Seizure risk level:- Risk level- min. Previous night Sleep:- 8:34 hours sleep. Resting heart rate:- 63 resting bpm. Shift times :- 1700hrs to 2000hrs. ACTIVITIES. Jigsaw. Walk. WEATHER: 13 degrees, overcast day. NOTES: Sachin and I kicked of the nights routines with a toilet break, medium urine output then I assisted him brush his,teeth. We then got set up in walking shoes and jumpers for our nightly walk. Sachin and I took it easy tonight, just 3 laps of the block, 24 minutes worth, Sachin kept a good steady pace and appeared to enjoy it. When we returned home Sachin and I made up cups of tea and set off to work in the programs room on the 500 piece past and present puzzle. This puzzle is of 1800’s London, when it’s completed you can then scratch off a layer to reveal present London. Sachin really went to town and put quite a lot together, no procrastination from him tonight, just straight into piece after piece. Medication dispensed at 1800 hours by Sajit. Sachin and I then found a few puzzles and put command strip’s on the backs to hang them up, we measured out and marked the walls then hung the pictures. At 1900 hours sach was taken to the toilet by his father, medium urine out put. At 1930 we packed up and went to the lounge to watch TV with family. Shift finished at 2000 hours.

Seizure risk level:- Risk level- min. Previous night Sleep:- 10:25 hours sleep. Resting heart rate:- 64 resting bpm. Shift times :- 1700hrs to 2000hrs. ACTIVITIES. Jigsaw. Walk. WEATHER: 13 degrees, overcast rainy day. NOTES: Sachin was in a go slow tonight so we went at his pace and still managed to complete our full night routine. Sachin was encouraged to use the toilet, he did not need to go so we moved on to brushing teeth. We then got into walking shows and left the house for our evening walk. Sachin has been displaying strange behaviour the last few times we’ve gone walking, he gets a frowny face half way along, tonight I stopped and tried to find a trigger but was unsuccessful. I gave Sachin the choice of directions, left, right , go on or go back to house. Sachin chose to go on to the end of the street, as normal. Sachin used the toilet when we returned, we then made our cups of tea and went to work in the programs room. Sachin is doing a easy jigsaw this time, it is a 500 piece and a scratchy puzzle at that, much the same as Oz lotto scratchies. This is a land scape past to present style of 1800’s London which you scratch off to reveal 2000’s London. Sachin appears to be liking this one and is fairly quick in putting pieces together, sometimes asking for my help as well. At 1930 we packed up the programs room, had another toilet break, medium urine, then had a seat in the lounge watching TV with mum. Support worker Steve Risk level minimal
8hrs 25 minutes sleep split 66 rhr Weather 16 ° party cloudy ️ Activities: Walk up Cambridge drive to Harvard Street and back, puzzle Room to work on puzzle, toilet Progress Notes: Sachin was having his dinner when I arrived tonight so just waiting till he finished then got Sachin ready for his walk and headed down Cambridge drive and got to near the park when Sachin began crying so we just stopped for a minute whilst I gave him plenty of reassurance then continued up to Harvard Street then turned around making our way back at his pace giving Sachin lots of positive praise we then grabbed our cups of tea and made our way into the program room to get to work on his new puzzle which Sachin was really excited about and was making great progress and was quite well focused stopped at 6pm to take his meds from Saj then continued on with his puzzle putting all the edges together, Sachin taken to the toilet at 7pm extremely large urine output then got abit distracted going into the laundry to check the washing then eventually headed back to program room and worked on jigsaw till we packed up at 7.30pm ,heart rate sat between 75 to 85 bpm for most of the session probably due to excitement with new puzzle now relaxing in the lounge till end of shift at 8pm. WEATHER: 15 degrees, sunny day. NOTES: Sachin and I walked the whole length of Cambridge drive for a walk tonight, Sachin was compliant and appeared to enjoy getting out. Sachin appeared happy and well rested, he made vocal noises that sounded happy. Sachin was encouraged to use the toilet when we returned ( he did not pass urine ) sachin was then assisted with brushing his teeth. Together we made cups of tea, as usual I took the lead and Sachin assisted with acquiring the ingredients. Sachin works quiet well now with getting what I point and indicate to. We then made our way to the programs room for puzzle time. At 1800 hours I ushered Sachin to the toilet, he felt the need and passed a medium amount of urine this time. Sachin was possessed tonight, he went ahead and put so much together, one after the other with no pause, like a machine. Sachin finished 2,1/2 lines tonight. At 720 we packed up the programs room, had another toilet break, medium urine, then had a seat in the lounge watching TV with mum. Support Worker: Luke Corkett
Debrief with Saj, discussed seizure activity and change of medication. I got stats from IPad, resting heart rate was 66bpm. I prompted Sachin to use the toilet, there was no urine output. I assisted with brushing teeth. We moved to program room. Sachin turned his light on and started his puzzle. Heart rate was sitting around 90bpm. Sachin kept going into the next row of his puzzle, heart rate sitting at 87bpm. I collated pieces of the next puzzle. 6.00pm Saj administered meds for Sachin. I prompted him to use the toilet shortly after. He had a medium output. Sachin returned to puzzle, heart rate was at 96bpm. Sachin’s heart rate dropped to 67 bpm while continuing puzzle. Prompted Sachin to pack up and use toilet again. Heart rate was down to 75 bpm. Sachin used the toilet and produced a medium output. Support worker Steve. Risk minimal
10 hrs 19 minutes sleep 66 rhr Weather 17° overcast Activities: No walk due to weather teethbrushed, puzzle room to work on puzzle Progress Notes: Arrived at 5pm to start shift with Sachin who was still eating his dinner so just had a chat and a catch up with Saj and Preveena until he was finished, didn't require toileting due to going earlier so only had to assist with brushing his teeth no walk tonight due to weather conditions so grabbed our teas and headed into the Activities room where Sachin was eager to get into it. Sachins hr remained quite high throughout the session sitting between 90 and 98 bpm meds given St 6pm via Saj ,Sachin continued along making great progress myself and Saj attempted to assist with puzzle pieces but Sachin wasn't having a barr of it at all , Saj took Sachin to the toilet at 7pm whilst I was checking puzzle pieces for his new puzzle and produced a large output. We then finished up at 7.30pm and headed into the lounge room so Sachin could relax on the couch with Preveena for remainder of shift ,Sachins hr was sitting between 85 and 90 bpm and continued to monitor Sachin till end of shift at 8pm . NOTES: Sachin was finishing off his pot of tea when I arrived, once he was finished we went for a walk around the block. 15 minutes. Sachin used the toilet when we returned (large urine) and was assisted brushing his teeth. We then made up our cups of tea and started work on the puzzle, Sachin was focused tonight. Sachin finished 1,1/2 lines tonight, he even got me to help out, a pleasant change. At 730 we packed up and went to the toilet one last time (medium urine) before catching a movie with mum. Shift finished at 2100 hours. |
