1715, Walk up Table Top rd. to the second bridge and back, 4470 steps in 40 minutes. Brisk pace set by Sach. 1758, Took Sachin to the toilet. 1800, Assisted in brushing Sachin’s teeth. 1806, Music session with Sach for 15 minutes, an improvised jam with no specific goal. He seemed to enjoy the different activity. 1822, Moved to other side of the room to work on current New York puzzle and made very good progress. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip to help relaxation. 1830, Medication administered. 1928, Moved to family room for sleep tea and videos on the iPad. 1950, Took Sachin to the toilet.

Organised weekly medication with Sachin’s assistance. • Mood: Happy and calm, a little lethargic. • Weather: 12c mostly cloudy.
1738, Walk around Brooklyn fields for 30 minutes, 2745 steps at a steady pace. 1818, Took Sachin to the toilet, small urine. 1822, Assisted in brushing Sachin’s teeth. 1825, Organised weekly medication with Sachin’s assistance. 1830, Medication administered. 1848, Moved to activities room to work on Current puzzle, Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Seizure risk level:- Risk level- 3. Previous night Sleep:- 6.01 hours sleep. Resting heart rate:- 61 bpm. ACTIVITIES. Walk. Jigsaw puzzle work. WEATHER: 14 degrees, overcast patchy rain day. Mood: Sachin did have minimal sleep but appeared normal, minimal vocal noises today, good attention and focus to his activities. NOTES: Support worker had a new staff member shadowing today, Sachin appeared to accept him and his presence. Sachin was encouraged to use the toilet at start, he did produce medium content urine. Sachin was assisted with brushing his teeth. We went for a walk along Tabletop road to first Bridge, Sachins step count started at 941 and finished at 2862, 18 minute walk completing 1921 steps. We made cups of tea and went into the programs room for Jigsaw work. 1730 hours medication given. Sachin was attentive and focused well on his puzzle today, he was not willing for me to help or touch pieces, immediately taking them from my hand. Medication given at 1830 hours. At 1930 hours Sachin had extra medication then turned off his desk light and exited to the family room. Sachin was again encouraged to use the toilet, large urine produced. Sachin had a bowl of pop corn and a sleep tea, while watching a Indian Comedy. Shift ceased at 2100 hours. Support worker:- James.

Mood: Sachin appeared in a good mood today, he appeared to enjoy his walk in a new sub division. He showed good focus to his activities. Notes: Sachin had used the toilet already when I arrived, we drove straight to Brooklyn fields for our walk. We started at 1713 hours and with 800 steps on his fitbit, we walked the footpaths in the newly build sub division, Sachin appeared to like this area, he smiled alot. At 1733 hours we finished and recorded 3902 steps on fitbit, 3102 steps for the walk. We returned home and had cups of tea waiting for us, We took our cups and went into the programs room for Jigsaw work. Medication given at 1830 hours. At 1930 hours Sachin turned off desk light and exited to the family room. We watched a Indian horror comedy which Sachin appeared to like. Shift ceased at 2100 hours Support Worker Steve
Sleep 7 hrs 33 minutes rhr 62 bpm Weather 13 ° light rain Toileted at 5.10pm large urine output Activities: Walk up Table top road to second bridge and return 33inutes total 4083 steps at good pace no wobble Board due to risk level Teeth brushed at 5.50pm then program room to work on his jigsaw puzzle Progress notes: Sachin was pretty calm and relaxed when I arrived tonight, toileted straight away passing a large amount of urine then got him ready for his walk and headed up Table top road to the second bridge and back keeping a nice steady pace lots of vocalising walk took 33 minutes avg hr 89 bpm teeth brushed on return then made his way into the program room to begin work on his jigsaw puzzle Soft music on Lavender difuzer put on along with some Lavender oil on his top lip every 20 minutes, Sachins hr a little higher than normal sitting between 75 to 80 bpm at start ,Saj came in and sorted some of the puzzle pieces which had a positive effect meds given at 6.30.pm by Saj and now hr sitting below 70 bpm and making slow but steady progress on his jigsaw finished up at 7.30pm making his way into lounge room for a nice head massage from his Grandmother and another cup of tea and now relaxing quietly. Mood: Sachin was awake and alert when I arrived , Sachin appeared well rested and his focus on activities was good. Notes: Sachin used the toilet straight up, he produced a large urine, Sachin was then assisted with brushing his teeth. Sachin, Madison and I went for a drive to a suburb under construction a few kms from home, we went for our afternoon walk there. Sachin started his walk at 794 steps on his fitbit and finished at 3414, that’s 2620 steps during a 25 minute walk. We returned home and made up cups of tea We then went into the programs room for Jigsaw work. Medication given at 1830 hours. Sachin wanted A new cuppa at 1900 hours. At 1930 hours Sachin turned off desk light and exited to the family room. Shift ceased at 2000 hours. Support worker:- James.

Mood: Sachin appeared to be well rested, he was alert and focused with his activities, but he was making breathing noises often. Notes: Sachin used the toilet at start of shift, he produced a large urine. Sachin and I went for a 14 minute walk in the street just out front of his house, then weather was overcast and cold wind so we didn’t stay out too long, Sachin completed 1595. Sachins mother was making home made tea, so we didn’t make our tea tonight, it wasn’t ready just yet so we went into puzzles straight away. Soon afterwards we were treated to cups of tea. Medication given at 1830 hours. At 1930 hours Sachin turned off desk light and exited to the family room. Shift ceased at 2000 hours. Shadow shift today
