Sach was just getting out of the shower when I greeted him today. Be sat down at the table and patiently waited for his dinner. He appeared visibly excited for dinner and smashed it down. We then cleaned up together and made teas, Sachin was focused and not showing signs of fatigue or lack of sleep. We then cleaned teeth and he went to the toilet (small output). We then grabbed our preprepared teas and went to the programme’s room. Sach settled in immediately and was getting pieces in very quickly. His heart rate was up and down, he occasionally needed reminders to focus, but he got nearly 3 rows completed. We finished up and I prompted Sach to the toilet (medium output). He then sat for a head massage from Praveena. He began vocalising very loudly and very frequently during this, which continued for the rest of the evening. Despite this his heart rate was good (between 65-75 bpm) and no issues.

Sach was drinking his afternoon tea when I greeted him this afternoon. When he was done, I assisted him in preparing it for tomorrow. After this, Sach and I ate dinner together before cleaning up. I then prompted him to the toilet (no go), before helping him clean his teeth. We made teas and headed to the programme’s room. Sach got right into the puzzle and settled to mid 80s bpm. He got a lot done and was alert the entire time. We finished up at approx. 1920 and I put his runners on. We only walked to the end of the street, but he was in high spirits, laughing and smiling. We got back and Sachin puttered around for a short while before settling on the couch with his sleep tea and some corn chips. It took a lot of prompting but I eventually got him to eat the chips himself. I prompted him to the toilet at 2010 (no go), again at 2045 (large output). He sat back and settled, with a bpm in the 60’s and 70’s. No issues.

Sleepy Start to the Week

Monday 18th March 2024

Sach was in the shower when I walked in, I greeted Praveena and assisted with cleaning while she cooked and helped Sachin get dressed. I helped him make an early chamomile tea to help him settle while he waited for dinner. We sat and ate together before Praveena helped him wash up, he immediately wanted to go sit on the couch with the rest of him tea, but I prompted him to brush his teeth beforehand. We went straight to the couch and Sach finished his tea. Praveena and I then helped Sach sit on the floor so he could have a head massage. This lasted approx. 75 mins and he was relaxed and alert (bpm 60’s). I then prompted him to toilet, he was very slow to go, and very concerned with how the toilet rolls were arranged on the window sill. He eventually went (small output). We then sat back on the couch - Sach had his tea and a snack - and watched x-files. Sach appeared a little tired but was alert and had a bpm steadily in the 60’s. I prompted him to toilet again at approximately 2030 - he went fast this time (med/large output). At 2050, he began falling asleep on the couch so we prompted him to bed a little earlier than usual. No issues overall.

Sach was finishing his arvo tea when I arrived. I helped him prepare the tea for tomorrow and the assisted him in putting his runners on. We went for a 30 min walk along table top rd. Sach was enjoying himself and swinging his arms a lot. We got back and Sach had his dinner. We cleaned up together and I prompted him to the bathroom to clean his teeth. We then made teas and headed to the programme’s room. As Sach was settling into the puzzle, he had a very short blackout (approx. 5 seconds). He snapped out of it as soon as I put my hand on his cheek. He immediately got back into the puzzle and completed a fair amount of it. His bpm was mid 90’s to begin with, and then settled to high 80’s. We finished at 1930, I prompted his to toilet (small/medium output). We then sat with Praveena on the couch with teas and snacks while watching xfiles. His bpm was in the 70’s - no issues.

Notes:
Sachin was just finishing off his dinner when I arrived tonight and appeared quite happy to see me, then was hovering around the kitchen with Preveena prompted Sachin to use the toilet but not sure if he went as he'd closed the door, teeth brushed when finished then out to the kitchen to grab our cups of tea again Sachin was hovering near the bin before wanting to head into program room his hr was sitting at 120bpm at start and steadily began coming down as he got into a rhyth.Sachin was up to his old tricks again wanting staff to pop the pieces in but kept taking them out until they made the right sound which was becoming a little frustrating,Sachins hr was hovering around 79bpm and 85bm for most of the session,Sachin was making great progress tonight but was making sure I was the one popping all the pieces in and doing it properly but eventually ended up completing two whole rows. Took several prompts to get Sachin to finish up, straight away got him to use the toilet using sign very large output then relaxing on the couch hr was sitting at 90bpm and slowly making its way down to mid 70's little bit fidgety at times, Preveena grabbed some corn chips for Sachin and his sleep tea which he thoroughly enjoyed.Very cheeky tonight which was great to see.

NOTES:
Sachin was drinking his pot of tea when I arrived, I sat with him until he finished. We done some puzzles till 5, then he had his dinner. I encouraged sachin to use the toilet, he didnt go. I assisted Sachin brush his teeth. Sachin and I made cups of tea and went back to puzzles, Sachin was stuck so I helped him find it, 15 minutes it took us to find the piece, the length of time to find may have agitated him. At 6:20 Sachin dazed off, I called to saj to assist. Saj and praveena attended and together we assisted Sachin avoid a seizure. We moved to the couch once Sachin felt better, at 730 sachin used the toilet, large urine output. Sachin had a sleep tea and a bowl of dorito chips.
Sachin appeared anxious when I started shift, his bpm was high, triple digits. Sachin skipped routines and forgot little things like routine of making tea, couple this with the stress of being stuck in his jigsaw I believe assisted with his black out. Once all was done & he was resting on the couch his BPM dropped to low 80’s and appeared relaxed and content.
 
Shift ceased at 2130 hours.

Sach and Praveena got home approx. 1615, Sach immediately sat down for dinner - KFC wicked wings - his favourite. He dug into this and thoroughly finished them off. Once done, we cleaned up and I prompted him to the toilet (no go after standing there for a while). I then helped him clean his teeth and we made teas. I prompted him to couch to change his slippers, but he wanted to change his entire outfit, so I assisted him in this. We then grabbed our teas and headed to the programme’s room. Sach settled in and was immediately focused, getting pieces in quickly despite his heart rate being in the high 90’s. I prompted him to toilet at approximately 1845 (no go again). West got back into the puzzle until 1930 and then finished up. Sach had a wander around the house for a while, and refused to go to the toilet again before settling on the couch. I gave him his sleep tea and some corn chips. Sach finally went to the toilet at 2050 (large). He was settled for the rest of the evening with a bpm in the 60’s and 70’s.

NOTES:
Sachin hadn’t been long awake when I arrived, shortly after that I assisted his father and mother fry rice for dinner. Sachin had his share and appeared to enjoy it, I encouraged him to use the toilet after, he didn’t need to go. I assisted Sachin brush his teeth, we then made cups of tea, we took them into the programs room. Sachin worked well, no issues with finding pieces. At 7:15 pm I encouraged a toilet break from Sachin, he let go a medium amount of urine. We went for a walk around the block, 8 minutes and 1023. From 7:30 to 8 pm we sat in the lounge with his mother watching XFiles. Sachins current bpm has been high this evening, he has been high 90’s constantly.
At 8pm we joined his mother and father in the garage for family time.
 
Shift ceased at 2130 hours.

NOTES:
Sachin had just woken up when I arrived, his mother showered him and assisted getting him dressed. Once his shower was done he had dinner. Sachin had fried rice, he ate up quick like a starving kid, he appeared to really like it. With dinner out of the way Sachin was encouraged to use the toilet, he didnt go. I then assisted Sachin brush his teeth and together we made cups of tea. We entered the programs room for puzzle time, so far sachin has been behaved, his attention, dexterity and attitude all excellent but his BPM has been concerning. Sitting at 95 + bpms and at times nudging 103, Sachin should have a bpm like this if he he doing physical activities like walking fast but he’s been doing nothing. I’ve encouraged a calming environment of quiet, soothing music, lavender for him to smell, dim lights and NO energetic behaviour from me that might excite him or stimulate him.
After approximately 40 minutes in the programs room Sachin settled down a bit and his bpm went down slightly, mid to low 90’s.
Sachins bpm stayed in this range until 7:30 pm when we settled on the couch. At 730 Sachin had a toilet break, large output. At 8pm Sachin and I made cups of tea and put a few recyclable things into the recycling bin. Sachin was content on the couch watching X files and snacking on corn chips, his bpm dropped into 75 to 85 bpm range
 
Shift ceased at 2130 hours.

Sachin was finishing off his afternoon tea at the dining table when I arrived. Once done, I helped him prep it for tomorrow and then he headed to have a shower (assisted by Praveena). I then prompted Sachin to come sit in the couch with Praveena showered. He wanted to wait on the bed so it took a lot of prompting to get him to come out, and once he did, he had his fingers in his ears to express that he wasn’t happy about it. We sat on the couch for approximately 10 mins. The fingers didn’t come out of his ears until he was prompted to the table and his dinner was put in front of him. Afterwards, we cleaned up together and I helped Sach clean his teeth. West then made teas and settled into puzzle time. Sach had no issues with finding pieces - but was very irritated when I told him to change the direction that he was putting the pieces in. It took him quite a while to get used to it. We finished up at 1930 and Sach went to the toilet without prompting (large output). We then went for a walk, Sach seemed keen to go further but we didn’t want to push him too far. West got back at 2000 and Sach sat on the couch while I made a sleep tea and snack for him. We relaxed for the rest of the evening with no issues. Heart rate between 80-90 bpm all evening.
