Seizure risk level:- Risk level- min. Previous night Sleep:- 12:09 hours sleep. Resting heart rate:- 64 resting bpm. ACTIVITIES. Jigsaw. Walk. WEATHER: 13 degrees, Overcast patchy rain day. NOTES: Sachin was a dream tonight. He was calm and well rested, he did not display and common traits of jitters, shakes, staring off, ocd, excessive vocal noises. Sachin used the toilet (small urine), was assisted brushing teeth. Sachin and I made our teas and hit up the programs room for jigsaw time, Sachin smashed it tonight completing two horizontal lines. After 7:30 Sachin and I retired to the lounge and watched James Bond with his mother, Sachin appeared interested and watched constantly. Over all tonight was a fantastic change of pace for Sachin, it was good to see him relaxed and calm. Shift finished at 2000 hours.

Seizure risk level:- Risk level- med. Previous night Sleep:- 7:15 hours split sleep. Resting heart rate:- 65 resting bpm. ACTIVITIES. Jigsaw. Walk. WEATHER: 11 degrees, Overcast patchy rain day. Mood/Behaviour: Sachin was quiet tonight, he was well behaved and posed no trouble to work with. Sachin was in a easy going mood and did not complain to instructions. NOTES: Sachin was already in his programs room doing his jigsaw when I arrived for shift. Sachin was ushered to the toilet where he produced a medium urine. Sachin And I went for a short walk tonight on account of the weather and just to keep some routine, 10 minutes of walking up and down the street. Sachin appeared to enjoy the short time out of house. When We returned home and Sachin was encouraged to use the toilet (he produced a medium urine) and assisted brushing his teeth. I assisted Sachin making cups of tea, I gave small verbal instructions and physical indications as well, Sachin followed quickly. Sachin had his medication at 630, dispensed by his father. Sachin has been displaying strange new behaviours with the puzzle lately, he digs through his pieces and finds the correct piece, he will then sit it just below where it needs to go and wait for me to put it in for him. He is including others where before we were not to touch anything. At 730 we packed up the programs room and went for another toilet break, medium urine out put. We then settled down in the lounge for a bit of TV time, Sachin bmp elevated for approx 30 minutes , he appeared excited and over happy, his mother and I remained calm and he soon followed suit, perhaps because of the snack he was waiting for. Arrived for shift, debrief with Saj. Sachin was resting in the lounge room and I got stats from Fitbit. Resting heart rate was 64 bpm. I prompted Sachin to use the toilet, only for a small output. Sachin May have already gone before his shower. Saj suggested we go for a walk, I took Sachin down to the next intersection and back. Sachin seemed happy during the walk. We returned from walk, heart rate was at 87 bpm. We made tea then started puzzles. I assisted find puzzle pieces, heart rate was elevated to 94 bpm. Heart rate dropped to 80 bpm as some of the puzzle was completed. Sachin stopped his puzzle for a while, I prompted him to drink his tea. Heart rate set around 80 bpm. 6.30pm Medication was self-administered. We continued the puzzle and I applied some lavender oil for Sachin. Heart rate sitting at 75 bpm Sachin seems to be doubting himself during the Beatles puzzle. Unsure about leaving pieces on the board. We finished up puzzle time and I took Sachin to the toilet, medium-large output. Support Worker: Luke Corkett
Debrief with Saj, he stated that Sachin had a seizure earlier in the week but had been sleeping more since. I checked stats from Fitbit on tablet, 63 bpm was resting heart rate. Sachin used the toilet, medium output then teeth were brushed. Sachin turned on lamp and started his puzzle, heart rate sitting at 75bpm during start of puzzle. Heart rate seemed to average around 82bpm when Sachin struggled to find correct puzzle pieces. Was still responsive to staff after seeming silent. 6.30pm Meds we’re self administered. Heart rate still sitting at 81bpm Sachin needed prompting to have his drink, seemed more fixated on his puzzle. Sachin still fixated on his puzzle, needed extra prompting to finish his tea. Saj assisted Sachin with puzzle, heart rate sat around 70bpm. Pack up time, heart rate sitting around 80 bpm. Sachin went to toilet after prompting, small to medium output. Support worker:- James.

Mood/Behaviour: Sachin was a bit on the vocal side tonight, he made a lot of “eeaww” noises. He wasn’t loud and what he uttered was kind of under his breath. Sachin appeared to stall at times, he’d be walking or doing some task and then would freeze for a few seconds. Sachin responded well to verbal and physical stimulus to snap him out of these moments. NOTES: Sachin And I started off our night with a walk in the neighbourhood where he lives, Sachin appeared eager tonight. At times he was walking faster encouraging me to keep up. We returned home and Sachin was assisted with removing his walking shoes and brace then into slippers. Sachin was encouraged to use the toilet (he produced a large urine) and assisted brushing his teeth. We then made cups of tea, Sachin was pre-empting my requirements for tea ingredients tonight, there was little I had to ask for because he had it all ready. We then went to the programs room and had some fun with the jigsaw for a while. Sachin lately has been wanting help, accepting my involvement and waiting patiently for me to help put the piece he finds into the spot. A big change from not wanting me to help at all. Sachin had his medication at 630, dispensed by his father. At 730 we packed up the programs room and settled down in the lounge for a bit of TV time. Shift finished at 2000 hours. 6hrs 17 minutes sleep 65 rhr Weather 14° clear Activities: No walk due to risk level, straight into Program room to start on puzzle, teeth brushed Progress Notes: Sachin relaxing in the lounge on arrival and due to high risk Saj has given Sachin Endep at 5.10pm ,Sachin was rhen Toileted passing a moderate amount of urine then headed into bathroom to brush teeth Sachin was a little shakey ,we then grabbed our cups of tea and made our way into the program room music put on and difuzer on and applied some Lavender oil inside his nostrils then got started on the puzzle, Sachin made great progress early but eventually ground to a halt showing no interest 6.30pm meds given by Saj then at 6.33pm Sachin had a small seizure lasting about 30 seconds Luckily Saj caught it quickly and helped support Sachin through it ,Preveena also came in to assist with recovery which was almost instant and was given time to settle then cut puzzle time and let Sachin relax in the lounge and during seizure his hr spiked to 123 bpm and whilst on the couch Sachin was quite fidgety then getting up several times going into kitchen filling up his tea cup with water straight out of the kettle drinking it and going in and out of the pantry ,Preveena then took Sachin to the toilet at about 7.30pm and got him to sit down but wasn't interested in doing anything then got up and made his way onto Saj and Preveenas room but wasn't in there long hr has been steady between 75 and 80 bpm even though not relaxed shift ended at 8pm Support worker:- James.

Mood/Behaviour: Sachin was quiet tonight, he was well behaved and posed no trouble to work with. Sachin was in a easy going mood and did not complain to instructions. NOTES: Sachin And I went for a walk to the end of the street he lives in tonight, he kept a casual pace not willing to speed up. He appeared bored at times and appeared happier when he was at home. When We returned home and Sachin was encouraged to use the toilet (he produced a large urine) and assisted brushing his teeth, Sachin had himself a small go after I completed the bulk work. We made cups of tea and then went to the programs room and knocked over a whole horizontal line during the time. Sachin had his medication at 630, dispensed by his father. At 730 we packed up the programs room and went for another toilet break, medium urine out put. We then settled down in the lounge for a bit of TV time, no issues. Shift finished at 2000 hours. 4hrs 30 minutes sleep 67rhr Weather 11° mostly sunny Activities: No walk tonight due to high risk day Toileted, teeth brushed,puzzle room to work on puzzle Progress Notes: Arrived at 5pm to commence work with Sachin who was standing in the kitchen and was straight away taken to the toilet but wasn't having anything to do with it, then from there straight into the bathroom to assist with brushing his teeth ,several times had to speak with Sachin as he was blank staring, and once completed we grabbed our teas and headed into the Activities room and let Sachin get to work on his puzzle, We also had a shadow shift with us tonight Luke who Sachin took an instant shine to and was only too happy to let Luke assist with the puzzle pieces, we didn't go for a walk tonight due to it being a high risk day soft music was put on and difuzer put on Sachins hr sat between 75 and 80 bpm throughout and a few times Sachin was staring into space but was quite responsive to staff when spoken to, tonight Sachin didn't appear to be to interested in the puzzle and lost interest pretty quickly ,medication was given at 6.30pm by Saj and was Toileted again at 6.40pm with a moderate output we then went back to the Activities room but Sachin had lost all interest so we packed everything up and made our way into the lounge room at 7.30 pm to relax with Preveena until shift ended at 8pm. 11hrs 24 minutes sleep 65bpm rhr Weather 12° clear Activities: Short walk up Cambridge drive to Yale CRT and return , puzzle work In program room Teeth brushed, toilet at 510pm but not interested. Progress Notes: Sachin was quite relaxed and up beat tonight ,Toileted at 5.10 pm and passed a small amount of urine then back into the lounge to put his shoes on to go for a walk heading up Cambridge drive to Yale Court and return ,Sachin was extremely eager tonight and was happy to set a good steady pace an dat times was quite vocal. When we got back we went straight into the bathroom to brush his teeth then grabbed our cups of tea and made our way into the program room and got to workbon his puzzle,Sachin was very slow to get going and appeared to get stuck and required some assistance which he was ok with.Sachins hr was sitting between 80 bpm and 85 bpm and seems to have lost interest in puzzle and was just happy to sit and drink his tea ,Toileted again at 7pm passing a small amount of urine. Medication was given by Sajit at 6.30pm we eventually finished up at 7.30 pm and made our way into the lounge room where Sachin was happy to sit and relax next to Preveena on the couch until end of shift at 8pm . Seizure risk level:- Risk level- max. Previous night Sleep:- 6:33 hours split sleep. Resting heart rate:- 63 resting bpm. ACTIVITIES. Jigsaw. WEATHER: 8 degrees, overcast + showers day. Mood/Behaviour: Sachin was quiet tonight, possibly because of his seizure at 4:55 pm, he did appear tired afterwards, Sachin was compliant with requests and in general well behaved. NOTES: Sachin was at the kitchen table with his mother and father when I arrived for shift, Sajit said to me Sachin just had a seizure a few minutes ago, and was coming out of it. Sachin was seated at the table so I tool up a seat next to him and monitored him until he was ready to move to the lounge. Sachin was assisted to the couch by his father and myself, Sachin had his 6:30 medications dispensed by his father. At 7:00 Sachin appeared more awake and focused so we went to the programs room for a bit of jigsaw work. Sachin appeared eager to work but was taking his time with a bit of procrastinating, we did manage to finish a section that appeared to be annoying him. At 7:50 we packed up and returned to the lounge room and settled in watching ninja warrior with his mother. Sachin had a sleep tea and snack during. Tonight was accomplished at sachins pace, all when he was ready. Shift finished at 2030 hours. |
