ACTIVITIES. Jigsaw puzzles. Movie time. WEATHER: 21° sunny day. NOTES: When I arrived sachins Father asked me to get Sachin ready for a drive, I assisted Sachin into shoes. We went for a quick 7 minute walk in the street out front of his home then we went for a drive to the local iga. Sachin walked with me while his father shopped. Sachin had his dinner when we returned home then a toilet break of large amount urine, I also assisted Sachin with brushing his teeth. Sachin and I made cups of tea together then commenced working on his jigsaw. So far Sachin has been great, his mood and heart rate exceptional. Sachin did not have any problems while working at his puzzle, he found pieces fast and accurately although asked me for help to put them in. We packed up at 1930 hours and had another toilet break ( medium urine amount ), Sachin took his seat on the couch and watched Jack Ryan tv show with his mother. A cup of tea and a bowl of dorito chips at 2000 hours saw Sachin through to 2130 hours. Sachin had no issues or concerning actions tonight, he appeared well rested and happy. Shift ceased at 2130 hours.

Seizure risk level:- Risk level- Medium/Max. Previous night Sleep:- 13:20 hrs sleep. Resting heart rate:- 66 resting bpm. Shift Times, Date :- 1700 hrs to 2130 hrs. Thursday the 28th September 2023. ACTIVITIES. Jigsaw puzzles. Movie time. WEATHER: 26° sunny day. NOTES: Sachin was at the dinner table having his dinner when I arrived, Sachin gave me a fist bump and went back to eating, and drinking his pot of tea. Sachin was very shaky in his arm movements, you could be mistaken for thinking he was shivering, he was unstable on his feet. Sachin skipped his toilet break, didn’t need to go, and I then assisted him brush his teeth. Sachin assisted me make cups of tea, he was skipping steps and getting confused, it appears yesterday’s Seizure is taking its toll on his cognitive ability and motor movements. Sachin found pieces very fast and he was correct every time, he struggled with fine precise hand movements to slide them in, he appeared to be getting frustrated so I assisted him tonight with putting them in to relieve his stress. 1938 hours we packed up and exited the program room, Sachin had a toilet break with a large urine amount produced then took his seat next to his mother. At 2000 hours Sachin had his sleep tea and small bowl of doritos snack. Jack Ryan was on TV and Sachin wasn’t interested in watching it so he used his ipad and scrolled YouTube Warner brose cartoons, he appeared content to do this, his bpm was low 70’s. At 2045 hours Sachin put his ipad down and started watching TV, 2110 hours he became fixated on finding lint on his electric blanket, picking pieces, this pushed his bpm to mid 90’s, 2130 hours he dropped this fixation and his bpm to mid 80’s. ACTIVITIES. Jigsaw puzzles. Movie time. WEATHER: 26° sunny day. NOTES: Sachin was at the dinner table having his dinner when I arrived, Sachin appeared normal, behaviourally speaking, he did appear a bit slow to comprehend instructions but we continued on. After Dinner Sachin had a toilet break, medium urine amount produced, then I assisted him brush his teeth. Sachin and I made cups of tea and headed to the programs room for puzzle time. We skipped the walk today on account of his low hours of sleep and his rising resting bpm of the last few days. Sachin was focusing well on his task, he did appear to be having difficulty at one point finding pieces, this turned out to be missing pieces. We eventually found a tray of missing pieces and Sachin took off putting pieces together. After the stall of missing pieces Sachin still managed to put a good amount together. At 1930 hours we packed up and Sachin had another toilet break, large amount of urine produced this time. Sachin sat in front of his mother in the lounge and received a head massage from his mother. At approximately 2003 hours Sachin was still having a head massage and he had a seizure, his mother cradled him and applied lavender oil to her hand for him to breath in, I uncrossed his legs out straight and maintained physical contact to ease Sachin. Approximately 40 seconds after his seizure started he relaxed and it stopped. I remained seated on the ground with Sachin until he was ready to get up, approx 20 minutes. Sachin was assisted to his feet by myself and his father, sachin then stepped backwards and sat in his spot on the couch. I kept a vigilant supervision on Sachin for the remainder of the Shift to assure he wasn’t staring off. Shift ceased at 2130 hours. Support worker:- James
Seizure risk level:- Risk level- Minimum. Previous night Sleep:- 10:45 hrs sleep. Resting heart rate:- 63 resting bpm. Shift Times, Date :- 1600 hrs to 2130 hrs. Tuesday the 26th September 2023. ACTIVITIES. Walk. Jigsaw puzzles. Movie time. WEATHER: 26° sunny day. NOTES: Sachin was at the dinner table chilling out when I arrived, he had finished drinking his tea pot and was watching his mother work in the kitchen. Sachin greeted me with a brief smile and fist bump, he appeared rested and happy. I encouraged sachin to leave the dinner table then I assisted him prepare his tea pot ready for tomorrow, we then stuck into the programs room for jigsaw time while we waited for dinner to be ready. Approx 20 minutes later we were called out for dinner, Sachin attended the table with haste and ate heartedly, he appears to really enjoy venison curry. Straight after dinner Sachin and I saddled up our walking shoes and headed off for a walk. Sachin and I walked down a farmers access dirt road that’s opposite his suburb. Sachin was surrounded by gum trees and cows in paddocks, he appeared to enjoy it and even had a faster walking speed tonight, 33 minutes of exercise. Back home we made cups of tea together and he had a toilet break ( medium ) and was assisted with brushing his teeth. We returned to the programs room for more puzzle time. Sachin put 14 pieces together, he took his time and displayed good focus to the job. Sachin did have difficulty with perfection, each piece had to be perfectly slid into place otherwise he would pull it out and start again, sometimes this would happen 6 to 12 times. At 1930 hours we packed up and exited the programs room, Sachin had a toilet break ( again medium amount ) then we sat in the lounge to watch my kitchen rules. At 2000 hours Sachin and I made cups of tea and also a small bowl of doritos for his snack, we returned to the lounge and my kitchen rules. Shift ceased at 2130 hours. Support worker:- James

ACTIVITIES. Drive. Walk. Movie time. WEATHER: 22° sunny day. NOTES: Sachin was sitting on the lounge couch when I arrived, I assisted him into his joggers and we departed for a walk. Sachin and I stayed in the suburb putting in 34 minutes of exercise at a good walking speed. Sachin had a very small toilet break at home, then he had his dinner. Sachin appeared to really enjoy his venison curry and dhal. After dinner Sachin used the toilet, again a small urine. I assisted Sachin to brush his teeth, we then made cups of tea and sat in the lounge. Sachin watched TV for approximately half hour then we accompanied his father on a drive to fuel up his car. Sachin giggled a few times on the drive, I’m assuming he had a good time with seeing the night lights. We returned home and settled into the couch. Sachin watched a Adam Sandler film ‘murder mystery 2’ with his mother. At 2000 hours Sachin’s bpm rose to triple digits, we applied some lavender oil and a camomile tea to drink, he lowered down to high 70’s approximately 40 minutes later. Sachin has appeared to have a good night with no real issues. Shift ceased at 2130 hours. Support worker:- James

Wednesday the 20th September 2023. ACTIVITIES. Drive to and waking in ALDI. Jigsaw. Movie time. WEATHER: 20° partly cloudy day. NOTES: Sachin was having his shower, being assisted with his shower routine by his mother when I arrived. Sachin had his pot of tea and breakfast straight after his shower. Sachin greeted me with a fist bump and giggled when he saw me, he appears to be in a good mood and well rested. I encouraged Sachin to use the toilet, he tried but didn’t need to go, I then assisted Sachin with brushing his teeth then handed him the tooth brush so he could finish off. Sachin and I then went straight into the programs room for jigsaw time. We didn’t have long in there, approximately 30 minutes work time before Sachins Father entered and requested we get ready for a drive. Sachin did manage to complete Half a horizontal line. I assisted Sachin into his walking shoes then encouraged him to use the toilet again, still no urine produced. Sachin and I then got into the car and left with his father to ALDI’s. Sachin appeared to enjoy the drive. At ALDI Sachin took hold of my arm and we followed his father around, Sachin was shivering at times, it was cool and Sachin did behave himself very well. We returned home to a successful toilet break (medium size), I made Sachin his cup of tea and sat in the lounge with him. Sachin had a small bowl of trail mix for a snack. Sachin watched home & away then my kitchen rules. Sachin sat contently in his spot, he appeared at ease, his heart rate was very good and low, high 60’s – low 70’s. Shift ceased at 2130 hours. ACTIVITIES. Jigsaw Movie time. WEATHER: 29° hot sunny day. NOTES: Sachin was hanging around his mother in the kitchen, together getting his pot of tea ready for tomorrow as I arrived. I assisted his mother and father by cooking dinner tonight, Sachin had his dinner when it was ready. Sachin cleared his plate and went for seconds. Sachin used the toilet for a large urine then brushed his teeth, I assisted and made sure they all were cleaned. Sachin and I made cups of tea and then hooked into his jigsaw puzzle in the programs room. So far Sachin has been alert and appeared happy. Sachin had been working well, he had completed more than usual. At 1904 hours I noticed Sachin staring straight ahead, I rubbed his arm and touched his chin, Sachin gave no response to my touch. I called for saj and praveena to assist, I got a pillow on the ground and moved his chair to give us access to lift him to the floor, Sachin appeared to snap out of it in about 20 seconds, I positioned myself under his right arm ready to move him, praveena entered and started rubbing lavender oil on his shirt to breath. Sachin came around in seconds, he drank his cup of tea and exited the program room. Sachin had a toilet break, small urine produced. He took up his seat on the lounge next to his mother and watched TV. Sachins bpm settled to mid 70’s. At 2000 hours I made sachin a cup of chamomile tea, he sat calmly and watched my kitchen rules. ACTIVITIES. Movies, At home. Walk. Drive to bunnings, and walk around. WEATHER: 25° warm sunny day. NOTES: Sachin had finished his pot of tea as I arrived. I assisted him into shoes and we then went for a walk. It was warm and sunny, Sachin was making a good speed and appeared to be enjoying his walk. Throughout the 36 minute walk Sachin appeared to walk with purpose. Back at home Sachin was encouraged to use the toilet, he didn’t need to go. Sachin had his dinner and cleared his plate to the sink, I encouraged him to try the toilet again, this time he did go, large urine. I assisted Sachin brush his teeth then we accompanied his father on a drive to bunnings. Sachin and I walked a few isles while his father shopped and when he was done we met him at the car. We all returned home and made cups of tea, we settled into the couch and watched a movie called 65. Sachin appeared interested in it, he would occasionally glance at the TV and then the kitchen. At 2000 hours we made another cup of tea and returned to the couch. At approximately 2040 hours I noticed Sachin staring at me, looking through me, I observed his ipad BPM was high, 125. I alerted his mother and she confirmed Sachin was dazed. I put lavender oil under his nose and held my fist up for a bump to provoke a response from him, he did respond and fist bumped me, Sachin came out of this daze in seconds. Sachins mother and I continued interaction until his BPM dropped to 70’s. I handed Sachin his personal ipad when he was better. Sachin continued on his night with no effects from the daze and also no further concerns. Shift ceased at 2130 hours. Support worker:- Katy

Walk Puzzle TV show WEATHER: 24 degrees - sunny Sach was relaxing on the couch when I arrived. We decided that a short walk would be best today and headed out. Sachin clearly wanted to go for a longer walk, but was still happy to be out. We got back and I helped Sach get prepped for a shower, praveena took over and I went to sort out more of the next puzzle. Once out, Sach and I ate dinner together. We cleaned up and I washed Sachin’s hands and brushed his teeth. We made teas together and headed to programme’s room. Sach was a bit distracted to begin with and zoned out a couple times. After 1800, however, he settled in and began getting pieces in at a faster rate. His heart rate became good as well (60s and 70s). We finished up at 1930 and headed to couch to relax. I made Twas and got snacks at 2000, he was happy and relaxed. I prompted him to toilet at 2045 - after fiddling around with the toilet roll he went (large output). We watched Jack Ryan until shift end - Sach appeared very happy. |
