Notes :
Sachin a little distracted when I arrived for shift required several promts to use the toilet, medium amount of urine passed then went for a good walk up Cambridge Dr twice lots of vocalising and some heavy breathing Sachin kept a really good pace completing 2.44km in 28 minutes gave me some assistance to put together a bedside table and enjoyed using the drill lots of smiles throughout then into program room to work on puzzles Lavender difuzer put on along with some Lavender oil on his top lip  and soft music playing Sachin making slow but steady progress very calm throughout finished up at 7.40pm making his way into lounge room to relax new cup of tea and sitting on couch at end of shift.

Sachin accepts puzzle assistance!

22 degrees, sunny.
 
Mood:
Sachin appeared normal this afternoon, he was a little vocal on our evening walk, he maintained pace with me and was steady on his feet.
Sachin appeared relaxed and went with the flow without objection.
Notes:
Saj informed me Sachin had a split sleep last night, he was out of routine with no support worker or puzzle time yesterday, be cautious and watchful of Sachin tonight.
We left the house at 1710 hours and walked along Tabletop road to the first Bridge only, then back home, arriving at 1726 hours, 16 minutes. Sachin started steps at 588 and finished at 2357, that’s 1769 steps, a short walk today.
Sachin then did 15 minutes on his exercise wobble board.
Sachin then used the toilet for a medium sized urine, I then assisted brush his teeth.
Sachin and I then made our cups of tea, we then did Jigsaw Puzzles in programs room.
Medications given at 1830 hours.
Sachin allowed me to touch the jigsaw tonight, I separated the blue sky pieces from the green tree pieces.
At 1930 hours we came out to the lounge and made a new cup of tea each and one for Nan too, Sachin then settled on the couch under his heated blanket.
Shift ceased at 2000 hours.

20 degrees, sunny.
 
Mood:
Sachin appeared fine and well rested this afternoon, he was relaxed and went with the flow, No visible signs of trauma from his seizure last night.
Notes:
Saj informed me Sachin had a seizure last night at approximately 5:15pm, came out of the blue and lasted for approx 1 minutes, be cautious and vigilant with Sachin this evening.
We left the house at 1711 hours and walked along Tabletop road to second Bridge, a quick around then back home arriving at 1734 hours, 33 minutes. Sachin started steps at 679 and finished at 4662, that’s 3983 steps. Back at house Sachin used his exercise wobble board for 15 minutes, Sachin then used the toilet for a large urine, I then assisted brush his teeth. Sachin and I then made our cups of tea, we then headed to the programs room for Puzzle time.
Medications given at 1830 hours.
Sachin had 3 bottom rows completed on his puzzle, he deconstructed the 3 rows for some reason, perhaps he knew there was a mistake there and went back to fix it, time will tell for this one.
At 1930 hours we came out to the lounge and made a new cup of tea and settled on the couch.
Shift ceased at 2000 hours.

Program room to work on his jigsaw puzzle

Progress notes:
Sachin had just finished eating his tea and was toileted straight away passing quite a large amount of urine ,washed his hands then got ready for his walk which due to a lack of sleep was kept short walking up to the first bridge and back taking 15minutes at an easy pace heart rate avg 89 bpm
Wobble Board 5 minutes ,then brushed his teeth and straight into program room to work on his jigsaw slow but steady pace very calm and relaxed heart rate sitting between 55bpm and 65bpm finished up at 8pm then headed to lounge room to relax on couch .

Sachin very focussed on jigsaw going back into activities room  and began dismantling all the progress he'd made taking puzzle right back to edges eventually came back out sitting on floor getting a  head massage of his Grandmother.

1802, Organised weekly medication with Sachin’s assistance.
1830, Medication administered.
1832, Assisted with brushing Sachin’s teeth.
1838, Moved to activities room to work on new Blue Mountains puzzle. Camomile and green tea, relaxing music in the back ground with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad.
1940, Took Sachin to the toilet.
1948, Family room for sleep tea and youtube videos on the iPad. Lavender oil diffuser on.

Sachins chaos!

Walking.
 Exercise wobble board.
Weather:
18 degrees, sunny.
 
Mood:
Sachin was again in a good placid mood, same as yesterday. He focused well on tasks at hand and showed good concentration to his Jigsaw puzzle.
Notes:
As start of Shift We walked along Tabletop road to second Bridge then returned home, our walk took 30 minutes, completing 3700 steps.
Back at house Sachin used his exercise wobble board for 15 minutes, Sachin then used the toilet for a large urine, I then assisted brush his teeth.
Sachin and I then made our cups of tea, we then headed to the programs room for Puzzle time.
Medications given at 1830 hours.
Sachin appeared disturbed by something with his puzzle, this was ascertained by his current BPM sitting in the high 70's.
Sachin had a good portion of the puzzle pieces jumbled up resting in a pile inside his jigsaw puzzle already but once he emptied the remaining 4 boxes of spare pieces into the pile he appeared calmer, more settled and his Resting BPM sat lower in the 60's.
It appeares Sachin has evolved, changed the way he approaches and completes his puzzles.
A few months ago Sachin was content having his pieces segregated in to storage trays by colour or shape which he would peruse through at his leisure, pick a piece and put it in the right spot, but now all pieces have to be spaced out and spread on the table inside the jigsaws boarder, it simply looks chaotic to me but it could very well be organised to him.
Sachin continued in his programs room until 1940 hours, we then went to the kitchen and made another cup of tea for ourselves and an extra one for Nan, we then sat in the lounge room.

Good Mood

Mood:
Sachin appeared well rested today, he was in a good mood, fairly quiet vocally and focused well on tasks at hand.
Notes:
Toilet break at 1705 hours, Sachin had a big urine.
We then went walking along Cambridge drive for 30 minutes, completing 3544 steps.
Back at house I assisted Sachin brush his teeth, he then did 15 minutes on his exercise wobble board.
Sachin and I then made our cups of tea, we then headed to the programs room.
Medications given at 1830 hours.
Sachin continued his puzzle till 1940 hours, we then packed up and went to the kitchen and made another cup of tea and sat in the lounge room watching TV with Nan.
Shift ceased at 2000 hours.

Notes :
Sachin well rested and calm today and straight away taken to the toilet passing a large amount of urine then washing his hands and out to the patio to get ready for his walk, we walked along table top road completing just on 4km at a very good pace, not very vocal tonight but breathing was a little heavy at times spent just 5 minutes on the wobble Board then assisted with putting a bedside table together and was really concentrating on what was happening worked on this Activity for about 400 minutes packed up and made our way into the program room to work on his jigsaw puzzle Soft music put on Lavender difuzer put on along with some Lavender oil on his top lip very relaxed and calm heart rate sitting between 60 and 70 bpm worked on his jigsaw until 7.50pm then out to lounge room to relax and finish his cup of tea.

Worked on new Blue Mountains puzzle. Walk up half of Cambridge dr. and back, 1234 steps in 10 minutes at a very relaxed pace due to very high risk day and Sach walking a little gingerly on his left foot.

1713, Walk up and back half of Cambridge dr. 1234 steps in 10 minutes at a very relaxed pace.
1728, Assisted with brushing Sachin’s teeth.
1734, Moved to activities room to work on new Blue Mountains puzzle. Camomile and green tea, relaxing music in the back ground with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every 20 minutes. Low light environment.
1830, Extra and regular medication administered.
1930, Extra medication administered due to high risk day.
1945, Took Sachin to the toilet.
1948, Family room for sleep tea and youtube videos on the iPad. Lavender oil diffuser on.

Worked on current puzzle and is nearing completion. Walk up Cambridge dr. and back, 2850 steps in 31 minutes at a steady but relaxed pace Due to Sach walking a little gingerly on his left foot.

A little lethargic and restless, settled to calm and relaxed as the night went on.


• Weather:
20c Mostly sunny.

1743, Sachin went for a shower assisted by his father Sajit.
