Movie time. Walk Supermarket, drive. WEATHER: 17° Sunny day. NOTES: Sachin was his tea as I arrived, I assisted his father with a job in the garden then went inside. Sachin hung around the kitchen watching his mother prepare dinner. Sachin appeared to focus intently on everything she did, it looked remarkably like KFC, Sachin favourite food. Sachin got very excited before dinner, his heart rate flew up to 89-92 bpms in excitement when he saw it cooked and looked just like store bought. Sachin ate his dinner then he used the toilet (large urine). I assisted Sachin brush his teeth then into joggers. We went with his father to woolworths. Sachin and I walked around while he shopped. We returned home to a toilet break and cup of tea, we watched a film with his mother called “my sisters keeper”, Sachin also had an ice cream. Sachin has been spoiled tonight, and had a good night. Shift ceased at 2130 hours.

Shift Time:- 1600 to 2130 hrs Date :- Thursday 27th June 2024 Zach was in the kitchen with Praveena when I arrived. He already had his shoes on, so we immediately went for a 15 minute walk through the estate. He kept an excellent pace the entire time. Once home, we had some puzzle time before being called for dinner. After, we cleaned up and I prompted each to the toilet (medium urine) and then cleaned his teeth. We then made teas and went to programmes room again. His bpm was immediately excellent and stayed between 68 and 78 the whole time. We finished up at 1930 and went to the lounge room. Sach settled very quickly. Snack and sleep tea at 2000. No issues. Support worker:- James

ACTIVITIES. Movie time. Walk Jigsaw WEATHER: 16° partly cloudy day. NOTES: Sachin was very good tonight, he followed his nightly routine with ease and no complaint. Sachin and I went for a walk before the sun went down, Sachin walked well, no struggles. He had his dinner and was assisted with brushing teeth. Sachin and I put in some time on his jigsaw puzzle. At 730 he used the toilet and settled on the couch to watch state of Origin. Sachins heart rate stayed in the range of 68 to 82 all night, his behaviour and attitude were perfect. Sachin has appeared to have a very good night. Shift ceased at 2130 hours. Dinner for Kings

Shift Time:- 1600 to 2130 hrs Date :- Monday 24th June 2024 When I arrived today, Sach was sitting calmly at the dining table. He appeared to be alert and happy. I prompted him to get up, popped his runners on, and we headed for a walk around the estate. Once back, we sat with mum on the couch for a short while, before sitting down for dinner. Tonight, we had pilau, which is one of Sachin’s favourite dishes, so he was very happy. We cleaned up together, prepped teas and I prompted Sach to the toilet (small output) and brushed his teeth. West grabbed our cups and headed to the programmes room. Sach took approx. 30mins to properly settle into the puzzle, but once he did, he had a bpm in the low 70’s and was working at a decent pace. We finished up at 1930, and while Sach was quickly finishing his tea, he had to stop and rush to the toilet - I helped wipe and we went back to couch. Sach didn’t settle so Praveena and I prompted him to toilet again (only medium urine). Once back out, I made him a sleep tea and got him a snack. He sat and settled with a bpm in 60’s predominantly, but refused to drink tea and eat corn chips. No issues. Support worker:- James

Movie time. Walk Jigsaw WEATHER: 15° partly cloudy day, light rain evening. NOTES: Sachin was home drinking his pot of tea when I arrived. We got prepped and went for a walk before the temp got to cold. Sachin was walking stiff legged, and appeared to struggle a little with balance. He did however finish the walk, 2.7 kms in 36 minutes. Sachin was introverted with occasional outbursts of vocal noises or skipping jumping actions. His high resting BPM concerned me so his facial expressions and behaviour were closely monitored. Sachin had a normal routine night, dinner, walk, puzzle and movie time. Sachin had a few moments of vacant expression and or dazing off, but with a nudge from me he came back to reality. ACTIVITIES. Movie time. Drive Jigsaw WEATHER: 11° partly cloudy day, light rain evening NOTES: Sachin Sachin had 9:55 hours of sleep, it’s a very good amount of sleep but taking into account his resting bpm has been climbing the past 7 days it puts Sachin into a medium risk of siezure category. Sachin was drinking his tea when I arrived, when he was done we went for a quick drive, just long enough to enjoy it. Sachin and I done some jigsaw before dinner, he enjoyed his dinner of lamb curry. We went with his father to fuel up the car and a walk in woolies. We had half hour of TV time then joined the family in the garage for family sitting. Sachin had a good night. ACTIVITIES. Movie time. Walk. WEATHER: 11° partly cloudy day. NOTES: Sachin had 10:10 hrs of sleep and did indeed look rested and happy as well. He finished drinking his pot of tea and we departed the house for a walk. Sachin was a bit stiff legged for the first 500 metres then loosened up and started walking properly. We completed a 2.7 km track in 35 minutes. Sachin had his dinner when we arrived back, he had kfc left overs, but this is of no concern, sachin loved it. Sachin did not use the toilet after dinner, i did assist him brush his teeth. Sachin and i made cups of tea and put in some time on his jigsaw. Sachin didn’t show the usual amount of interest tonight, he was distracted and only achieved ½ a line completed. 7:30 saw Sachin use the toilet and a rest up on the couch watching tv. At 8 we made our last cup of tea and also a treat of dorito chips. Sachin has had a pretty good night. Shift ceased at 2130 hours. Support worker:- James

Movie time. Birthday WEATHER: 13° fine sunny day. NOTES: Sachin was home when I arrived for shift, he was inside the house with his mother. Sachin appeared happy and rested. Today is Sachins Birthday, I assisted his mother and father with a few errands to woolies and chemist, then getting dinner. Sachin had his KFC for dinner, he appeared happy to have it. Sachin used the toilet (large urine output), I assisted Sachin brush his teeth. We made cups of tea and sat in the lounge with his mother and watched a current affair, Sachin was chill and happy, making happy vocal noises. Sachin had a birthday cake and a video call with his brother. Sachins vocabulary is improving, when it was time to end the call he followed my lead and said “bye bye” we made another cup of tea and returned to the lounge with his mother. Sachin has had a good night, he has had his favourite foods, a call with his brother, good behaviour and a excellent BPM. Shift ceased at 2130 hours. A Boisterous Boy

Shift Times, Date :- 1600 to 2130 hrs Monday 17th June 2024 Sachin was drinking his arvo tea when I arrived. I sat with him while he finished, he appeared alert and cheerful. Once finished, I helped him make the tea for tomorrow, he was fast and decisive in his actions. After this, I helped put his shoes on and we went for a short 10 min walk which he appeared to enjoy. We then ate dinner, cleaned up our dishes and hands, and then prepped teas. While waiting for the water to boil, I prompted him to the toilet (small output) and cleaned his teeth. I then poured the boiled water and we grabbed our mugs and headed to the programmes room. We got a solid 2 hours of puzzle time in and Sach got heaps done. We finished at 1930, but Sach didn’t want to settle. He ran around the house loudly for approx. 15 mins before he was ready to come sit down. I made him a tea at 2000 and got him some corn chips (which he refused to eat). Promoted to toilet at 2100 due to bpm spike (medium output) with no effect on bpm. No issues otherwise and his bpm eventually came down. Support worker:- James

Movie time. Jigsaw. Drive WEATHER: 13° cloudy day. NOTES: Sachin was observing his mother make dinner when I arrived, Sachin smiled and fist bumped me. Sachin and I went for a walk before dinner, we went along tabletop for a 35 minute walk. Sachin hasn’t been out walking in a while but he made a good fast pace and appeared happy. Sachin had his dinner when we got back home, after dinner sachin did not use the toilet, I assisted him brush his teeth. Sachin and I put in a few minutes doing jigsaw then accompanied his father on a drive to chemist. Sachin and I returned to the jigsaw when we got back, at 7:30 we packed up and Sachin used the toilet, we settled in the lounge and watched TV with his mother. At 8:00 we made cups of tea and a bowl of chips, Sachin watched posidon movie. Sachin has been no concern during shift, his behaviour and attitude was good also his bpm was good and low, no concern. Shift ceased at 2130 hours. |
