ACTIVITIES. Walk. Jigsaw. Tv time. WEATHER: 14° cloudy, misty rain day. NOTES: Sachin had a good evening, praveena helped him with his shower routine, then had his breaky. Sachin had just one cup from his tea pot then I interrupted him for a walk, Sachin wasn’t impressed at not being allowed to finish his 3 odd cups before walking and was quiet and moody. We kept the walk simple, we stayed in Cambridge drive and completed a 30 minute walk. When we returned home Sachin finished off his pot of tea and then dinner. Sachin smashed up his dinner in almost record time, clearly enjoyed it. I assisted Sachin brush his teeth then together we made cups of tea. Sachin and I worked away in the programs room Sachin worked well on his own until we had a passive assertive argument regarding a certain jigsaw piece. He was adamant he had the right piece, yes it was the correct shape bit it was off colour to the surrounding pieced a little bit, sachin wasnt willing to accept that. I removed the piece and he pit it back in, so i removed it again, and again he put it back in. This round a bout went on for a few minutes, SO we compromised, i left it in and he continued working and while he did so I secretly found the correct piece and changed it, he watched me change it, stared at it, thought about it, then smiled and chuckled and continued working. At 1930 hours we packed up and had a toilet break and made new cups of tea, Sachin and I took up seats in the lounge and watched PREY with his mother. Sachin appeared to have a good afternoon, he had a good bpm all shift between 70’s and 80’s. Shift ceased at 2130.

Walk Puzzle Movie WEATHER: 14 degrees overcast Sach was at the dining table when I arrived, Saj was cooking him breakfast and he was drinking his special tea. I helped with breakfast and then went to re- sort puzzle while Praveena showered Sachin. We then went for a short 10 minute walk down the road before coming home and eating a delicious dinner together. We then cleaned teeth and made teas. We then headed to the programs room where Sachin started off slowly with finding pieces - but then sped up and smashed out quite a few. He was being very good with sharing the pieces allowing me to help, he even let me put a couple pieces in all by myself. We finished up at 1930 and went to toilet. Sach took quite a bit of convincing to go, but ended up with small/med output. I made teas at 2000 and got Sachin a snack. We relaxed for rest of evening on couch watching movies and playing with fidget toys. Heart rate was steady throughout evening. Never went over 85. Support worker Steve Risk level low

Weather 13° Overcast Activities: Brushed teeth, puzzle Room to work on puzzle no walk due to low sleep Progress Notes: Sachin was relaxing in the lounge with Preveena when I arrived tonight and was very happy to see me , had a quick chat with Preveena then sat down and had something to eat with Sachin when finished straight away taken to the toilet large urine output then into the bathroom to brush his teeth,we then grabbed our cups of tea and went into the program room to start work on his puzzle very slow start tonight due to how intricate it is hr was a little high at 85bpm and eventually Sach got a little frustrated so we had a break and headed to the toilet only a small out put then re engaged to the puzzle with absolutely no luck then SaJ came in to help Sachin didn't seem to worried though so we were just about to head out when Saj found the vital missing pieces and put them in but we packed up anyway sitting in the lounge to relax for remainder of the shift hr was a little elevated at between 75 to 85bpm shift ended at 9.30pm. Puzzle Tv Show WEATHER: 16 degrees overcast Sach was in the shower when I arrived, so I sorted out the new puzzle in the programs room. Once out, I helped Praveena with dinner while Sach ate leftover noodles happily. Once I ate, we made teas, then Sach went to toilet (small) and we cleaned his teeth. We then went into programs room and started the new puzzle depicting Paris. Sachin was very focused and was allowing me to help out, so we got all the edge pieces into place! We came out at 1930 and went to toilet (medium). Sachin went and sat on couch while I made teas and got snacks - we then relaxed on couch for rest of evening - heart rate was fluctuating and peaked twice while sitting down at 102. Outward appearance was calm and he did not zone out at all. Support worker:- James

ACTIVITIES. Walk. Jigsaw. Tv time. WEATHER: 14° sunny day. NOTES: Sachin was awake showered and dressed when I arrived, he was drinking the last of his pot of tea. Straight after that we went for a walk, we kept it simple and stayed in the street. Sachin put in a 22 minute walk tonight, he appeared to walk fine, no struggles, he appeared happy and was a bit vocal, but sounded happy. When we got home I assisted his father cook dinner, Sachin waited patiently in the lounge or stalked his mother in the kitchen. Dinner was served, Sachin appeared disgruntled at Noodle stir-fry but soon got over it and ate it all up. After tea we teamed up and made teas and coffee, we took them to the programs room.sachin was a bit slow at first, reminiscent of last night’s beginner mode but soon picked up speed to the normal sachin speed. Again I wasn’t allowed to help, all I was allowed to do was click the pieces into place he found. Sachin had a good BPM below 80’s whilst in the programs room. Sachin finished his puzzle tonight, I encouraged him to put the last piece in himself without help. We packed up and went for a toilet break(medium urine output), Sachin and I then rested on the couch watching a Spanish thriller with his mother. At 2000 hours Sachin had a snack of murukku and a sleep tea. Sachin was very relaxed during couch time and was a little vocal at times. Shift ceased at 2130. Support worker:- James

ACTIVITIES. Walk. Jigsaw. Tv time. WEATHER: 15° sunny day. NOTES: Sachin was in the shower when I arrived, he was being assisted with his shower routine by his mother. Straight after his shower he had his breaky. I assisted Sachin put his joggers on then we went for a short walk. We went along Tabletop road for a 20 minute walk, Sachin so far has been in a good mood he kept a good walking speed. Back at home Sachin and I killed a few minutes in the programs room doing jigsaw while we waited for dinner to be ready. Sachin didn’t appear flustered by the routine change. Soon we were called to the table for dinner, once he was done we returned to the programs room, with cups of tea for more puzzles. Sachin focused well but couldn’t find the pieces he needed, normally he can look and find the piece fast but tonight he acted like a slow beginner. I offered assistance occasionally but he would stop what he was doing and watch me like a hawk, and take pieces out of my hand as if to say “no thanks, I’ve got this”. We went over time and at 1940 hours we packed up and exited. Sachin had a toilet break, large one, then we sat in the lounge watching ‘Signs’ movie with his mother, at 2000 hours he had a snack and a sleep tea. Sachin has appeared to enjoyed his evening. Walk Magnets Puzzle Tv Show WEATHER: 13 degrees - sunny Sach was in the shower when I arrived. Once out, we went for a 20 minute walk around the estate. About halfway through, Sach appeared to get upset and a little sad, so we stopped for a minute for him to calm down. He was ok and very happy once we got home and he realised that Praveena was cooking his dinner for us. We ate, made teas, brushed teeth and Sach went to toilet (very small output). We then went into programs room. We played with the magnetic blocks for about 40 mins and then went onto the puzzle. Sach absolutely smashed through the puzzle - doing more than 2 rows. We packed up and 1930 - sat on lounge. At 2000, we made teas - tried to get Sach to go to the toilet but no output. We then had snack and relaxed on couch. Saj came out at approx. 2040 and got Sach to go to toilet - large output. Support worker:- James

Tv time. WEATHER: 15° sunny day. NOTES: Sachin had a very easy afternoon, he already had a shower by the time I arrived. Sachin and I fluffed around in the lounge watching TV and fiddling with toys. Sachin had take away from Newmarket hotel for tea, he appeared to thoroughly enjoy his sticky BBQ ribs and other varieties of cooked meats. Sachin used the toilet at 1820 and 2000 hours, both occasions produced a medium amount of urine. Sachin had a snack at 2000 hours of dorito chips. We finished off the night watching movies with his mother. Sachin appeared to have no issues tonight, his bpm was good and consistent in the 70’s. Shift ceased at 2130. Support worker:- James

ACTIVITIES. Tv time. Jigsaw, walk. WEATHER: 15° sunny day. NOTES: Sachin was having a shower when I arrived, when he was done we went straight out for a walk along Tabletop road. Sachin and I had a 25 minute walk, he had no issues and walked well. I encouraged sachin to use the toilet when we got home, he didn’t need to go. Sachin had his dinner, I then asked him to use the toilet again, he didn’t go so on to brushing teeth. With those jobs out of the way we made cups of tea and went into the programs room. Sachin worked well, he was back to normal, no issues like,last night. Sachin completed a whole line. At 1930 hours we packed up the program room and exited. I tried Sachin in the toilet and again he didn’t need to go. Sachin took his seat on the lounge while I made him a hot water bottle, he watched post cards with his mother. At 2000 hours I made him a sleep tea and small bowl of dorito chips. At 2030 hours I encouraged sachin to use the toilet, this time he did go, large urine quantity produced, he returned to his seat and watched paw patrol movie with his mother. Sachin has had a good night, he didn’t have any issues to report, he was alert and well rested. Shift ceased at 2130. Support worker:- Katy

ACTIVITIES. Walk Magnets Puzzle Movie WEATHER: 15 degrees - sunny When I arrived, Sach and Praveena were out, I went to programs room and made sure everything was neat and the diffuser was full. Sachin came home with KFC and was clearly very excited for it. We ate dinner together, then Sach went to toilet (med/large output). We then went for a short walk - Sach was powering through and was also very talkative. We got back, brushed teeth, made teas and headed to programs room. We played with magnetic blocks for the first hour. At the beginning, he was making patterns with the blocks but was confused when the poles weren’t lining up - I showed him how to fix it and he picked up on it immediately. We then did the puzzle, Sachin was very focused and did not zone out at all. We packed up at 1939 - Sach went to toilet but no output. Sach then sat on couch while I made read and got some corn chips. Sach ate these quickly - it took him a while to settle back into couch - he was very fiddly and chatty. Eventually he calmed down and relaxed for evening. Heart rate was good and below 70bpm from 2000. |
