Walk Puzzle TV Show WEATHER: 15 degrees - mostly cloudy Sachin was with praveena on the couch when I walked in. Sach and I immediately prepared to go for a walk and he appeared eager to go. We walked to the second bridge and back. Sachins pace at the beginning was great and kept a slower, but still steady, pace on the way back. Sach took himself to toilet when we got back (small output) and we then ate dinner together. Sachin appeared to enjoy his left over chicken curry very much. I helped Sach clean his teeth and then we made teas and headed to do some puzzle. Despite him needing a few reminders to focus, him constantly trying to take his gloves off, and a slightly elevated heart rate (av. 85bpm) - Sachin was finding pieces at lightening speed and completed nearly four rows! We finished at 1930 and I prompted Sach to the toilet (medium output). He then got a 30 minute coconut oil head massage from mum - during this his heart rate settled to mid 70’s and stayed steady for rest of the evening as we settled on the couch with Praveena, tea and snacks.

Walk. Movies. WEATHER: 17° Sunny day. NOTES: Sachin was chilling out on his couch in the lounge when I arrived. I assisted Sachin put his joggers on then we went for a walk. Sachin set the speed today, a slow to medium speed. We walked 3 kms in about 48 minutes, quiet slow. Sachin had his dinner when we returned home. After dinner I encouraged sachin to use the toilet, he didn’t need to go so we brushed teeth instead. I completed the first proper clean then Sachin had a go himself. Sachin and I worked together and made cups of tea, we took them to the lounge and settled in for Sunday night Movies. We finished Indiana Jones the quest for the holy grail, then started kingdom of the crystal skull. Sachin was calm and had a good bpm, he appeared happy with the film choice. Shift ceased at 2130. Friday the 28th July 2023. ACTIVITIES. Drive Walk Shopping Jigsaw Movies. WEATHER: 14° overcast day. NOTES: Sachin was home when I arrived, he was showered and dressed. Sachin and I went for a drive to fuel up the family car then to ALDI for shopping. Sachin followed me around aldi without issues, he pushed the trolley and avoided other shoppers perfectly. We returned home and went for a 20 minute walk along Tabletop road, consider Sachin has had two weeks off he walked well. Sachin had his dinner then used the toilet for a large urine. I assisted Sachin brush his teeth then we made cups of tea, Sachin and I went to the programs room. Sachin worked very well on his puzzle tonight, he was quick at finding pieces and was not overly concerned with perfection of slotting pieces in. Sachin completed 1 & 1/2 lines tonight. At 1930 hours we called it quits and had another toilet break, medium output. Sachin finished off his night with a bowl of snacks, cup of sleep tea and movies. Sachin has had a very high heart rate all night, 95 average bpm regardless of what we tried to lower it. Shift ceased at 2130. Support worker:- Katy

Walk Puzzle TV Show WEATHER: 15 degrees - sunny Upon arrival, Sachin was in the programs room, sitting at the table and already doing some of his puzzle. I prompted him to get up so we could go for a walk. We only went to the end of the street, but Sachin was walking very slowly, so it took us longer than 10 mins. Once back, he wanted to go straight back into doing the puzzle, but I prompted him to couch to sit with mum, where we stayed until dinner. We ate together and once done, I prompted Sach to the toilet (med/large output) and I assisted with cleaning his teeth. He appeared distracted during these tasks. We made teas and headed to programs room - Sachin immediately got into the puzzle. He became very focused and completed more than 4 lines! His heart rate was between 85-95 to start, but after the first half hour, settled to between 75-85. At 7:30pm, we finished our teas and I promoted Sachin to toilet again (med output). We sat on couch with Praveena - it took a while for his heart rate to settle - but Sach appeared happy and calm for the rest of the evening with snacks and teas. Saj mentioned that sachin’s foot was swollen so I gave him a quick massage with voltaren cream. Sachin appeared to enjoy it. Support worker:- James

ACTIVITIES. Tv time. Socialising with guests. Walk. Jigsaw time. WEATHER: 10° Rain day. NOTES: Sachin was at home when I arrived, he already had his breakfast and pot of tea. Once Sachin had finished his pot of tea I assisted him into his joggers and jacket, we exited the house and went for a walk. We had a good break in the weather and with no rain we managed a long walk along Tabletop road. Sachin appeared into the walk today, he walked fast, almost excited, and at times I had to slow the pace so he wouldn’t get puffed out. He completed a 35 minute walk. We returned home and I assisted him out of his joggers and jacket, back into his slippers. Sachin then emptied his tea pot and put new bags in ready for tomorrow. Sachin and I waited in the lounge for dinner to be ready, we watched who wants to be a millionaire on TV and observed guests. Sachin had his dinner with family and guests then used the toilet, large urine, and brushed teeth. We made cups of tea and went to the programs room for jigsaw time. Sachin was struggling tonight, he is at a point of doing the outer edges and it is cloudy sky, he is finding it difficult to join the correct pieces. Sachin had 25 minutes of jigsaw time with hardly any success and we rejoined his family and friends in the lounge for TV time. We made new cups of tea at 2010 hours and a snack of corn chips while watching YouTube fiji clips. Sachin has appeared to have a good night despite the jigsaw fiasco, a good bpm and attitude. ACTIVITIES. Tv time. Socialising with guests. Jenga blocks, dexterity exercise. WEATHER: 11° Rain day. NOTES: Sachin was at home when I arrived, he already had his breakfast and pot of tea. I spent some time with Sachin at the kitchen table constructing objects and towers with Jenga blocks. Sachin tried to be gentle but he was shaky with his hands. Sachin had his dinner and just as he finished some family friends arrived from out of town. Sachin went through some greetings then he was encouraged to use the toilet, no production. I assisted Sachin brush his teeth then we acquired cups of tea his mother made for us and we went to the lounge to socialise with guests. At 1840 hours Sachin was encouraged to use the toilet once more, he didn’t go. So far Sachin has been behaved and has had a decent BPM below 84. Sachin appeared to accept company well, he didn’t display any signs of agitation, he appeared content sitting in his spot watching family and friends. Toilet breaks at 2000 and 2100 hours, large and medium urine output. Sachin appeared to enjoy his night, no incidents to report. Shift ceased at 2130. Support worker:- Katy

Puzzle TV Show WEATHER: 11 degrees - sunny/cloudy Sachin was finishing off his tea when I arrived, I sat with him- he seemed to be a bit zoned out, especially when we were making his tea for tomorrow. We ate dinner together and I prompted Sach to toilet - med/large output. I cleaned his teeth - he was fighting me with his checks while I was trying to get to the back of his mouth. We prepared teas and went to programs room. Sachin quickly finished off the puzzle and he put in quite a few of the pieces in the last row himself. We began the next puzzle. The pieces weren’t clicking together as well as the last puzzle, so it took some adjusting for Sach - but he got used to it and was allowing me to help place pieces without them clicking. His heart rate was steady during this process. We came out at 1930 and relaxed on couch until 2000, when I made tea and prompted Sach to toilet - med output. We got snacks and sat back on couch. Sach had a steady heart rate for the evening - but appeared to be distracted so I kept a close eye on him. Support worker:- James

ACTIVITIES. Tv time. Walk, Aurora light show. WEATHER: 14° Sunny day. NOTES: Sachin was having a shower when I arrived, he was being assisted by his mother. Sachin was encouraged to use the toilet, he didn’t go. Sachin, his mother and I drove to MacDonalds and had dinner, Sachin had fries and nuggets. Sachin was encouraged to use the toilet while there, he produced a large urine. We left MacDonalds and went to the Botanic Gardens for the Aurora light show. Sachin appeared to enjoy the walk and sights, he paid particularly long attention to the 3d images and not much interest to the lazer lights or fiber optics. We returned home and sachin changed into tracksuits, we made cups of sleep tea and settled in the lounge watch the rest of Alien 3. At 2000 hours Sachin was encouraged to use the toilet, small urine produced. We made more cups of tea, sleep tea blend this time. Sachin and I returned to the couch and started watching Alien Resurrection. Sachin appeared to have a good night, he displayed good behaviour and had a good bpm all night. Shift ceased at 2130. |
