Seizure risk level:- Risk level, minimal. Previous night Sleep:- 8.24 hours split sleep. Resting heart rate:- 65 bpm. ACTIVITIES. Walk. Jigsaw puzzle work. Weather: 14 degrees, Overcast, patchy rain. Mood: Sachin was alert and vocal today, he appeared uncertain and or confused at times during our evening walk, also vocal noises under his breath. Notes: Sachin was ushered to the toilet at start of shift, Large urine produced, Sachin was then assisted with brushing his teeth. Sachin and i had a return to normality tonight, we went for a evening walk! 27 minute walk along Tabletop road, Sachin started at 629 steps on his fitbit and finished at 3542, that’s 2873 steps in 27 minutes. When we returned to the house we made cups of tea and went into the programs room for Puzzle time. Sachin was straight into it, he appeared more focused and was concentrating on one area at a time, searching for the piece, finding it and moving on to the next, I have not witnessed Sachin focus like this in a while. Medication given at 1830 hours. Sachin and I continued on his puzzle until 1930 hours, at that time Sachin was instructed to turn of his desk lamp, this practice was initiated to help Sachin understand it is finishing time. We then went to the kitchen and made new cups of tea and retired in the lounge watching TV. Shift ceased at 2000 hours.

Walk up Table top rd to first bridge and up and back Cambridge dr, 3112 steps in 33 minutes. 2 minutes on wobble board, stopped due to high heart rate of 130bpm. • Mood: Calm and quiet. • Weather: 15c Sunny.

Progress notes: Sachin was standing in the kitchen putting away dishes with his Nan when I arrived for shift ,promted Sachin to use the toilet using basic sign large urine output then got ready for his walk which was only short due to level of risk only walking up to park and back once completing 960 steps ,Sachin very heavy on his feet and was not pushed ,no wobble Board tonight but folded up some tea towels then brushed his teeth and straight into program room to work on his jigsaw, Sachin extremely distracted tonight requiring a few promts and often appeared stuck extra Camomile tea given tonight medication given at 6.30.pm and Lavender oil put on his top lip every half hour to assist with him calming, Sachins hr constantly fluctuating throughout the evening sometimes going above 75 bpm then going as low as 58 finished up in games room at 7.40pm and toileted passing a very large amount of urine now relaxing in lounge but still a little agitated and a lot of vocalising whilst sitting in the lounge,happily eating a bowl of Doritos until end of shift at 9pm. Mood: Sachin was more alert, vocal and physically active than normal, he had a higher than normal BPM and it would spike unpredictability. Notes: Called in for assistance today, Sachin has had minimal sleep and a abnormal high resting heart rate, all precautions and calming aids used to ensure a calm environment. Sachin, his family and I went for a car ride this evening in attempts to keep him calm and stress free, Sachin likes car rides and in particular appeared to like the city night lights at a look out we went to. We returned to the house and at approximately 1740 hours Sachin used the toilet, No urine produced, Sachin was then assisted with brushing his teeth. Medication at 1700, 1830 and 1930 hours tonight. Sachin and I sat in the lounge room tonight, saj decided against doing Jigsaw puzzles, he believed if Sachin got stuck it might cause unwanted stress on Sachin and therefore a higher risk to seizure. Sachin had a snack on the couch, then was taken to the toilet by his mother. Sachin had a brief video call with his brother Rahul, Sachin glanced at the phone a few times. Lavender was applied to Sachins top lip every half hour and the lavender diffuser was on in the lounge. Sachin had double does chamomile tea tonight. Shift ceased at 2000 hours. Seizure risk level:- Risk level 1. Previous night Sleep:- 8.08 hours sleep, split. Resting heart rate:- 63 bpm. ACTIVITIES. Jigsaw puzzle work. Weather: 14 degrees, Cloudy with rain. Mood: Sachin appeared Calm and rested when I arrived, he was quiet and reserved. Notes: Cold rainy and cloudy day, Sachin and I could not go for our exercise walk, and lately he has not been a fan of using the Treadmill. At approximately 1730 hours Sachin used the toilet, large urine produced, Sachin was then assisted with brushing his teeth. We took cups of tea his mother made us and went into the programs room for Puzzle time. Medication at 6.30. Sachins heart rate was not stable this afternoon on my shift, it appeared to me any little distraction, interruption or getting stuck on his puzzle would raise his rate, and it would take a while to come down. During this time in the programs room Sachin was closely monitored, all calming aids were used, and gentle assistance from me in puzzle help kept him around high 60's to low 70's. Sachin has once again reverted to putting too many pieces on the board, a few times this afternoon I encouraged him to stop putting more pieces in there, perhaps to him its understandable but to me it looks like unorganised chaos with no room to move. At 1930 hours I instructed Sachin to turn off his desk lamp, I then followed him to the kitchen where we made new cups of tea and settled on the couch watching TV. Weather mostly sunny 15° Toileted at 5.15 pm large urine output Activities: Walk 20 minutes up Table top road and Cambridge Dr Wobble Board 5 minutes Program room to work on puzzles Progress notes: Sachin still eating his tea on arrival tonight, toileted straight after finishing large urine output then we got ready and headed off for a walk going up Table top road to first house then back going along Cambridge Dr to park twice very heavy going Sachin vocalising quite alot covered 1.52 km avg hr was 88 bpm taking 20 minutes then spent only 5 minutes on his wobble Board due to hr being a little high teeth brushed then Rahul giving Sachin 200mg epilem at 5.10pm into program room to work on his jigsaw slow steady pace bpm at start sitting between 71- 75 bpm but at around 7pm coming down to around 60 and appeared quite relaxed and calm . Sachin has had a Seizure at 7.45pm lasted over a minute assisted into recovery position on floor recovery quite slow then when ready Sachin assisted to lounge to lie down in a comfortable position.
