Rainy But Still A Good Day

• Activities:
Continued to work current puzzle. PWalk up Table top rd. Organised weekly medication.


• Mood:
Alert and calm with good engagement for the full routine.


• Weather:
16c Cloudy.

1710, Took Sachin to the toilet but he did not go.
1720, Walk up and back Table top rd and back, 3145 steps in 30 minutes at a brisk pace.
1800, Organised weekly medication with Sachin’s assistance.
1830, Medication given to Sachin and he self administered.
1832,Took Sachin to the toilet, medium urine.
1834, Assisted in brushing Sachin’s teeth.
1836, Moved activities room to work on current puzzle, showed good interest and made great progress. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip to help relaxation.
1930, Took Sachin to the toilet.
1932, Moved to family room for sleep tea and videos on the iPad.

Sleep deprived

Toileted before shower. Toileted again after walk. Large urine output 


Activities: 
Walking up and back Cambridge drive. 
975 steps complete avg hr 91 bpm in 11 minutes. 
Wobble board not completed. Puzzle room to work through Sachin’s jigsaw. 




Progress notes: 
Greeted Sachin’s family at 1700. Sachin appeared to be in a cheeky mood this evening. He was ignoring me when I was saying hello. I organised Sachin’s shoes for our walk but he had already gone and sat down in his puzzle room and started to work on his puzzle. It took a lot more encouraging then usual to get Sachin for his walk. We set off with the umbrella but only lasted 11 minutes. Sachin was very vocal on our walk this evening. Completing 975 steps as Sachin was very lethargic due to not enough sleep. Once arriving home Sachin went to the toilet and produced a large urine output. I assisted him to wash his hands and brush his teeth. We received our tea and headed into the puzzle room. Lavender applied to Sachin’s upper lip area a few more times then usual due to risk level. Extra Meds given tonight at 1800,1830,1930,2100. Sachin worked through his puzzle and was very focused but was getting stuck along the way. He would not allow me to help him. Sachin’s heart rate was reaching 80 a few times tonight but came down after some time. At 1900 Sachin became very uninterested in his puzzle tonight and was swinging back and fourth on his chair. Sachin was able to talk to his brother on the phone tonight. Sachin needed a lot of reminding to drink his tea whilst we were in the puzzle room tonight. At 1930 Sachin had his medication and then we headed into the family room. Sachin enjoyed another cup of tea and a bowel of corn chips whilst watching tv with his family. Last lot of medications given at 2100. I said goodbye and I would see them on Friday. Shift ended at 2100.

Back on track.

• Mood:
Well rested, a little defiant overall positive.

22c mostly sunny.

• Notes:
1705, Took Sachin to the toilet but he did not go.
1710, Jumped in the car to drive to Brooklyn Fields estate.
1717, Walk around Brooklyn Fields estate, 2829 steps in 30 minutes at a steady pace.
1758, Took Sachin to the toilet but he did not go.
1800, Assisted in brushing Sachin's teeth, whitening agent used at the request of Sajit.
1808, Moved to activities room to work on current puzzle. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip to help relaxation.
1830, Gave medication to Sachin and he self administered.
1930, Moved to family room for sleep tea ad videos on the iPad.

Toileted at 5.05pm but didn't go

Activities:

Good walk along table top road to second bridge and back setting a good pace taking 34 minutes completing 2.74 km total steps 3806 avg hr 86 bpm ,teeth brushed and toileted again passing small amount, program room to begin on puzzle


Progress notes:
Sachin was already in the program room working on his puzzle when I arrived tonight and required several promts to come out toilet was attempted but wasn't interested so headed off for our walk up Table top road to second bridge and back setting a good pace little bit of vocalising to start walk completed in 34 minutes Teeth brushed when we got back and toileted again passing small amount of urine grabbed our teas and headed into the program room where Sachin got stuck in hr was a little higher than normal sitting between 75 and 80 bpm but slowly came down medication given at 6.30.pm, Sachin ground to a halt so Saj gave him some more puzzle pieces and continued until 7.30pm then packed up and went into the lounge room to relax for remainder of the evening.

Slow and steady

Toileted when I arrived but didn’t go as he had been before shower. 


Activities: 
Walking around the whole of Cambridge Drive and then home.
3,033 steps complete avg hr 96 bpm in 32 minutes. 
Wobble board not completed. Puzzle room to work through Sachin’s jigsaw. 




Progress notes: 
Greeted Sachin’s family at 5pm. Sachin was in the dining room eating his dinner with his mother and grandmother. Sachin was very quiet this evening. It took him a little longer then usual to eat his dinner. Saj toiled Sachin but he didn’t go, Pravena then let me know he had just been toileted before his shower. I brushed Sachin’s teeth and we set off for our walk at 5:30pm. We walked our usual walk. Up and around Cambridge for 32 minutes. Sachin wasnt sure if he wanted to keep a steady pace or if he wanted to walk a little slower then usual. Sachin completed 3,033 steps on our walk. We arrived back home at 6pm and received our tea then headed into the puzzle room. I noticed Sachin had started his new puzzle and made some good progress over the last 4 days. Sachin was very interested and eager to get his puzzle done and wouldn’t allow anyone to help. Sachins heart rate spiked between 75 and 77 for most of the time in here. Saj administered 6:30pm meds and Sachin took them. Sachins focus was incredible tonight and he made some really good progress. Lights off at 7:35pm and into the lounge-room to relax. Sachin sat on the lounge and was very calm tonight. I said goodbye. shift ended at 8pm.

Indecisive and cheeky.

• Notes:
1700, Sachin sat at the dinning table for eat his dinner, pork stir fry and finish his morning cup of tea.
1728, Organised weekly medication with Sachin's assistance.
1826, Took Sachin to the toilet.
1830, Gave regular and extra medication to Sachin and he self administered.
1832, Assisted in brushing Sachin's teeth. 
1836, Moved to activities room to work on current puzzle. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip to help relaxation.
1930, Extra medication given to Sachin and he self administered.
1942, Moved to family room for sleep tea and videos on the iPad.
1945, Head massage from grandmother to try and promote relaxation and restful sleep.
2032, Took Sachin to the toilet but he did not go.

ACTIVITIES.
Walk.
Jigsaw puzzle work.
 
WEATHER:
15 degrees, sunny day.
 
Mood:
Sachin appeared more calm and normal today, he needed more time processing before actions, he would sit paused for a few seconds before moving.
He was limited on his vocal noises. Sachin focused well on his puzzle.
 
NOTES:
Sachin used the toilet at start of shift, medium urine and then i assisted him brushing his teeth.
Sachin and I walked in his own suburb tonight, Sachin was keeping a good pace with me but called it quits after 26 minutes., completing 2725 steps.
We made up cups of tea, we took them and went to the programs room for jigsaw puzzle time.
Sachin had his medication at 1830, administered by his father.
Sachin achieved alot tonight with his puzzle.
At 1930 hours we packed up and went to the lounge where we watched TV with mum and nan.
Shift ceased at 2100 hours.

Active Sach!

ACTIVITIES.
Walk.
Jigsaw puzzle work.
Drive.
 
WEATHER:
14 degrees, clear day, cold wind.
 
Mood:
Sachin appeared well rested today, almost too rested!
Sachin was very vocal and energetic today, he wouldn’t sit still for too long. Sachin only half focused on his Jigsaw puzzle and the rest was looking around the room at everyone and anything else.
 
NOTES:
Sachin used the toilet at start of shift, large urine and then we brushed teeth straight after.
Sachin and I drove to Brooklyn fields and walked did our route through the streets, Sachin was very vocal.
For the 30 minute walk Sachin racked up 3312 steps.
We made up cups of tea, we took them and went to the programs room for jigsaw puzzle time.
Sachin had his medication at 1830, administered by his father.
Sachin was very energetic and did not give his puzzle 100% tonight, he did not focus to well getting distracted often.
At 1930 hours we packed up and went to the lounge, Sachin did not sit still, and wandered the kitchen paying attention to his father carving pork roast.
Sachin was taken to the toilet once more and did produce medium urine content.
Shift ceased at 2000 hours.

Activities:
Good walk along table top road to second bridge and back taking 33 minutes 44 seconds, 2.73 km ,4085 steps completed set a pretty good pace teeth brushed then into program room to work on his jigsaw

Progress notes:
Arrived at Cambridge to start work with Sachin who was still eating his dinner so had to wait attempted to toilet Sachin to no avail headed out for our walk heading up Table top road to second bridge and back setting a good pace very quiet and no vocalising brushed his teeth when we got back and was eager to get into program room as he was starting on a new puzzle at 6.17 pm but needed some assistance as puzzle wasn't meeting up wouldn't come out at 7.30pm until all edges were done with a huge smile on his face and was now happy to relax in the lounge til end of shift .

Toileted when I arrived but didn’t go
Toileted again upon arrival home after walk and still didn’t go 
Toileted during puzzle time with a small urine output  
Activities: 
Walking around the whole of Cambridge Drive and then home.
2,644 steps complete avg hr 101 bpm in 27 minutes.  
Wobble board not completed. Puzzle room to work through Sachin’s jigsaw. 




Progress notes: 
Greeted Sachin’s family at 5pm. Sachin was in the dining room drinking tea when I arrived. Sachin was very vocal this afternoon. I waited until he had finished to organise for our walk. Saj tried to toilet Sachin but he did not go. I assisted Sachin in putting his ankle brace and shoes on then we set off for our walk. Sachin was very fast tonight. Steps starting at 545 and ending at 3,189. 2,644 steps complete. I tried to direct Sachin around Oxford drive which goes around Cambridge to make our walk a little longer as he had an adequate amount of sleep and lots of energy but Sachin was pushing my hand to keep going the direction we were in which was down Cambridge drive. We ended up having to cut our walk at 27 minutes which was a shame as it started to rain. We arrived home and Pravena tried to toilet Sachin again which resulted in still no output. I washed his hands and brushed his teeth. We got our cups of tea and headed into the puzzle room. During the time Sachin eagerly completed his puzzle I sorted out the next puzzle from where James left off. At 6:30pm Saj bought in Sachin’s meds and Sachin took them. At 7pm Sachin became very agitated and he took his cup out to the kitchen. He was bouncing around making a lot of noise. Saj tried to toilet him but he did not go. After 5 mins Pravena tried again to toilet Sachin and he produced a very small urine output. Sachin headed back into the puzzle room and I continued to sort the pieces. I counted 1,014 pieces. At 7:30 I turned off the music, lamp and diffuser assuming we would be heading into the lounge-room to relax with Sachin’s mum and grandmother but Sachin wouldn’t budge so we stayed in the puzzle room and Sachin continued on with the puzzle while I wrote my notes. At 7:45pm Sachin got up and decided to go out to the lounge-room. I turned off the lamp and joined him. Sachin was standing in the corner of the kitchen watching everyone being quite vocal at times. Saj started the sleep tea and Sachin was bouncing around making noises. 8pm came around and I said goodbye and that I would see the family next week. Shift ended at 8pm.
