9hrs 30 minutes sleep rhr 64 bpm Weather partly cloudy 23° Activities: Short walk up Cambridge drive waling about 1km then back to house Watch Sachin sort weekly medication Program room to work on puzzle Progress Notes: Arrived to start work with Sachin at 5.15pm tonight, Sachin was sitting at the dinner table having a cup of tea brief chat with Sajjitt and Preveena to ask how Sachin had been going I then got Sachin ready to go for a walk o ly going up Cambridge drive and back covering about 1 km then back to house grabbing our cups of tea and heading into the Program room putting on the difuzer and soft music . Sachin was just getting started on a new puzzle and was slow to get going hr at the start was a little high sitting at about 85 to 90 Bpm but as he settled it started to come down quickly to just above 80 bpm,Saj brought in Sachins 6.30pm meds which he took and continued with his puzzle ,Sachin toileted just after 7pm medium output and eventually finished up just after 8pm making his way into the lounge room to relax shift ended at 8.15pm.

Resting heart rate 64 bpm. Risk level 1. On arrival Sachin had showered and had dinner. I hadn’t seen Sachin for 2 weeks but he appeared happy to see me. At 5.30 we went to puzzle with our cups of tea. Sachin has started new puzzle and completed the edges. At 6.30 after placing 8 pieces of second row he was encouraged to use toilet. Medium urine stream. Throughout the course of my stay Sachin was very open to interacting with me. His heart rate was around 85 to 90 bpm a little high. Use of a mist spray did bring it down. At 7.30 pm he received head massage from Praveena after stopping with puzzle completing 2/3 rd of second line. Appeared to relax him and he enjoyed. Another toilet stop medium urine. Bedtime tea and a snack. Sachin was very engaged while in tv room. 8.45pm HR 78 bpm Big smile for me on departure. Support worker:- James.
Seizure risk level:- Risk level- 3 Previous night Sleep:- 5:16 hours sleep. Resting heart rate:- 60 resting bpm. ACTIVITIES. Jigsaw. WEATHER: 17 degrees, Rain. Mood: Sachin appeared to be in a happy mood for shift, he sounded vocally chirpy and happy. Sachin has had a minimum amount of sleep so a calming environment was established and maintained through put the night for him. Relaxing music, lavender diffuser, lavender oil on his chin, low volume voices. NOTES: Sachin was already in his programs room when I arrived doing his Jigsaw. Sachin was encouraged to use the toilet, He produced a Large quality of urine. Sachin was then assisted with brushing his teeth, extra time spent tonight cleaning very good, Sachin had a Go afterwards himself. Sachin was content tonight with a casual speed doing his own thing. 1818 hours Sachin appeared blacked out, staring at the wall, i attempted to rouse Sachin with no success I called his father for assistance, together we lowered him to the floor from his chair and into the recovery position. Sachin appeared to come to, he looked around the room, sachin was encouraged and let to lay there till 1900 hours. Sachins mother dispensed his medications at 1830 hours. At 1900 hours I encouraged Sachin to his feet and to the toilet, Sachin produced a medium amount of urine. We then went back to the programs room and continued puzzles. Sachin mother came into the room with a snack for him, to prepare his stomach for Kava, another step to help him calm. At 1930 hours we packed up and left the programs room and went into the lounge, I handed Sachin his ipad with youtube loaded, he watched his Cartoons. At 1930 hours we finished in the programs room and went to the lounge room to settle and watch some TV. At 2000 hours Sachin began his course of Kava drinks, every 12 minutes or so he would be given approximately 90 mls. Sachin appeared to calm and relax right down . Shift ceased at 2100 hours. Fine and sunny 22 degrees Risk level 1 Resting heart rate 63 bpm Sleep: 9 hours 45 min Mood: Sachin in over the time I was with him was overall good. He was a little anxious at the start of our first short walk solo. He was Co operative with instruction and suggested things to do. Notes: As I arrived Sachin had showered and having lasagna for dinner. Which he ate quickly and enjoyed. After dinner brushing of teeth. he got ready for short walk. I took him alone for the first time and at the start seemed a bit anxious. He enjoyed walk then we played catch. I introduced him to a foam ball. He was very curious about the texture of the ball more than playing catch. Sachin encouraged to go to the toilet (medium urine). Routine preparation of day tea and night tea. Sachin had also folded some tea towels before I arrived. Sachin put cutlery away. Then to the program room. Started on jig saw puzzle. Today we got stuck two times. Saj saved us. I was really happy when Sachin was stuck his heart rate remained steady between 70 to 84 bpm. Sachin was encouraged to go to the toilet every 80 to 90 min thought the time I was there (medium urine). 7.30 pm Sachin a mandarine and a small snack. As I was preparing to leave sachin went to toilet. (Medium stream). I think sachin was in a good mood and had a good day. Support worker: Kate Hargreaves 4pm till 9pm.
Seizure Risk Level: 1 Previous Night Sleep: 9 hours 56 min Resting Heart Rate: 63 BPM Activities: Jigsaw and Connect 4 Weather: 22 Degrees and sunny Mood: Sachin was very good today. He was very focused on the puzzle. He made excellent progress and was willing for me to help him out. He showed great enjoyment in being successful. He is a very determined young man. Notes: On arrival Sachin was having dinner. He had already had a shower and toilet. We went to program room after preparing a cup of tea. HR 91 BPM. By 6pm he had completed ¾ of a line. Sachin was encouraged to go to the toilet (small urine) and brushing of teeth. Sachin prepared another tea and a coffee for me. Back to the program room. I tried to introduce the game connect 4, we didn’t play a real game, but he was happy and wanting to make his own pattern. 6.30 pm he took his medication. Sachin then went back to jigsaw. Lot of pieces fell into place and was feeling very happy with himself and completed 3 lines by 7.10. HR 76BPM Sachin was encouraged to go to toilet (medium urine). Sachin wound down with some Ipad. 8.45 pm Toilet. HR 66 BPM. Support worker: Kate Hargreaves 4pm till 9pm.
Seizure Risk Level: 1 Previous Night Sleep: 11 hours 27 min Resting Heart Rate: 62 BPM Activities: Went to the park. Had some time on the swing and a little walk. Jigsaw Weather: 25 Degrees and sunny Mood: Sachin was in a good mood when I arrived and remained so for the shift. He enjoyed time at the park, and his dinner. There was laughter through the day. Notes: Sachin was hanging out washing when I arrived. Heart rate 86 bpm. He did some jigsaw and then had dinner at 4.30. Routine of toilet (small urine), brushing of teeth. At 4.50pm got ready and went to the park and had a go on the swing and a quick walk, that he seemed to really enjoy. When we got back he folded some washing. Routine of cup of tea and onto the jigsaw Good progress was made completing a whole line. Line 6 is now completed. At times a little distracted but not overly possible because his mum is away. At 7.15 we went to lounge for some Ipad time. 8.15 heart rate at 65bpm. Sachin seemed relaxed and in good mood. When in jigsaw room I encpurged him to identify how he was feeling on the chart. Seizure Risk Level: 1 Previous Night Sleep: 10.25 hours Resting Heart Rate: 63 BPM Activities: Short walk, playing catch and jigsaw Weather: 24 Degrees some cloud but mostly sunny Mood: Sachin was in a good mood when I arrived. He seemed a little nervous on our short walk I think because it was the first time I had walked him. By the end of my time with him he was very engaged with me. I saw lots of smiles he seemed really happy. Notes: At around 5.30pm we had been for a short walk, played catch been to the toilet (small urine), brushed teeth and made a cup of tea before we made our way to the jigsaw puzzle. During the time doing puzzle Sachin’s heart rate fluctuated between 74 bpm and 87 bpm. Heart rate crept up when he was a bit stuck. At times he was willing for me to help. Sachin placed 21 pieces to the puzzle. When Saj was in the room making some funny noises there was a lot of smiles. 6.30pm Sachin took his meds. 7.30pm we went to the lounge and prepared nigh cup of tea. At 8pm resting heart was 64 bpm. I finished my shift with Sachin observing a very happy boy. Lots of smiles and fist pumps. Support worker: Kate Hargreaves
Seizure Risk Level: 1 Previous Night Sleep: 9.25 hours Resting Heart Rate: 68 BPM Activities: Jigsaw Weather: 23 Degrees Cloudy with some showers Mood: Sachin was in a happy mood. This my first solo visit with him and he seemed very accepting of me. While doing jigsaw puzzle he seemed very alert and was quite happy for me to help him and at times we shared many fist pumps. Being only my second visit with him, I was able to identify this mood as the first time I met him he had had a seizure the day before, and appeared tired and more withdrawn. Notes: At roughly 5 pm I sat with Sachin while he ate his dinner. He was asked to the toilet (medium amount of urine) washed hands and then brushed his teeth. We made a cup of tea and the proceeded to jigsaw puzzle room. Sachin turned on light and music and vaporiser on. During this time while doing the puzzle he completed the borders and almost completed the first row. Sachin seemed happy with his progress. At times he was more than happy for me to help. At 5.30 his resting heart rate was at 92bpm and at 6.30 when he had his medication his resting heart rate was 71 bpm. At 7.30 we packed up. Sachin went to the toilet (medium amount of urine). Cup if night tea and a bit of TV. Sachin was well behaved and I enjoyed spending time with him. On leaving at 8pm heart rate was at 68 bpm and I got another fist pump and a smile. Seizure risk level:- Risk level- 1 Previous night Sleep:- 10:17 hours sleep. Resting heart rate:- 63 resting bpm. ACTIVITIES. Jigsaw. Walk. Drive. WEATHER: 21 degrees, overcast day. Mood: Sachin appeared well rested today. He was very much in his own head, he showed little acknowledgement to my presence, but did fist bump me every time I prompted. Sachin was well behaved and showed no negative behaviours. NOTES: Sachin was having a seated at the kitchen table when I arrived. Sachin was encouraged to use the toilet, He produced a small quality of urine. Sachin was then assisted with brushing his teeth, I completed the bulk of the cleaning then Sachin had a small go himself. Sachin and I then went for a walk along Tabletop road, his father said he has had enough sleep so do the full 30 minute walk. Sachin appeared in a good mood, he appeared to not struggle walking so we made a good time on our walk with a fast paced speed. When we returned home we accompanied his father on a car ride to Lavington iga then to Thurgoona woolworths, Sachin and I accompanied his father inside and walked a few isles while he done his shopping. Sachin appeared to cope well with the walk in woolies, he did not appear to show any anxiety towards the large amount of people there. We returned home and Sachin was again encouraged to use the toilet, medium urine produced. Sachin and I then made our cups of tea and went to the programs room. Sachin finished his puzzle the previous night so tonight he started a new one, Sachin managed to put 3/4 of the edges together by the time we had to pack up. At 1930 hours we finished in the programs room and went to the lounge room to settle and watch some TV. Shift ceased at 2000 hours. Support worker:- James.
Seizure risk level:- Risk level- 1 Previous night Sleep:- 8:46 hours sleep. Resting heart rate:- 64 resting bpm. ACTIVITIES. Jigsaw. Walk. WEATHER: 24 degrees, sunny day. Mood: Sachin appeared well rested today. He did show a few assertive moments earlier in the arvo, mainly around making cups of tea and making sure everything was put back the way it is supposed to. Overall Sachin was well behaved and showed no negative behaviours. NOTES: Sachin was once again in his programs room getting a head start with his puzzle, when he seen me pick up the flash cards he stood up and headed towards the toilet. Large urine output then he was assisted with brushing teeth. We then got ready for a evening walk, we went along Tabletop road for 10 minutes then returned to his home street and continued walking for another 5 minutes, 25 all up. Sachin did not struggle, he appeared to walk casually and confidently. Sachin and I made our cups of tea and went back into the programs room. Sachin was very close to finishing his puzzle, he had three lines left to do. Sachin had his 1830 medications, dispensed by his father. Sachin finished his puzzle, he appeared happy afterwards. At 1930 hours we packed up in the programs room and went to the lounge and watched TV with his mother. Shift ceased at 2000 hours. |
