NOTES:
Sachin has had plenty of sleep today, his bpm was good and generally in the 70's. Sachin appeared rested and happy, he was quiet though. Sachin was inside the house drinking his tea when I arrived. I assisted his mother make dinner then sat with Sachin while he ate. I encouraged sachin to use the toilet, medium urine output. I assisted Sachin brush his teeth then we made cups of tea. Sachin put in 1:15 hours of work into his jigsaw puzzle, he worked quickly, finding many pieces bit was over fussy with connecting them together. At 7:30 hours Sachin was encouraged to use the toilet, no go. We sat in the lounge watching Netflix documentary of Steven Avery murder trial. At 8 pm we made cups of tea, dorito snack and sat back in the lounge. No issues to report for Sachin, he had a good night.
 
Shift ceased at 2130 hours.

NOTES:
Sachin was inside the house sitting quietly at the table drinking his pot of tea when I arrived. I assisted his mother make dinner then when I had done Sachin and I went for a short walk around the block.
Sachin had his dinner when he got home, he used the toilet for a medium amount urine then I assisted him brush his teeth. Sachin and I made cups of tea and worked on his jigsaw puzzle. At 1930 hours Sachin used the toilet and sat with his mother watching Steven Avery court case, on TV. At 2000 hours we joined his father in the garage for family sitting. Sachin had a decent amount of sleep, a short walk, puzzle and movie time today he appeared happy and content.
 
Shift ceased at 2130 hours.

NOTES:
Sachin had plenty of sleep last night, shortly after he finished drinking his tea we exited for a walk. Sachin and I went along tabletop road for a 2.8 km walk that took 35 minutes. Sachin appeared to have no problems with his othotic inserts for his shoes. We returned home for Sachin to have his dinner, he appeared to enjoy his chicken dinner. Sachin used the toilet (large output) then I assisted him brush his teeth. Sachin and I made cups of tea and entered his programs room, Sachin finished his puzzle then started a new one tonight, he didn’t have issues finding pieces, he appeared to work well and enjoy it. Sachin and I went for a drive along kaitlers road, Sachin likes drives.
We returned home for a toilet break and movie time, at 2000 hours Sachin used the toilet again (medium output). Sachin and I sat in the lounge with his mother and watched true crimes tv shows.
Sachin has had a good night, he was rested and happy, nil issues to report.

Monday 27th May 2024


Sachin was getting dressed, fresh out of the shower, when I arrived. I sat with him while he ate breakfast and drank his tea from the teapot. Once finished, I helped him prepare the tea for tomorrow. We relaxed in the couch, I helped Praveena cook, and did some cleaning while waiting for dinner to be ready. We ate together, Sach appeared to enjoy his dinner a lot and smashed it down. I cleaned up and then prompted Sach to the toilet (small urine, moderate poo). I then assisted with cleaning teeth, and then we went to prep teas. Each was in the ball tonight, and didn’t need any additional prompting while making teas. We heard to programmes room and got approx 75 mins of puzzle time. Once again, sach didn’t need any reminders to focus and he had an excellent bpm almost immediately. We finished up at 1930 and went to the couch. Sach relaxed very quickly. I got him a tea at 2000 and some Dorito chips. Toilet again at 2100 (large). No issues.

Thursday 16th May 2024

Sachin and Praveena arrived home from art class at approx. 1620. I helped Sachin change into his slippers and sat with him while he drank his afternoon tea. We ate dinner together, there was no meat so Sach has to be hand fed again. After, we cleaned up and prepped our tea cups. While the water was boiling, he went to the toilet -medium output- and I cleaned his teeth. I poured the water into our cups and followed each to the programmes room. Once again, he settled immediately and began working away at the puzzle - bom in 70’s. He was struggling to find pieces this evening and didn’t even finish one row. His heart rate was great however. Finished at 1930 and Sach came and sat on the couch. I made tea at 2000 and Praveena made popcorn. Sach’s bpm was great until approx. 2045. I promoted him to the toilet - medium output - and he settled a small amount. BPM in high 70’s for the rest of the evening.

Sachin was on the couch with Praveena when I arrived. I chatted shortly before helping Saj and Praveena cook dinner. Sachin was up and about, following us around and being quite vocal. We all ate together - Sach appeared to enjoy it. We cleaned up and I prompted him to the toilet (large output) and cleaned his teeth. We prepped teas and immediately went to the couch. Praveena gave Sach a head massage, he didn’t really settle so after an hour, we got him up and sat him on the couch. He appeared to be emotional, so I got him his snack early. He settled somewhat after this, but his bpm never went below mid 70’s. Toilet at 2015 (small output). No major issues, just Sach appearing to be emotional and a little unsettled, which is understandable due to lack of sleep.

Sachin was sitting at his table with his afternoon pot of tea when I walked in. Soon after, his OT arrived and we all went to the programmes room to do something activities to assist when he goes to the toilet. We did this for approx. 45 mins before Sachin got up and walked back to dining table to finish off his tea. Despite the abrupt end to his lesson, he appeared focused and happy during. We had dinner together, Sach was a gentleman and waited for me to finish eating before he got up from the table. We cleaned up together and made teas together. I prompted him to the toilet (small output) and cleaned his teeth. We then grabbed the teas and headed to the programme’s room. Sachin struggled to begin with, but we eventually figured it out and Sach began getting pieces quickly. We finished up at 1930 and Sach settled quickly on the couch. I got him a snack at 2000 and made him a tea. He was stubborn about eating his chips himself and wanted us to hand him them. It took a bit of convincing but he eventually gave in and ate them himself. He was prompted to the toilet at 2050 (large output). No issues.

Sachin appeared to be in a good mood, happily sitting on the couch when I arrived today. We decided to go for a short 15 min walk before dinner. He kept a good pace and was smiling all the way. We got home and ate dinner together, cleaned up together and prepped teas. I prompted him to the toilet (large output), and then brushed him teeth. We grabbed our tears and headed to the programme’s room. He was smashing out the puzzle and didn’t need reminding to focus at all. We finished up at 1930 and Sach immediately wanted his next cup of tea so we made it together and then went and sat on the couch. I got him a snack at 2000. No issues.

Sach was having some lone puzzle time when I arrived today. He appeared relaxed and was completing the puzzle well. I got him up to finish the rest of his tea pot, and then assisted him with preparing the tea for tomorrow. I helped Praveena with cooking, and once complete, Sach and I ate together. Sach appeared to enjoy his dinner very much. We cleaned up together, he went to the toilet (small output) and I helping him clean his teeth. We then made teas, he appeared very impatient to get back to the programme’s room. Once seated, his bpm was excellent and he was focused the whole time. We finished up and spent the rest of the evening relaxed and settled. No issues.
