ACTIVITIES. Tv time. Walk. WEATHER: 10° scattered showers. NOTES: Sachin was awake and inside the house when I arrived. We got out for a walk today, Sachin appeared to enjoy it, he walked fast. I encouraged sachin to use the jenga blocks to make towers, he worked slowly and with precise movements. Sachin had his dinner and then a toilet break, large urine output. Sachin and I took it easy and watched TV with his mother. Sachin had a toilet break at 2000 hours, a medium output. We made cups of sleep tea and watched Alien 3. Sachin has had no issues tonight. Shift ceased at 2130.

ACTIVITIES. Puzzle TV Show WEATHER: 8 degrees - cloudy/ rain Sachin and Praveena were at art classes when I arrived. I busied myself by tidying the programme’s room and packing away dishes. They arrived back at 1620. Praveena started cooking dinner while I helped Sach prepare his tea for tomorrow. We ate dinner together and prepared teas - while the water was boiling, we cleaned his teeth and I prompted Sach to toilet. He went right away - medium output. We poured teas and headed to programmes room. Sachin was keen to get started. I was strict from the beginning with him pushing the pieces and bending them - and he stopped doing it for over 2 whole lines. We finished up at 1930 and sat in couch with mum. I made teas at 2000 and prompted Sachin to the toilet - medium output. We sat back on the couch with snack and a hot water bottle and watched black mirror with Praveena. His heart rate was predominantly in the 70ms throughout the evening but did spike to around 90 for approx. 20 minutes while on the couch - Praveena and I kept a close eye on him and he calmed down after a time. ACTIVITIES. Jigsaw. Tv time. Drive WEATHER: 10° scattered showers day. NOTES: Sachin got out of bed when I arrived, I made him his pot of tea. Sachin and I used jenga blocks and constructed towers to improve his motor skills. Praveena made Sachin his dinner tonight and once he was done he was encouraged to use the toilet, large urine output. I assisted Sachin with brushing his teeth then we made cups of tea. Sachin and I moved into the programs room for jigsaw time, we got approximately 15 minutes into puzzles when Sachins Father entered and requested we go for a drive. Sachin, his mother and I went for a drive into town, we drove past albury Gardens to view the Aurora light show then to office works and a petrol station. Sachin had a toilet break back at home and a cup of sleep tea. He finished watching Alien and moved on to watch Aliens movies. Sachin appeared to have a good night with no issues. ACTIVITIES. Tv time. WEATHER: 8° rain day. NOTES: Sachin had just woken up and come out of his bedroom when I arrived. Sachin took his seat at the dinner table and waited patiently while I prepared a fried egg and his pot of tea. Sachin was approximately half way through drinking his tea when I started assisting his father with making dinner. Sachin appeared content with watching me make his hot chicken curry dinner. Sachin had his dinner and then a toilet break straight afterwards, medium urine passed. I assisted Sachin clean his teeth, he then finished off brushing by himself. Sachin and I made cups of tea together then passed on jigsaw time and went straight to the lounge for TV time. Sachin had a minor blackout tonight, he was seated on the lounge in a safe position, praveena was quick to apply lavender oil to Sachins chin area, this appeared to snap him out. Sachin rested for a while then was encouraged to have a toilet break, another medium urine output. Sachin returned to his seat and finished off the night watching Aliens movie with no incident. Puzzle TV Show WEATHER: 10 degrees - cloudy/ rain Sachin was in his room getting dressed when I arrived. I heated up his tea and made his egg for breakfast while Saj gave Sach his meds. I sat with Sach while he ate and had his tea. Mum then got home from work so we helped with preparing dinner - and also prepared tea. We ate together and I then prompted Sach to toilet - he was very hesitant to go and did not respond to my hand signals. It took about a minute of prompting and he eventually went to toilet - small output. I then assisted him with cleaning teeth and we made teas together and headed to do some puzzle. Sachins heart rate was in the 90’s and 100’s for all of puzzle time - and even though he was finding pieces relatively quickly - he was taking a long time to place them into the puzzle. Once his bpm hasn’t settled by 1900, we finished early and went to sit on the couch. He almost immediately settled and his heart rate went into the 70s. While I was making sleep tea at 2000 for Sach, he headed to the laundry to fold some of the clothes in there. Sach went to the toilet approx. 2045 - medium output - and we relaxed on the couch for the rest of the evening - heart rate in the 70’s. Support worker:- James

ACTIVITIES. Drive Jigsaw Tv time. WEATHER: 8° rain day. NOTES: Sachin was finishing his pot of tea when I arrived and once he was done he emptied the spent tea bags from it. His mother washed and filled the pot for him and he put new bags in, ready for tomorrow. Sachin and I skipped the evening walk on account of the rain and instead we went to the programs room for jigsaw time. I fixed his FitBit to his wrist and turned on his ipad to monitor his BPM. Immediately his BPM was too high, 97 – 106, considering that he has not done any exercise or physical Labor this is too high for an at rest BPM. I applied lavender oil to his chin for him to inhale and I encouraged a calming environment for Sachin ( low noise, slow movements, calm demeanour ). After approximately 20 minutes Sachin appeared to settle in and his BPM slowly decreased to 85 – 95. Sachin worked well on his puzzle, he found a decent amount of pieces and his need for perfection was good and low, didn’t pose a problem for him. At 1640 hours Sachin was called for dinner, he eagerly chewed up his butter chicken and cleaned his plate. I encouraged sachin to use the toilet, he didn’t need to go, I then assisted him with brushing his teeth. Sachin, his father and myself went for a drive to the local doctors practice for appointments, I stayed in the waiting room as per his father’s request. When they were done we went to ALDI and the fuel station on the way home. At home Sachin was encouraged to use the toilet, Large urine output. Sachin and I made cups of tea and took them to the couch for Tv time with his mother. Sachin has behaved brilliantly tonight, with all the out of routine activities we conducted it did not appear to affect him at all. Shift ceased at 2130. Support worker:- James

Wednesday the 21st June 2023. ACTIVITIES. Walk. Drive Jigsaw Tv time. WEATHER: 11° sunny day. NOTES: Sachin was drinking his pot of tea when I arrived for shift. When he was done we went for a long walk along Tabletop road, 35 minutes long. We accompanied his father on a drive to woolworths, we walked the isles while we waited. Sachin had his dinner and then attempted a toilet break, he didn’t need to go. Sachin and I made cups of tea and went to the programs room, Sachin worked well tonight, he was fast at finding his required pieces but over time he did start displaying ocd traits, pieces had to be perfect and put in just right. We called it quits at 1920 hours, we exited the program room. I encouraged sachin to use the toilet, he produced a large amount of urine. Sachin and I made new cups of sleep tea and settled in the lounge to watch David Attinborough Our Planet with his mother. No issues for Sachin tonight, he appeared to have a good night. ACTIVITIES. Walk. Tv time. WEATHER: 6° sunny day. NOTES: Sachin had just woken up when I arrived. Toolbox with Saj revealed Sachin has had split sleep and a excessively high resting BPM, medium risk night. Sachin and I went for a walk along Tabletop road, approx 25 minutes, Sachin walked at a reasonable speed and didn’t appear disturbed. We returned home and Sachin began having dinner. Sachin was approximately 2 minutes into dinner when he had a seizure, Sachin had a bite of meat in his mouth when he started. I gained his mother’s attention and she assisted. I attempted to remove the food from his mouth but he was clenched tight. We decided to NOT put him in a recovery position on the tiled floor, we supported him to remain seated upright. We lent Sachin forward and kept his head upright to keep his windpipe open. Fortunately Sachin did not choke and came out of the seizure in 1:30 to 2 minutes. Sachin appeared exhausted afterwards so we remained supporting him for approximately 15 minutes till he was awake and strong enough to move. We assisted Sachin to the lounge where he rested more. At 1830 hours Sachin was encouraged to use the toilet, he didn’t go so we returned to the lounge for TV time. Sachin appeared to be better, he was wide awake and alert, he interacted more and responded more to me, At 2000 hours I encouraged sachin to use the toilet, he produced a large amount of urine. We then made cups of sleep tea and returned to the couch to watch David Attenborough Our Planet. Support worker:- Katy

Puzzle TV Show WEATHER: 12 degrees - rain Sach and Praveena were in the kitchen, I helped Sachin prepare his tea for tomorrow and then helped Praveena prepare dinner. Sach was very vocal and clearly enjoyed his dinner, which was leftover duck. Once done, we prepared teas and I assisted with cleaning teeth. I prompted Sach to toilet - med/large output. We then grabbed the teas and headed to do puzzle. Sach was getting through the puzzle quickly - and was very focused this evening, so I used it as an opportunity to try and get him out of some habits he has developed- such as pushing down and bending the pieces before placing them. This sped up the process even more, and we got nearly 3 full lines completed. We came out at 1930 and sat on couch with mum. I got up and made teas - Sachin got up to help so I prompted him to toilet - he didn’t go. We sat back down with a slice of yesterdays birthday cake (which Sachin tried to steal from me). Saj came in to take Sach to toilet - he didn’t go again. I then peeled some mandarins from sachins tree for us and we relaxed on couch for the rest of the night. Sach had a good bpm and was in a good mood for the whole evening. Support worker:- James

ACTIVITIES. Walk. Tv time. WEATHER: 12° sunny day. NOTES: Sachin was awake and dressed when I arrived. Soon after arrival Sachin and I went for a long walk along Tabletop road. We made it to the second bridge, 1.5 kms in 20 minutes then about faced and returned home. Sachin and I watched his mother cook tea for awhile then watched a little bit of TV until it was ready. Sachin ate his duck curry and rice with haste, he appeared not interested in his dhal, he pushed it away a few times but eventually he did eat it. Sachin was encouraged to use the toilet, he didn’t need to go. Sachin and I took up seats in the lounge, then remembered to make cups of tea. Once we made them we returned to the lounge once more and watched TV. At 2000 hours Sachin had a toilet break, large urine produced. We made another cup of tea and got a snack, we returned to the lounge and watched a movie called extraction. Sachin has appeared to enjoy his night, he did have a bpm higher than I would have liked and at one stage did spike to 110, cause unknown, lavender diffuser and calming tea appeared to help.
