Activities:
No walk tonight due to rain
Teeth brushed ,program room to start on new puzzle.

Toileted at 5.30pm moderate output.

Progress Notes:
Started shift tonight with Sachin at 5pm ,Sachin was standing in the kitchen with Preveena and was prompted to use the toilet with basic sign ,moderate output then grabbed our cups of tea and made our way into the program room difuzer and music put on and applied some Lavender oil inside his nose via a cotton bud .Sachin was very eager to get started and made good progress on the edges then ground to a halt and was completely disinterested and was happy to just take in his surroundings, Sachins hr at times eas sitting at 69bpm at then getting to 75 bpm ,staff gave Sachin numerous prompts to keep going wyh puzzle but to no avail 6.30pm medication was given by Saj and was then Toileted again at just after 7pm this time large urine output and ended up finishing at 7.30pm heading into the lounge to relax with mum ,ate some Domino's which he enjoyed hr sitting at 70bpm a d monitored til 9pm when shift ended .

Title: Puzzle Blitz
Sleep: Broken sleep 8hrs 37 min

Walk & Exercise: none
Activities: jigsaw puzzle
Mood: very good. Seems happy with progress on puzzle
Weather: 14 degrees. Cloudy & showers
Notes: Sachin on my arrival was in study. Showered an eaten. Praveena made us some tea. Sachin brushed teeth then went to toilet(medium). 5.15 very focused on puzzle as it gets closer to completion. HR 78 bpm. Sachin absolutely blitzed the jigsaw puzzle today. Completing 4 1/4 rows. Saj administered medication at 6.15pm. Sachin heart rate fluctuated between 61 bpm and 76 bpm. 6.45 pm Sachin encouraged to use toilet (small). We continued puzzle till 7.30pm. Took a mandarin. Tv and couch time with Praveena under a warm blanket.

Resting heart rate:- 62 resting bpm.


ACTIVITIES.
Jigsaw.
 Movie 


WEATHER:
13 degrees and Overcast.


Mood:
Sachin appeared alert, calm and behaved. 


NOTES:
Started the shift at 5:30pm with Sachin going to the toilet and brushing his teeth. 
We then waited for Praveena to make a cup of tea for Sachin and a coffee for myself. 
We then sat in the programs room where we listened to music with the diffuser running. Sachin competed a row he had commenced on his puzzle and commenced the pieces for the next row. 
BPM sitting around the 72 to 76 BPM rate at 5:56pm. I noticed he would slide the right piece into the puzzle positioning a few times over. 
BPM rose to 89 at 6:26pm and dropped back to 78 BPM at 6:27 a minute later. 
Handed Sachin his medication at 6:30pm and he self administered. Sachin then continued his puzzle. BPM 71 at 6:41pm. Saschin’s rose to 77BPM at 6:54pm. 
He completed 1 and a half rows of his puzzle before becoming disinterested in his puzzle at 6:55pm.
Sachin then used the toilet and sat in the lounge with praveena where he rugged up on the couch and we watched James Bond - The Spy Who Loved Me. Sachin had his iPad on his lap with cartoons playing at the same time. 8:10pm Sachin got up for a sticky beek in the kitchen and praveena made a cup of tea and provided Sachin with a mandarin after Sachin returning to the couch. 
Shift finish at 8:30pm.

ACTIVITIES.
Jigsaw.
 Walk.
 
WEATHER:
19 degrees and Sunny.
 
Mood:
Sachin appeared to have slept well , he was calm and behaved. He was alert and aware.
 
NOTES:
When I arrived for shift Sachin was just finishing tea. First up we went for a short walk to the wnd of the street and back, 13 minutes. Sachin used the toilet ( large urine output ). Sachin was assisted with brushing his teeth.
Sachin and I then made up our cups of tea and made our way to the programs room. For the first half hour Sachin was not interested and required prompting to get focused. Once he got going he struggled with his old trait of putting the pieces in multiple times, I assisted him with moving on and not getting stagnant with this behaviour. Sachin was given his medication at 6:30 pm by his father.
Sachin kicked into high gear after his medication and really put pieces together fast, he put together a whole line before finishing time at 7:30.
At 7:30 we packed up and went to the lounge to watch TV with his mother. Sachin had a few cheeky moments of wandering the kitchen to sticky beak at the pantry, kettle and bins then got himself settled again on his couch.

Walk up Cambridge drive to table top Rd and back completed 2045 steps

Program room to work on his puzzle
Toilet at 5.30pm moderate output

Progress Notes:
Sachin was standing in the kitchen when I arrived tonight, got him ready straight away for his walk and was extremely eager to get going and set a cracking pace so much so that I had to slow him down.we walked up Cambridge drive to the Table top Rd turn off and back with an average hr of 104 bpm toileted when we arrived back then brushed his teeth moderate urine output, then grabbed our cups of tea and headed to the program room where Sachin got stuck straight into it making great progress.Sachin given his medication at 6.30pm and continued with his puzzle, Saj Toileted Sachin again just after 7pm small output ,for most of the session Sachins hr didn't go above 80bpm finished up just after 7.30pm making our way into the lounge where Sachin sat on the couch to relax and have his night tea shift ended at 8pm.

On arrival Sachin had showered and finished dinner.  At 5.45 toilet(medium) Brushed teeth and plates catch with rugby ball  Made cup of tea then off to puzzle room. 6pm Hr 74 bpm. 6.30pm Hr 89 bpm  medicine taken. 7pm he 71bpm. We went to lounge and played uni colour match. Sachin much quicker & more engaged than yesterday. 7.30 pm toilet (large urine). Couch & tv time. 8.15 pm night tea. Hr 76bpm

Sachin was in good and cooperative mood.

On arrival Sachin had showered and eating dinner. Was a bit cold so we did not walk today. At 5.25 we made cup of tea. Brushed teeth and went to toilet(large urine). 5.30 puzzle room. Hr 94bpm. 6pm Hr 83bpm. Sachin in very tricky part of puzzle (lot of green pieces). I really admired Sachin DETERMINATION  and how patient he was. Got a bit stuck but persisted and finished a row. 7pm encouraged to toilet (small urine). We then played a bit of uno cards taking turns to follow the colours. Hr 76bpm.
7.45 pm tv and couch time.
I really enjoy my time with Sachin he seemed really happy. 8.30pm hr 74bpm.

Road trip day.

WEATHER:
14 degrees and overcast, light rain day.
 
Mood:
Sachin was cool and calm tonight he appeared well rested and was easy going, Sachin was his relaxed self.
 
NOTES:
When I arrived for shift Sachin was having a shower, after his shower I assisted Sachin to brush his teeth, we then made up cups of tea and hit up the programs room for some jigsaw puzzle time.
Sachin appeared interested in it tonight, perhaps because he has missed the last few nights by being in Melbourne. Sachin had his 6:30 medications dispensed by his father. We continued jigsaw for a while, Sachin was working away contently and did not appear fazed that I assisted him. Sachin put 9 pieces in tonight, he worked slow and at his pace.
At 730 we went to the lounge and watched TV with his mother.

20 degrees and overcast, light rain all day.
 
Mood:
Sachin appeared calm and well rested tonight, he appeared to be in a good mood and happy to go with our nightly routine with out arguments or distractions.
 
NOTES:
I arrived at 5:00pm, Sachin was watching TV with his mother. Sachin was encouraged to use the toilet, he produced a large amount of urine. Sachin was then assisted with brushing his teeth, he had himself a quick go too. Sachin and i got ready for a walk then walked to the end of the street and back, a good 500 metres all up. Sachin and I made our cups of tea and went to the programs room for jigsaw puzzle time.  Sachin had his 6:30 medications dispensed by his father. After his medication Sachin appeared to loose interest in the puzzle, he required assistance to remain focused, at 730 we packed up and went to the kitchen. Sachin used the toilet again (medium urine output) while I made up a new cup of tea for him. Sachin and I then watched some TV with his mother.
