Seizure risk level:- Risk level 1. Previous night Sleep:- 7.30 hrs. Resting heart rate:- 62 bpm. Activities: Jigsaw puzzles in programs. Walking. Weather: 25 degrees, Sunny. Mood: Sachin appeared well rested and happy today, he was using facial expressions a lot, as well as verbal noises. Sachin did laugh at times and get short giggle fits. Sachin wanted to be in control of his jigsaw, his bpm did spike at times to high 80's, I believe he is pushing his cognitive limits with all the pieces on the board, but he won’t let us take some off. Notes: Start of Shift Sachin was ushered to the toilet, he produced large urine content, I then assisted Sachin by brushing his teeth. We started out for an evening walk at 1713 hours with 725 steps already on his fitbit, we stopped at 1751 hours and 4710 steps, 3985 steps for the 38 minute walk. Sachin had 15 minutes on his exercise wobble board when we returned to the house. Sachin and I took our cups of tea his mother made us and went into programs room for Jigsaw puzzle time. Sachin is managing to piece the puzzle together at a reasonable pace, despite having all the pieces in a pile in the middle of his puzzle. Judging by his bpm being high 70's spiking high 80's he is pushing his limits. We finished up at 1935 hours and came out to the lounge, a cup of sleep tea and relax on the couch. Seizure risk level:- Risk level 2 Previous night Sleep:- 7.45 split hrs. Resting heart rate:- 62 bpm. Activities: Jigsaw puzzles in programs. Walking. Weather: 24 degrees, mostly Sunny. Mood: Sachin was in control today when i arrived for shift, he was putting away washing up and getting his tea pot ready, he was not willing to cease his duties and commence activities with me until he had these things done. He appeared well rested and happy today. Notes: Sachin was assisting mum and nan with the washing up when I arrived, once he finished he used the toilet (large urine) I then assisted Sachin brushing his teeth. We started out for an evening walk at 1718 hours with 415 steps already on his fitbit, we stopped at 1749 hours and 3651 steps, 3236 steps for the 31 minute walk. Sachin and I got cups of tea and went into programs room for Jigsaw puzzle time. Sachin is still persisting with his tactic of spreading all the pieces over the board, it still looks like a huge mess to me but in the past 6 days he has managed to place a lot of pieces together. We finished up at 1935 hours and came out to the lounge room. Sachin received a coconut oil head massage from his nan. Weather: 20 degrees, misty drizzle showers. Mood: Sachin was a bit vocal during his evening walk, giggling and vocal sounds occasionally. He appeared well rested and happy today, when I arrived for shift I found him in his programs room with a cheeky grin. Notes: Sachin used the toilet at 1700 hours, we had a break in the afternoon rain so we got straight out into the street for a walk, we did the streets close to the house so we could quickly seek refuge from rain if needed. We started at 1711 hours with 476 steps already on his fitbit, we stopped at 1741 hours and 3705 steps. 3229 steps total while holding umbrella in the misty rain, not bad effort from Sachin. Sachin then did 15 minutes on his exercise wobble board. Brushed teeth and into programs room with a cup of tea for Jigsaw puzzle time. Sachin is displaying a new tactic for solving his puzzles, before he would have pieces in boxes next to his puzzle, now he is emptying all the pieces out into his puzzle and organising them inside. It looks like chaos, his father and I are letting him go to see what happens and how he solves it. We finished up at 1930 hours and came out to the lounge room. Seizure risk level:- Risk level maximum. Previous night Sleep:- 5.49 hrs. Resting heart rate:- 61 bpm. Activities: Jigsaw puzzles in programs. Exercise wobble board. Weather: 22 degrees, showers. Mood: Sachin has had minimal sleep and is in the maximum range, even so Sachin behaviour, attitude and mood were good and behaved. Notes: Sachin has had minimal sleep and is in the maximum range, extra medications and relax aids tonight. Sachin had a visit from his OT tonight, she observed Sachin and myself interacting and doing puzzles. Sachin used the toilet at start of shift, he was then assisted with brushing his teeth. We went to the programs room and worked on his puzzle, Sachin managed a lot of pieces tonight, I believe he is no in the hard part of his puzzle, the next few days will tell. Medication at 1730, 1830 and 1930 tonight. Sachin had his usual calming aids as well as lavender rubbed under his nose. Sachin appeared relaxed and content tonight, the lack of sleep has not appeared to affected his attention to his puzzle or details. Finishing up in the programs room at 1940 hours, Sachin then had 6 minutes on his exercise wobble board. A cup of chamomile and sleep tea and a head massage from Nan, Sachin was relaxed during and after these. Shift ceased at 2100 hours. Support worker:- James.

Weather: 27 degrees, Mostly cloudy day. Mood: Sachin appears relaxed, well slept today. Sachin was in a good relaxed mood, he has shown no adverse affects to being taken out of his normal nightly routine and taken to Bowna boat ramp. Notes: At start of shift I tried to get Sachin to use the toilet, again Sachin did not want to go to the toilet first thing, I assisted Sachin Brushing his teeth, and then tried the toilet again, Sachin did not want to go. Saj Sachin and myself went for a car ride to Bowna boat ramp for our evening walk today, Sachin took the car ride there well, there was no expressions of concern on his face . We walked along the lakes edge for ten minutes, stopping occasionally for Sachin to watch the sea gulls. We returned along the water edge and watched people load their boats onto trailers, Sachin did not appear that interested in them. We packed up and left foe home, at the start of Cambridge drive Sachin and I got out and walked the rest of the way home. When we returned home Sachin used the toilet, OB and urine, his mother assisted him. I assisted Sachin get his cup of tea ready and then went to the programs room. Sachin immediately set to work on his puzzle, he was excited because today he would finish it, and he did, half hour into it. Sachin has now commenced working on a new scenic puzzle called Procida Island, Italy. Very colourful and should challenge him. Medications at 1830 hours. At 1930 we finished in the programs room and went to the kitchen, I assisted Sachin make his sleep tea as well as a cup for Nanna.
