Sachins go slow night.

Previous night Sleep:- 7:15 hours sleep.
Resting heart rate:- 63 resting bpm.
 
ACTIVITIES.
Jigsaw.
Walk.
 
WEATHER:
23 degrees, sunny day.
 
Mood:
Sachin was quiet and appeared to silently contemplate his actions tonight, he would take extra time to understand requests then proceed to do them at his own slow steady pace. He was well behaved and showed no negative behaviours.
 
NOTES:
Sachin was once again in his programs room getting a head start with his puzzle, when he seen me pick up the flash cards he stood up and headed towards the toilet.
Large urine output then he was assisted with brushing teeth.
We then got ready to go for a small walk to the local park for a swing.
Sachin had approximately 13 minutes on the swings, I pushed him back and forth for a while.
We walked home and Sachin again was encouraged to use the toilet, small urine output.
Sachin and I made our cups of tea and went back into the programs room.
Sachin appeared content to go at his steady pace, he appeared stuck a few times and I assisted him with a few pieces, he didn’t appear bothered that I helped.
Sachin had his 1830 medications, dispensed by his father.
Sachin started to really flow with his puzzle after meds he completed a whole line and appeared very happy with himself when it was time to leave.
At 1930 hours we packed up in the programs room and went to the lounge and watched TV.
Shift ceased at 2100 hours.

ACTIVITIES.
Jigsaw.
Walk.
 
WEATHER:
28 degrees, sunny day.
 
Mood:
Sachin was content with our nightly routine he flowed along really well and did not argue or go slow. Sachin did not display any concerning behaviours. He appeared relaxed.
 
NOTES:
Sachin was already in his programs room when I arrived, he was engaged in his puzzle.
I picked up the flash cards and indicated to the toilet one, Sachin appeared to think for a second then stood up and made his way to the toilet, large urine output.
Sachin was assisted with brushing his teeth, and after I got the bulk done he had a go himself.
Sachin and I then set off for an afternoon walk along Tabletop road, Sachin appeared interested and kept a good walking pace with me.
I put on a few Ed Sherran songs for us to walk to, Sachin appeared to not mind.
We returned to the house after a 36 minute walk, Sachin once again was encouraged to use the toilet, medium urine output.
Sachin and I made cups of tea, Sachin assisted me by getting what we needed then by putting them away.
We went to the programs room and commenced work on his puzzle, Sachin appeared stuck a few times and accepted help with no concerns.
Sachin had his 1830 medications, dispensed by his father.
Sachin did not make much progress tonight , he only managed a few pieces before losing interest, he was not willing to be redirected or to engage again with it.
At 1900 hours we packed up in the programs room and went to the lounge and watched TV.
Shift ceased at 2000 hours.

Resting heart rate:- 61 resting bpm.
 
ACTIVITIES.
Jigsaw.
Walk.
 
WEATHER:
28 degrees, sunny day, slight smoke overcast.
 
Mood:
Sachin appeared happy and content, he looked well rested.
He did make a few vocal sounds but nothing that registered as alarming to me. Sachin did not display any concerning behaviours.
 
NOTES:
When I arrived I found Sachin already in his programs room hooking into his Jigsaw, getting an early start.
I showed him the flash cards, the toilet one, he sewn it and got straight up and went to the toilet. We then brushed teeth, Sachin had free time and brushed l
A little himself.
Sachin and I then made our cuppas and headed back into the programs room.
Sachin appeared to be struggling a bit at first, he had a bpm of 95.
I decided to work fast and colour co ordinate his boxes better so it was a bit easier for him, for 15 minutes Sachin bpm upped to 100 – 105 but after wards tapered off to quickly to mid 80’s.
Sachin was definitely flowing better now, the pieces were flying into the right spots and he looked calm.
Sachin had his 1830 medications, dispensed by his father.
At 1850 Sachin was encouraged to use the toilet again, medium urine produced.
Sachin and I went for a walk along Tabletop road, Sachin kept good pace with me  he appeared happy and to enjoy his stroll.
When we got back home Sachin and I couched it watching TV.

First Solo Shift

ACTIVITIES.

Jigsaw.

 

 WEATHER:

27 degrees, Sunny.

 

Mood:

Sachin appeared to be content with rolling along with our night routine, lack of sleep made him a little high in the current heart rates, he seems to elevate a little when he gets stuck.

Heart rate generally fluctuated between 80-90bpm.


 NOTES:

Sachin was sitting at the dining table just finishing his dinner on my arrival. Sachin mother then assisted him with a shower. Sachin then went into the programs room to work on jigsaw puzzle. Sachin then brushed his teeth with assistance from his father Saj. Sachin then used the toilet medium urine output. Sachin then returned to the programs and continued his jigsaw puzzle. Sachin took his 18:30 medication without issue. Lavender oil put under Sachins nose each hour. Saj made Sachin another tea at 18:45, Saj instructed Sachin to wait in programs whilst tea was made, Sachin however wondered back into the kitchen area looking a bit confused but was again directed back into the programs room. Sachin an one stage got stuck on a peice so began working on a new row. Sachin continued working on his puzzle until 19:40 when he used the toilet again low urine output. Sachin then organised his sleepy tea then went over and sat on the couch and played on his Ipad, Sachin remained there until end of shift at 20:00.

ACTIVITIES.
Jigsaw.
Drive.
Art class.
 
WEATHER:
25 degrees, sunny day.
 
Mood:
Sachin appeared sleepy and or confused at start of shift, praveena informed me he had just woke up.
Sachin did wake up fully but quite often through the evening he had a confused look. Sachin appeared content to follow nightly routine.
 
 
NOTES:
Sachin was brought to his Art class by his mother at 1500 hours today, we went inside the studio and met his worker Janine.
Janine worked well with Sachin, she engaged him well and got him to draw and paint with minimal effort.
Sachin completed 3 paintings with her help then It was te to go home.
When we got to the carpark, Sachin was confused, it appeared he was looking for his mother and her car, he wasn’t interested in riding home in my car.
We walked the carpark looking for her car, and when he was satisfied she wasn’t there he did get in my car.
I took Sachin home, he was alright during the car ride, he didn’t show any signs of distress.
Sachin had his tea and a shower, he was assisted by his mother in these.
I then brushed his teeth and with his help made cups of tea.
We went to the programs room and started work on his puzzle again.
Sachin managed a few pieces in to night, he seemed to struggle with focusing and got distracted a lot.
Sachin had his medication at 1830 hours, dispensed by his father.
 At 1836 hours Sachin appeared to have a micro blackout , he stared at the wall and his eyelids fluttered, this lasted a few seconds, he snapped out of it when I touched his,arm.
At 1845 hours we called it quits in the programs,room, we went to the kitchen and made another cup of tea and sat in the lounge with family.
Sachins BPM continued to drop while we watched TV, by the end of shift he averaged 65.

ACTIVITIES.

WEATHER:
25 degrees, sunny day.
 
Mood:
Sachin appeared to be in a good mood, he was compliant with keeping routine on time and did not argue or go slow.
Sachin was quiet but full alert with his surroundings.
Sachin was bluntly, cool, calm and easy going.
 
 
NOTES:
Sachin was in the lounge when I arrived, he was happy and gave me a fist bump.
Sachin was encouraged to use the toilet, he produced a large urine, and after that he was assisted with brushing his teeth.
Sachin and I then made our cups of tea, I gave instructions by way of pointing at things we needed and he’d go get them, team work, then he would put them away.
We went to his programs room where we continued his puzzle.
Sachins high BPM was of concern so he had lavender oil put under his nose to assist the diffuser with his calming
Sachin had his medication at 1830 hours, dispensed by his father.
At approximately 6:37 pm Sachin began to go into a seizure, Sajit was called into the room by myself for assistance.
Sajit and myself lowered Sachin to the floor from his chair and into the recovery position, Sachin seized for approximately 90 seconds then stopped, sometime during the 90 seconds he was incontinent, wetting his pants.
Sachin came out of it enough and wanted to stand up, he was unsteady on his feet so praveena and i assisted him to his feet and supported him to the toilet where he had a small urine and his cloths changed.
Sachin was then assisted to the lounge where we rested, his BPM was still very high around 120’s
Sachin attempted to get to his feet a few times, we intervened and stopped him
It took 45 minutes for sachins BPM to drop down to average of 75.
By 8pm Sachin had calmed right down and appeared back to normal, BPM average of 70.
Shift ceased at 2000 hours.

WEATHER:
32 degrees, sunny.
 
Mood:
Sachin appeared to be well rested, he was energetic and vocal.
He appeared in a good mood tonight, he was content with our routine.
 
 
NOTES:
Sachin was in the kitchen assisting his mother with drying the cutlery and putting some away when I arrived.
Sachin was encouraged to use the toilet, he produced a large urine content, I then assisted him brush his teeth.
Sachin and I made up our cups of tea and went to the programs room.
Sachin appeared happy doing his jigsaw tonight, he displayed good focus to it, he did not get stuck too many times and also let me assist him.
Sachin had his medication at 1830 hours, dispensed by his father.
Sachin continued his jigsaw until 1850 hours, he then used the toilet again, large urine.
Sachin and I then went for a half hour walk along Cambridge drive, Sachin appeared happy to walk he kept pace with me well.
Sachin and I returned to his house, he used the toilet once more, small urine, then sat up on the couch with Nan and his mother.
No behaviours or incidents to report.
Shift ceased at 2000 hours.

Missing puzzle piece found

Toileted before puzzle time. Medium urine output. 57BPM RHR. 
Activities: 
Sorted weekly medications, 35minutes. Puzzle time. 


Progress notes: 
Greeted Sachin’s family at 1700. Sachin was eating his dinner and drinking tea when I arrived. We sorted through the medications for the week. I assisted Sachin with brushing his teeth and he used the toilet producing a medium urine output. We grabbed our cups of tea and headed into the puzzle from to continue the new puzzle that was started yesterday. Sachin and I found a random edge piece floating in the box. The edge was already completed. Sachins HR then jumped into the high 90’s because we both couldn’t figure out where the piece went. Saj even tried as well with no luck. Approx. 5 minutes later Sachin found where the piece went and was quite chuffed with himself and had a grin on his face. Immediately Sachin’s HR dropped back to the mid 70’s-80’s. From then on Sachin began to flow really well with the puzzle. 1830 meds given. At 1930 we headed into the family room for some iPad time and relaxing on the couch. Sachin’s heart rate went down to 65 whilst sitting with Praveena on the couch. He appeared to be very relaxed watching Batman on the iPad. Shift ended at 2000.

Blackout this afternoon

Continued to work on Lego stadium and also started a new puzzle. 

• Mood:
Tired but happy.

28c Showers.

• Notes:
1500, Helped scan legal documents and also family photos
1620, Sachin woke up for the day and had his dinner, lasagna.
1638, Sachin had his morning pot of tea.
1722, Sajittook Sachin to the toilet.
1725, Moved to activities room and continued on Lego stadium and has made good progress, started on page 38 and got to page 48. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every half an hour to help relaxation and reduce heart rate. 
Blackout at 1816, spent 10 minutes on the floor and then was back in his chair by 1827. Recovered well.
1830, Praveena gave Sachin his regular medication with a large cup of cordial.
We swapped to a puzzle to ease stress.
2000, Shower and toilet by Praveena.
2030, Family room for sleep tea and videos on the iPad.
