Sleep 6 hrs 39 min rhr 62 bpm Weather Sunny 14° Toileted 5.10 pm large urine output Activities: Walk up Cambridge Dr up and back 14 minutes total 1773 steps Wobble Board 10 minutes Brush teeth Program room to work on puzzles Progress notes: Sachin just finishing up his tea when I arrived tonight appeared pretty relaxed and calm toileted at 5.10 pm large amount of urine then went for a walk up to end of Cambridge and back as didn't want to push Sachin to hard took 14 minutes total wobble Board for 10 minutes then into bathroom to brush his teeth once done we headed into program room to work on his jigsaw very slow going tonight and appeared a little distracted lots of heavy breathing extra medication given at 7pm . Sachin had no interest in staying in the program room and made his way intonthe lounge at 7.20 pm ,toileted again at 7.25 pm large amount of urine output, Sachin then given a nice head massage by his Grandmother finished up shift at 8.30pm.

Walk up half of Cambridge drive and back at a relaxed pace for 10 minutes and then 10 minutes on the treadmill, 1609 steps in total. 1734, Vibration machine for 10 minutes, cut shirt due to heart rate of 120bpm. 1752, Moved to activities room to work on Twelve Apostles puzzle, made great progress unassisted. Camomile and green tea, relaxing music in the back ground with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. 1830, Medication administered. 1930, Took Sachin to the toilet. 1934, Family room for sleep tea and youtube videos on the iPad. Walk up half of Cambridge drive and back at a very relaxed pace, 1442 steps in 17 minutes. Organised weekly medication with Sachin’s assistance. • Mood: Very vocal, a bit distant but cooperative. • Weather: 13c Mostly cloudy.

Camomile and green tea, relaxing music in the back ground with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Low light environment and lavender oil above the upper lip every half an hour. 1830, Regular and extra medication administered. 1942, Took Sachin to the toilet. 1948, Head massage from his grandmother to aid relaxation and promote good sleep for tonight. 2030, Family room for sleep tea and youtube videos on the iPad. Seizure risk level:- Risk level minimal. Previous night Sleep:- 8.27 hrs split. Resting heart rate:- 63 bpm. Activities: Jigsaw puzzle time. Walking. Exercise wobble board. Weather: 15 degrees, overcast morning, patchy rain afternoon. Mood: Sachin appeared well rested, he did seem unfocused with routine procedures but appeared happy and alert. Notes: Sachin and I went for a 24 minute walk in the street right out front of his house, then returned home. Sachin finished at 2529 steps. Sachin used the toilet, medium urine. I assisted Sachin brush his teeth, he then had 15 minutes on his exercise wobble board. We then made our cups of tea and went into the programs room. Sachin made some progress, he seemed unsure at times and did undo his work but ended up putting it back together. Medication at 1830 hours. We continued jigsaw puzzle till 1930 hours, we then went to the lounge for TV time, fresh tea aswell. Shift ceased at 2000 hours. Support Worker Steve

Progress notes: Sachin in great mood today when I arrived and straight away I signed asking him to go to the toilet and Sachin signed back large output of urine went for walk up Cambridge Dr to park and repeat for 30 minutes with Sachin keeping good steady pace little bit of vocalising and some moderate breathing, Wobble Board for 10 minutes then into bathroom to brush his teeth and from there headed into program room to work on his jigsaw puzzle Sachin making steady progress but at times getting a little distracted and staring off into space heart rate started to go above 80 bpm Praveena administered extra medication at 6.41pm Sachin eventually settled to where his hr was sitting at about 65 bpm and as of 7.20pm hr sitting now at 61 bpm but not really making a lot of progress on his jigsaw finished up at 7.50pm making his way into the lounge room to relax and unwind with another cup of tea, staff stayed back until 9pm Author: Jarrett Polkinghorne.

Mood: Sachin appeared calm and well rested today, there was a few times when Sachin was adamant with what he wanted to do first, but our schedule was kept even with little delays. Sachin appeared to be in a good mood. Notes: Sachin and I went for a 20 minute walk along Tabletop road to first Bridge, then returned home. Sachin started at 326 steps and finished at 2448. Sachin used the toilet, medium urine. I assisted Sachin brush his teeth, we I then made our cups of tea and went into the programs room. Sachin finished off his Jigsaw puzzle tonight, and started a new one, he got the outline done then called it quits for the night.
