shift.
 
Shift ceased at 2130 hours.

Sachin was sitting on the couch with Praveena, he appeared relaxed and immediately greeted me. We all sat together for a short while before we ate dinner together. Sach got impatient while I was doing the dishes and went into programmed room and started puzzle. I had to prompt him a couple times to get up and go to the toilet (small). I then cleaned his teeth and we made teas together, then headed to programmes room. Sach settled in immediately, bpm in mid-high 70’s. We finished up at 1930 and got prepped for a short walk. We went to the end of the street and back, Sach appeared to enjoy it. Once back, he wanted to go back to do more of the puzzle. We went back in for another 10 mins before I prompted him to go to the couch. I got him a tea and snack and we sat and relaxed. At 2045, I promoted him to toilet (med/large). Sach’s bpm was great the whole evening.

Family Night

Sach was just getting out of the shower when I came inside from checking the pumpkins for Saj. He appeared calm and was relaxed on the couch. He drank his arvo tea and had breakfast while on the couch. I assisted Praveena in the kitchen during this. I helped him get his dinner ready, and prompted him to the table - he came across very excitedly, giggling and jumping. We ate together and cleaned up together. Sach wanted to go to the programme’s room immediately again, and it took a fair bit of prompting to get him to the toilet (no go) and then bathroom to clean teeth. We then made teas and headed to programmes room. Sach finished off the puzzle we were working on and then did 3/4 of the edges on the new puzzle. His heart rate was in the 90s for most of the puzzle time - but settled into 80’s for the last half hour. At 1930, I prompted Sach to finish up, he took a while but eventually got up. I prompted him to toilet (medium) and then we went to sit in shed with the rest of family to look at photos and videos of their holiday. Sach was relaxed and smiling for the evening, with a bpm predominantly in the 80’s.

Drunk legs sachin.

NOTES:
Sachin was inside the house, watching tv with his mother when I arrived. I went for a 10 minute drive to woolworths then when I returned i sat with Sachin. Sachin appeared well rested, he was orientated to people and his surroundings. Sachin had his dinner shortly afterwards, when he stood and tried to walk from the couch to the kitchen table he wobbled and stumbled all the way requiring constant support to stay up rite. He appeared to be intoxicated like a person drinking alcohol, but Sachin hasn’t been drinking, his mother did state he has had a cold and flu tablets, drowsy kind, thus would explain it.
Sachin had his dinner, his fine motor skills with his hands and eyes have not been affected, just his legs and balance. I encouraged to use the toilet Sachin no go. I assisted Sachin brush his teeth. Sachin was still wobbly on his feet so I encouraged him to sit on the lounge while I made teas and cleaned up. His mother ended up making Sachin and I a ginger chai tea. We sat in the lounge till 6 pm sipping these. At 6 pm we moved to the jigsaw room and done some puzzle, Sachin went OK, not his best effort.
At 1940 hours we packed up, washed our cups and Sachin had a toilet break (large output ). Sachin sat contently in his lounge chair for the remainder of the evening drinking a cup of tea and watching X files with his mother. Nil issues or incidents to report.
 
Shift ceased at 2130 hours.

Not enough sleep.

NOTES:
Sachin was inside the house drinking his pot of tea in the lounge when I arrived. Sachin had minimal sleep and a high resting bpm, his current bpm was concerning high as well. Sachins current bpm consistently sat in triple digits ( not above 110 ) until 1930 hours. At 1930 hours it was couch time with his mother and he generally relaxes and settles down. Sachin skipped his toilet break after dinner, I assisted Sachin brush his teeth. We make cups of tea together and done some of his jigsaw puzzle, Sachin wasn’t into it tonight, he did put a few pieces together but I could tell he wasn’t into it by lack of attention to his details.
1930 hours we packed up and washed our cups, Sachin was encouraged to use the toilet but he didn’t go, he did however, go at 2000 hours. Sachin and I sat in the lounge with his mother watching X files, and sure enough, his bpm did come down and settled roughly around 85bpm.
Sachin was constantly monitored all night, nil incidents to report.
 
Shift ceased at 2130 hours.

A Reunion

Sach was sitting on the couch when I arrived, he appeared relaxed and was smiling. We sat there together while Praveena prepared dinner. I assisted him with preparing his tea for tomorrow, and then prompted him to the dining table. We ate our dinner together and then cleaned up together. He immediately wanted to go to his puzzle, and headed to the programme’s room. It took quite a bit of prompting to get him up to go to the toilet, when he eventually went, it was a small amount. I helped his teeth and then we prepped our teas together. He seemed slightly slower to react to instructions. Regardless, we made our teas and headed to the programme’s room with no other issues. His heart rate was not ideal for most of puzzle time, but he was focused and didn’t require me to bring his attention back to the puzzle at all. He got a few rows completed, we finished at the regular time and went to the lounge. Before he sat down, he went to the laundry to organise the clothes in there. He sat down after this, I got him a snack (which he refused to eat unless I handed him each individual chip) and his tea. His bpm settled slightly, but was still in high 80’s to 90’s. I prompted him to the toilet at 2045, he got up and went with no issues (large output). His heart rate stayed between 85-92 for the rest of the evening. We kept a close eye on him, but no issues apart from bpm.
