New 1008 piece puzzle started

Previous night Sleep:- 6.58 hours sleep.
Resting heart rate:- 59 resting bpm.
 
ACTIVITIES.
Walk.
Jigsaw puzzle work.
 
WEATHER:
20 degrees, partly cloudy.
 
Mood:
Sachin appeared happy today, he was dedicated with his puzzle work and succeeded in finishing.
 
NOTES:
Sachin used the toilet straight away, he did not go, Sachin was then assisted with brushing his teeth.
Sachin and I walked from his house and around the neighbouring streets today. Sachin had 1070 steps on his Fitbit at the start and 4345 at the end, Sachin did  3275 steps in 31 minutes.
Sachin and I had our cups of tea made for us when we returned and then we went to the programs room for jigsaw puzzle time.
Sachin had his medication at 1830, administered by his father.
Sachin finished his puzzle tonight and started a new one, he managed to finish the boarder.
At 1945 hours we packed up and went to the kitchen, we made up new cups of tea and went to the lounge room.
Sachin had a head massage from his nan.
 
Shift ceased at 2100 hours

Weather 14° cloudy 

Toileted at 5.17pm medium output

Activities:
Walk up Table top road to second bridge and return slow pace due to risk level completing 3916 steps avg hr 88 bpm taking 36 minutes, brushed his teeth no wobble Board tonight Program room to work on his jigsaw puzzle.

Progress notes:
Arrived at 4.55pm to start shift with Sachin who was still eating and was toileted at 5.17pm medium output then got ready for our walk heading up Table top road to second bridge and return not really pushing him to hard lots of vocalising and laughing brushed his teeth when we got back then went into the program room music put on Lavender difuzer put on along with Lavender oil on his top lip Heart rate was a little higher than normal not dropping below 70 bpm but was making great progress on his jigsaw as he was close to finishing it, still appeared quite happy and relaxed ,Sachin required several promts to turn his light off and head into the lounge to relax on the couch at 7.30pm staff stayed back til 9pm to monitor Sachin no issues .

Weather 14 degrees- partly cloudy
Toileted when I arrived  -  very small urine output. Toileted again after puzzle room - medium urine output. 


Activities: 
Walking around Cambridge Drive and then home.
2,497 steps complete avg hr 90 bpm 
Wobble board not completed. Puzzle room to work through Sachin’s jigsaw. 




Progress notes: 
Greeted Sachin’s family at 5pm. Sarj was in the kitchen and Sachin was finishing up eating his dinner. Saj and I chatted as it had been 2 weeks since we had seen each other. Pravena toileted Sachin for me and it took him about 5 minutes to produce a small urine output as he had just gone before his shower. I assisted Sachin in washing his hands and brushing his teeth. I went in to get Sachin organised for his walk and put his brace, shoes, gloves and jacket on. Sachin was full of energy this evening and we walked around the Cambridge block. Sachin completed around 2,497 steps and his heart rate averaged around 90 bpm. We kept a steady pace and Sachin was keeping up well and fist bumping me every now and then. We arrived back home and got our cups of tea and went in to the puzzle room. We turned the lamp on, diffuser and calming music. I applied the lavender to Sachin’s upper lip every 20 minutes.  Sachin was quite stuck tonight on his puzzle and his heart rate was fluctuating between 75-80. This was due to not being able to proceed with his puzzle. He kept putting the right pieces together but then quickly taking them apart. Sachin wasn’t very keen on me helping him tonight. 6:30pm meds administered by Saj. Sachin drank his tea and was ready to pack up by 7:10pm but I convinced him to keep going for a few more minutes. We proceeded to go out to the lounge room at 7:30pm. Pravena toileted Sachin again for me where he then produced a medium size urine output. We sat on the couch and watched a movie with Sachin’s grandmother and mother. Sachin was very unsettled and kept getting up and down. He was waiting for his next cup of tea. Saj put that on for him and Sachin calmed down on the couch with his blanket waiting patiently for his tea. I said goodbye and see you next week.

Happy and energetic, at times a little clumsy.

1717, Walk up and back Table top rd. 3731 steps in 38 minutes at a steady pace.
1802, Took Sachin to the toilet, medium urine.
1808, Organised weekly medication with Sachin’s assistance.
1830, Gave regular medication to Sachin and he self administered.
1836, Assisted in brushing Sachin’s teeth, whitening agent used at Sajit’s request.
1840, Moved activities room to work on current Almalfi Italy puzzle. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip to help relaxation.
1934, Took Sachin to the toilet, medium urine.
1938, Moved to family room for sleep tea and videos on the iPad.

Happy but distracted

Walk along Cambridge Dr to park and back for 24 minutes avg hr 95 bpm total steps 2738
Brushed his teeth no wobble Board tonight
Program room to work on his jigsaw puzzle.

Progress notes:
Sachin was standing in the kitchen when I arrived tonight and straight away got him ready for his walk due to a break in the weather walking up and down Cambridge Dr to park and back for 24 minutes pace was pretty relaxed due to risk level lots of vocalising and some moderate laughing toileted when we got back only small output then brushed his teeth and straight into program room to work on his jigsaw music put on along with Lavender difuzer oil put on his top lip every half hour Sachins hr good tonight sitting between 60bpm and 65 bpm not making a lot of progress tonight but very calm and relaxed and lots of good positive interaction laughing and fist bumping finished up at 7.30pm and toileted again large output, Sachin grabbed some tea bags and put them into the teapot for tomorrow then took Preveenas hair band from her wrist and put it in the bathroom now just relaxing quietly in the lounge until the end of shift.

Activities:

Very short walk along Cambridge Dr to park and back 887 steps in total avg hr 92 bpm 14 minutes duration. Walk kept short due to high risk.
Went off to have a shower
Program room to work on his jigsaw puzzle no wobble Board.

Progress notes:
Sachin at high risk today and looking at times a little confused, toileted straight away at 5.10 pm large output then went out for a very short up Cambridge Dr to park and back only once 5.30pm medication given then went with Preveena for a shower then when he came out Sachin took himself to the toilet again large output then we headed into the program room to work on his puzzle music on Lavender difuzer put on along with some Lavender oil on his top lip,Sachin didn't really appear interested at all was more interested in sorting out pieces his heart rate also was bouncing around quite alot and Remained alot higher than normal sometimes going above 85 bpm then dropped to 63 then back up to 75 and evened out at 73 ,Saj came in and distracted Sachin with some cd sorting which had an immediate impact dropping his hr below 70 we then packed up and headed into the lounge Saj gave medication extra at 7.30pm then prompted Sachin to use the toilet again large output now just relaxing on the couch ,Sachins hr continually monitored until 9pm and still remained quite high .

1835, Moved activities room to work on current puzzle. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip to help relaxation.
1930, Extra medication administered due to high risk day.
1932, Moved to family room for sleep tea and videos on the iPad.
1945, Took Sachin to the toilet.
2000, Head massage from grandmother to aid in relaxation and promote restful sleep. ​

Jigsaw puzzle work.
 
WEATHER:
15 degrees, partly cloudy.
 
Mood:
Sachin appeared to be in a good calm mood today, he was very easy going. Happy, content and smiled a lot.
 
NOTES:
Sachin used the toilet straight after tea, he produced large urine. Sachin was then assisted with brushing his teeth.
Sachin and I went for a walk in his own suburb today. Sachin completed 3622 steps in 35 minutes.
Sachin and I took cups of tea to the programs room and continued jigsaw work.
Sachin had his medication at 1830.
Sachin gave his puzzle good attention this evening, he focused really well.
At 1935 hours we packed up and came out to the lounge and watched TV with family.
Shift ceased at 2000 hours.

Well Rested

ACTIVITIES.
Walk.
Jigsaw puzzle work.
 
WEATHER:
14 degrees, Sunny day, cool wind.
 
Mood:
Sachin appeared well rested today, he was well behaved as always, compliant with tasks and minimal vocal noises.
 
NOTES:
Sachin and I went for a walk along Tabletop road to the second bridge, Sachin appeared eager to walk, straight away setting a good decent pace. We listened to Luke combs music as we walked. Sachin completed 4226 steps in 37 minutes.
Sachin was taken to the toilet when we returned, he produced a large urine.
Sachin was assisted with brushing his teeth then we took our cuppas into the program room and kicked off jigsaw time.
Sachin showed excellent attention to it straight away, not needing any assistance from me, he appeared quite content on his own.
Sachin had his medication at 1830.
Sachin was consistent with his heart rate all evening during jigsaw time, he maintained 68 to 73 bpms.
At 1940 hours we packed up and came out to the lounge.
Sachin finished off his night with TV time.
 
Shift ceased at 2000 hours.
