Meat lovers nightmare

Weather: 13degrees mostly cloudy.
Sleep: 10.30
RHR: 63 bpm
Risk level: Minimal
Exercise: walk
Mood: Good. Cooperative and responsive
Notes: HR 86 bpm. 4.30 pm Sachin had shower and we went for a walk. Sachin had dinner. He was not impressed with the vegetarian meal. 5.50pm rousting of toilet(medium) brushing of teeth. Cup of tea and off to puzzle room. HR 76 bpm. At 6.27 pm Sachin had a black out. I called out to saj and Praveena. We were able to keep Sachin safely in chair and Sachin came out of black out and not go into a seizure. We took Savin to lounge and monitored him. 6.40 pm medication taken. Prior to episode about 20 pieces of puzzle done. 7.30pm toilet(medium). Couple of mandarins. 7.55 HR 75bpm

ACTIVITIES.
Jigsaw.
 
 
WEATHER:
12 degrees, Sunny but chilly day.
 
Mood/Behaviour:
Sachin appeared to be well rested, he was alert and aware of his surroundings, Sachin did appear confused a lot tonight, he would half finish his jobs that he would normally obsess about doing right.
 
NOTES:
Sachin was in the middle of his evening meal when I arrived, after he finished he was encouraged to use the toilet, he produced large urine. Sachin was then assisted with brushing his teeth.
We skipped the evening walk tonight, we ran late and just went on without. Sachin and I made up our cups of tea and headed to the programs room for jigsaw time.
Sachin engaged for approximately the first 5 minutes then was not interested, after a few minutes of interaction with him I noticed his hands and cheeks were cold, I asked Saj to turn on the house heater, Sachin started having little shivers here and there, I placed a blanket on his legs to help warm him up, he continued with the little shakes for a few minutes, I then asked his mother praveena for a electric blanket to use on his top half. Sachin came good in no time but he was still adamant he didn’t want to do any puzzle tonight, he was smiling And happy, he generally looked happy to just hang out with me.
Sachin had medication at 630, dispensed by his father.
Sachin had no more issues with the cold, I continued to persuade Sachin to engage his puzzle, unfortunately to no avail. At 730pm, we then packed up and headed to the lounge.
Sachin was encouraged to use the toilet again, medium urine produced.
Sachin prepared his teapot, placed his tea bags in ready for tomorrow then settled on the couch watching ninja warrior with his mother.
 
Shift finished at 2000 hours.

Activities:
No walk tonight due to high risk factor
Toileted at 5.15 pm but didn't go
Teeth brushed
Program room to work on puzzle


Progress Notes:
Arrived at 5pm to start shift with Sachin who was standing in the kitchen with Preveena and looked very relaxed and happy to see me. Toileted straight away but didn't go so headed to the bathroom to brush his teeth by that time was joined by Shadow shift Jack explaining to Jack that there was Ni walk tonight due to the weather being extremely cold,so we grabbed our teas and made our way into the program room and put o  some relaxing music and got Jack to turn difuzer on. At the start Sachins hr was sitting at 75 to 85 bpm and was making slow but steady progress in his puzzle ,6.30 pm medication was given and continued on his puzzle hr began to drop even further sitting between 53 to 60 bpm ,Sachin was was not vocalising alot tonight but was extremely comfortable with Jack sitting near him and positively interacting with Jack which was great to see we eventually finished up at 7.30pm and made our way into the lounge room so Sachin could relax staff Toileted Sachin at 7.40 pm large amount of urine output then back to the couch where Sachin was watching videos on and off on his ipad shift ended at 8pm with nil issues to report.

WEATHER:
12 degrees, misty rain day.
 
Mood/Behaviour:
Sachin appeared rested and behaved well during shift, Sachin was happy and easy going.
 
NOTES:
Sachin was encouraged to use the toilet at start of shift he produced large urine, he was then assisted into his,walking shoes.
Sachin and I went for,a walk tonight, 20 minutes worth, Sachin appeared to really enjoy his time out, he smiled alot.
When we returned we made up cups of tea and went into programs room for jigsaw time.
Sachin was really in the mood to get it done tonight, he put piece after piece inthe right spots.
Sachin had medication at 630, dispensed by his father.
Sachin continued on at his impressive speed till 730, we then packed up and settled on the couch watching Kevin heart with his mother.
Shift finished at 2000 hours.

Sleep: 11 hours & 52 min.

Ridley level: Minimal
Exercise: rugby catch & quick walk
Mood: Happy. Sharing and caring. Today sachin was letting me place pieces of the puzzle in. Finding the piece and waiting for me to place it.
Notes: 4.40 Savin arrived home from art class. We went for a quick walk a played catch with the rugby ball. Then we had dinner. HR 80bpm. Routine of toilet no urine. 6.05 off with our cups of tea. 6.30 saj gave medication. Sachin and I completed 1 row. Again Sachin was relaxed and his HR highest 80 lowest 70bpm doing puzzle. 7.30 pm off to tv room. HR 72 bpm.

ACTIVITIES.

WEATHER:
11 degrees, misty rain day.
 
Mood/Behaviour:
Sachin appeared to be well rested, he was well behaved.
 
NOTES:
Sachin was just finishing up his evening meal when I arrived,  he was encouraged to use the toilet, he produced a large amount of urine.
Sachin was then assisted with brushing his teeth, after that we made teas and went to programs room. Sachin was straight into it putting pieces in the right spots.
At approximately 1803 hours Sachin began seizing whilst seated, Sachins Father and I lowered him to the floor and put him in the recovery position, Sachin came out of it fairly quickly.
Sachin was encouraged to sit up he appeared groggy so he was encouraged to lay back down and rest.
Medication dispensed at 6:30 by sachins Father.
At 1915 hours Sachin was encouraged to his feet and ushered to the toilet, he produced a medium amount of urine, Sachin was not interested in going to the lounge to rest and returned to the programs room.
Sachin continued on like nothing happened, putting a good amount of pieces together.
 At 1940 hours Sachin and i packed up and went to the longe to watch stranger things with his mother.
Sachin had some chips for a snack and a cup of sleep tea.
 
Shift finished at 2100 hours.

Happy focused Sachin

Sleep: 7 hours & 47min.
RHR 65 bpm.
Exercise: rugby catch
Mood: happy and very focused on puzzle.
Notes: routine of toilet (small urine)and brushing teeth. HR 85bpm. Praveena prepared us some ginger tea and we started puzzle. Great progress with puzzle 1 row done by 6.30. Took medication. Toilet stop(medium urine). Played catch with rugby ball. 6.50 more puzzle till 7.50. Sachin completed 2rows today. Tv and lounge time. HR 76bpm. Sachin had some mandarin and a snack. Seems very content watching his iPad.

Relaxed Heart Rate

Sleep: 10hours & 11 min.
RHR 66 bpm.
Exercise: rugby catch
Mood: Sachin was in a very good and happy mood.
Notes: on arrival Sachin had showered and finished dinner. Usual routine of toilet (medium urine). While preparing tea we played some catch with rugby ball. Sachin was enthusiastic. 6pm into puzzle room. After 15 min we found the piece we needed and then Sachin was off and running. He completed 1 row. 6.30pm saj gave medication. 7.30 we settled into tv room. Sachin HR today was good. Fluctuated from 67 to 80 bpm. The lowest since I’ve been working with him during puzzle time.
