Activities:
Toileted - small output
Brushed teeth
No walk today
Programs room, to work on puzzle
Staff (Keely) organised new puzzle with Saj
Sach relaxed on couch with Praveena

Arrived at 5pm, conversed with Saj. Informed that Sachin had long but broken sleep. Sachin was relaxing on couch with a cup of tea. Praveena had informed me that he had a blackout today around 4:15pm. I sorted pieces of a new puzzle with Saj - giving Sachin some more time to relax before getting into his routine. At approximately 5:40pm Saj and myself made our way inside.
We prompted Sachin to use the toilet - small output.
Sachin was a bit wobbly on his feet during his short walk to bathroom, as well as slightly unbalanced while I brushed his teeth. Once finished we collected his tea off Saj & progressed to the programs room. As it had just gone 6pm Saj gave medication.
Sachin was relatively quite with me to start with and appeared flustered with his puzzle.
As time passed he settled & was happily allowing me to assist with his puzzle. Sachin was prompted to finish up in the program room and use the toilet. Saj and myself were in agreeance that Sach was enjoying his puzzle - so extra time was allowed. We finished in the programs room closer to 8pm.
The last roughly 1hr of the shift Sachin relaxed on couch watching TV with Praveena.
Heart rate fluctuating high (84-104bPm) at beginning of shift. Came down as shift progressed.
Overall good shift.

Shadow Shift With Keely

Activities:
Toilet small output, teeth brushed,short walk up Cambridge drive to the park and next street around corner ,program room to work on his puzzle.


Progress Notes:
Arrived at 5pm to commence shift with Sachin and was accompanied by shadow shift staff Keely had a brief chat and introduction with Saj then headed inside to greet Sachin,took him to the toilet and only passed a small amount of urine,short walk tonight given risk level and completed 1155 steps at a cracking pace then brushed his teeth even we arrived backand ran Keely through the process we then grabbed our cuppas and headed into the Activities room and showed Keely our routine,putting on soft music Lavender difuzer put oil inside his nostrils and let Sachin get to work on his puzzle Sachin really seemed to enjoy Keelys company and were both interacting very well with Sachin deliberately pulling pieces off and waiting for Keely to snap them into place Sachins hr stayed within acceptable range 75 to 85 bpm we finished up at 7.30pm and Toileted Sachin again with moderate output and relaxed on the couch with Praveena till shift close at 9pm .

A simple night in

17° overcast rain day.
 
NOTES:
Sachin was inside his house at the dinner table when I arrived, finishing off his evening meal.
Short hand over from Sajit conducted, he informed Mr Sachin has had plenty of sleep and appears fine, his resting BPM is lower than yesterday.
Sachin finished off his tea and was encouraged to use the toilet, Sachin did not go so we moved on to brushing teeth. I assisted Sachin by brushing first and getting all his teeth done then Sachin had himself a go.
I assisted Sachin prepare for a walk, fitted his ankle support bracelet and walking shoes. We waited for a break in the rain and went for a short walk up and down the street out front of his home. Sachin carried his umbrella just in case, he appeared to enjoy his time out evident by a smile he was constantly wearing.
We returned home and I encouraged sachin to use the toilet, this time he did go, medium urine out put.
Sachin assisted me this time making cups of tea, he acquired the items we needed, I poured the water and he put the items back where they go, we took our cups to the programs room.
Sachin was focusing well with his jigsaw puzzle tonight, minimal encouragement required and minimal procrastinating from him, he did loose focus towards the last 25 minutes but he remained receptive to encouragement to keep going.
At 730 pm we moved to the lounge where he watched TV with his mother.
Nil issues and a good night.
 
Shift finished at 2000 hours.

Evening Drive.

Shift times :- 1700 hrs to 2000 hrs.
ACTIVITIES.
Jigsaw.
Car ride.
 
WEATHER:
17° overcast day.
 
NOTES:
Sachin was inside his house at the dinner table when I arrived, his father said Sachin and myself can come for a car ride to the petrol station.
Sachin was prepared for his car ride, he was encouraged to use the toilet, small urine output, sachin was assisted with brushing his teeth. We exited the house to the car and once Sachin realised we were going for a drive he got himself in the back seat and did up his own belt, he appeared excited.
Sachin was behaved for the trip to and fro, he also did not display any anxiety or concern moments.
When we returned home Sachin and i made cups of tea and went to work in the programs room, sachin focused well on his puzzle and put a fair amount together without help.
At 1930 hours we packed up and went to the toilet, Sachin produced an unknown amount because his father took him. Sachin and I finished off the night watching a little TV with mum.
No issues or negative behaviours to report, Sachin appeared to have a good night.

Happy Sachin!

Arrived at approx 17:00pm, clocked in, walked thru the side gate and was met with Saj. Had a quick catch up conversation then walked inside and was met with Praveena and Sachin. Sach had finished his dinner and was standing in the kitchen, I put my hand out for a fist pump, he gave me pump them chuckled. I then prompted Sachin to sit on the couch so I could put on his shoes and brace for our walk. It took him a few moments to sit on the couch, he was distracted with his tea pot and I think he really liked his dinner (wanting more). I prompted again and he happily sat on the couch, I fully assisted putting on his walking shoes and ankle crave on his left ankle. Left for our way, Sachin led the pace tonight, we walked really quickly, he seemed to really enjoy being out this afternoon. Nice warmish weather. He giggled and chucked the whole way, really alert and happy. Did a very quick walk (due to the fast pace we were waking). Arrived back home, prompted Sachin to sit back down on the couch so I could take off his shoes. Changed him into his slippers and out of his walking shoes off, ankle brace and shoes back in spare room. Then I prompted Sachin to come and brush his teeth, he was slightly hesitant then with mums encouragement he quickly followed me. He went straight to the toilet but no urine output (he had gone earlier during shower time). Next I fully assisted brushing Sachins teeth, rinsed with water then got our teas and into programs room. I prompted Sachin to turn on his lamp which he did, I turned on defuser (checked water level), stereo and started my notes. Sachin was really giggly and making lots of joyful noises tonight, very alert and happy. He got right into his puzzle (I took a before and after picture of his progression this evening). He required me assistance with his puzzle. He competed x4 pieces in a row, then removed them and waited for me to put them in for him. He made happy noise once I did so. I thought I found a piece (it wasn’t the right piece) and he laughed at me because it was wrong lol. I applied lavender oil to the inside of his nostrils x3 times and disposed of q tip appropriately. Saj administered Sachins 18:00pm medication and helped us find a piece we were stuck on. Sachin and I completed a big section on his puzzle (not going horizontally at the moment, in clusters of colours/adding to the building on the jigsaw puzzle). He continued to allow me to put pieces in place for him, however if not done correcting (making the clicking sound, I have to try until it’s just right for him! A sign he trusts me. He was in a really good mood this evening, happy to see me I think and the feeling was mutual. I encouraged Sachin to drink his tea which he did so happily. Saj took him too the toilet at approx 18:45pm and he produced medium urine output. At approx 19:30pm I encouraged Sachin and I to pack up in programs room, he turned off his lamp and I turned off defuser, stereo and light. He was really into his puzzle at the stage, Saj prompted him to finish his tea and finish in programs room. We exited program room and Sachin organised his teapot for tomorrow, cleaning the outside of pot very carefully with a tea towel. Then he snuggled up on the couch with his blanket and his iPad, with Praveena and watched some tv. I sat and ate a delicious meal and then spoke with Praveena. It started to rain heavily with occasional thunder. Sachin peeped out the window to see after the thunder, he sat contently and happily with the both of us in lounge room. Praveena make Sachin a cup of tea and got him a little snack. At approx 21:00pm I said goodbye and left.

Me and my umbrella.

WEATHER:
18° overcast misty rain day.
 
NOTES:
Sachin was inside his house when I arrived finishing off a cup of tea from dinner. Once he had finished he was encouraged to use the toilet, Sachin produced a medium amount of urine. Sachin was assisted with brushing his teeth, as usual I did the bulk and Sachin finished off with a play.
Sachin and I went for a quick walk, it was raining very very slightly but we took umbrellas non the less, Sachin was shown how to hold his umbrella, he also watched me and mimicked my movements, for his first time using an umbrella on his own he succeeded. Walk lasted 15 minutes.
When we returned to the house Sachin and I made up cups of tea and went to work in the program room. Sachin started off slow but soon was flying with the pieces, his focus was excellent, Sachin appeared rested and alert at this stage. Sachin put a good portion together and by 1930 hours he was satisfied and ready to call it quits. At 1930 hours we packed up and went to the toilet one last time, medium urine produced then settled on the couch with mum and watched TV. This evening was a success for Sachin, he used his own umbrella and progressed well with his puzzle. No issues or negative behaviours to report.
Shift finished at 2100 hours.

Giggly Sachin

Arrived at approx 17:00pm, clocked in, walked in through the side gate and was met with Saj. All was well, he informed me that Sachin was well rested, he had had 10 hours plus, of broken sleep. I walked inside and said hey to Sachin, his face lit up when he saw me. I gave him a fist pump and he giggled. I sat with him at the dining table, he had finished his dinner and had half of his tea left. He seemed well, more colour in his face today. After dinner I took him to the toilet, I turned on the tap to prompt him. He had no urine output. Next I fully assisted in putting on his walking shoes and ankle brace on left ankle. Sachin seemed eager to leave, he waited at the sliding door for me, I encouraged him to come with me through the garage door. He quickly followed me through the door. We then departed for our evening walk. Sachin opened the gate and led the way, he put his arm out for me to link his arm and we started walking. Moderate pace today, Sachin led the way. He was giggling and making noises while we walked, he seemed happy to be out of the house. We walked too the end of the street then back home. Stopped and waited for passing cars. Upon arrival back home, Sachin sat down and I took off his shoes and his ankle brace. I then prompted him to brush his teeth but he sat on the couch picking at his lips. I applied some paw paw ointment to help sooth his dry lips. I tried to get him to come with me a brush his teeth however he wasn’t interested. I checked his bpm on Fitbit app and was sitting on 88mbp, 62resting. Praveena got his teapot ready and he shot up to do that task. He put his tea bag into the water then wiped off the pot with a tea towel. Then I prompted him to brush his teeth. He walked straight into the toilet and produces low urine output. Then I fully assisted brushing his teeth, x5 rinses with water. We then got our teas and made our way into programs room. I turned on the defuser and stereo and prompted Sachin to turn on his lamp which he did. I took a progress picture of his puzzle. Sachin got right into it, sorting through the pieces. He required me assistance and waited for me to place a few pieces in its correct spot, but of course has to make the right “clicking” sounds. This took a few attempts. I prompted him to drink his tea. I applied lavender oil multiple times to the inside of his nostrils. I think he comfortable having me around, makes me happy. Saj administered his 6pm medication. I helped sachin as much as I could/as much as he’d allow me to help him. I’d find pieces of the puzzles, put them together then he’d pull the pieces apart. He completed a small section of his puzzle in bottom right hand corner of his puzzle. I took a progress picture of his puzzle. Then at approx 19:30pm we packed up in programs room, sachin turned off his lamp then I turned off defuser and stereo. We then made our way into the lounge room. He snuggled up on the couch under his blanket and watched some tv and ate some chips. He kept giggling. I sat and ate a lovely meal, lovely chats with Saj and Praveena. At approx 21:00pm I said goodbye, clocked out and left

Absentee Seizure

Arrived at approx 17:01pm, clocked in and was met with Saj in the driveway. Had a quick catch up conversation with Saj and then walked inside. Sachin was sitting at the dining table finishing his dinner. I sat with him at the table and gave him a fist pump. His face lit up, I think he was happy to see me. Saj informed me that Sachin had only 6.5 hours sleep last night, he was at a high risk state. Next Saj took Sachin too the toilet, no urine output however he did flush the toilet. Next I encouraged Sachin to sit on the couch then I fully assisted putting on his ankle brace on let ankle and then his walking shoes. We then departed for our walk, Sachin let the pace. He was laughing and making lots of noises like he was trying to talk. I talked to him while we walked. Told him that I was happy to see him and that I had missed him. Told him about birds and the pretty sky. He had tears in his but kept laughing, he seemed well. We walked too the end of the street, turned around and came home. It was really nice because Sachin missed out on his walk the last few days. Upon arrival home, I fully assisted in talking off Sachins shoes and ankle brace, then put his slippers back on. After fully brushing his teeth, I gave him water and he rinsed his mouth and spat in sink. Then got his tea and I got mine and we made our way into the programs room. I turned on light, defuser and stereo(soothing Music). Sachin turned on his lamp and continued with his puzzle. I took a progressional picture to track his progress. Saj entered a few times to assist with puzzles. Sachin required my help putting pieces into its place, if I didn’t do it properly with the correct “clicking” sounds he’d take it out for me to try again till I got it right. Sometimes he would laugh if I didn’t put it in correctly. I sat with him and observed his progress. I applied lavender oil multiple times(high risk day) to the inside of his nostrils with a q tip. At approx 19:00pm, I glanced at Sachin and he was spacing out. I snapped my fingers and waved my hand in front of his face, he was going into an absentee seizure. I quickly called for help. Praveena came running to assist, Saj didn’t hear me but came running also when he heard Praveena get up. Saj held Sachin upright in his chair while I applied a decent amount of lavender oil under his nose. I fully assisted in laying sachin onto the floor and into a recovery position. Few minutes later, he then snapped out of it. It lasted approx 40-60seconds in total. I sat with Sach on the floor, rubbed his arm and assured him he was safe and that mum and I were here. Approx 10 minutes later Saj took Sachin into the lounge room. I packed up the programs room, did my notes and then sat with them in the lounge room. Sachin was still spacey but seemed to be doing better. We all had lovely conversations and I sat and had a delicious meal! I took a progressional picture of Sachins puzzle. Said goodbye, clocked off and left at approx 21:00pm
