Weather 21 degrees- windy
Toileted before shower. Toileted after walk. Medium urine output. 
Activities: 
Walked to the park, swung on the swings for 20 minutes and then walked home. 1,022 steps completed. 


Progress notes: 
Greeted Sachin’s family at 1700. Sachin was sitting at the dining table when I arrived eating his dinner. Sachin appeared to be taking his time today and Praveena mentioned he was slower doing shower routine this afternoon. We started our routine at 1800. I assisted Sachin in putting his shoes on and we headed off for our short walk. We walked down to the park and Sachin swung on the swings. Sachin was being very cheeky at the park and would only swing on the swing if I was next to him swinging on the baby one that I could hardly fit in. Sachin was very vocal this afternoon, having random laughing outbursts. We returned home just in time for 1830 meds. 1,022 steps completed on our short walk. Meds given and taken at 1830. Saj toileted Sachin and he produced a medium trine output. We didn’t end up getting to the puzzle room until 1845. Sachin appeared very calm this evening, with his heart rate staying in the 60’s. He made good progress on his puzzle. Teeth brushed after puzzle time at 1930 and then into the family room to watch tv. Sachin sat at the dining table drinking his tea. At approx 1945 Sachin disappeared into the bedroom. He was waiting for Praveena to get out of the shower. Shift ended at 2000.

warm spring day

Continued to work on Melbourne puzzle. Walk up Table top Rd. Organised weekly medication.

• Notes:
1707, Took Sachin to the toilet but he did not go.
1715,  Walk up Table to rd and back, 2897 steps in 35 minutes at a steady pace.
1800, Sajit gave Sachin a shower.
1810, Organised weekly medication with Sachin's assistance.
1830, Medication given to Sachin and he self administered.
1840, Took Sachin to the toilet.
1842, Brushed Sachin's teeth and used the whitening agent at Sajit's request.
1846, Moved to activities room to work on Melbourne skyline puzzle. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. 
1940, Moved to family room for sleep tea and video on the iPad.

Activities: 
Walked to the park, sat on the swings for 10 minutes but wasn’t interested in swinging. Walk up Cambridge Drive and back.  1,874 steps completed. 


Progress notes: 
Greeted Sachin’s family at 1700. Sachin was sitting at the dining table when I arrived. Toileted before shower time so I assisted Sachin in brushing his teeth and putting his walking shoes on. We headed off on our walk and stopped by the park. Sachin sat on the swings but didn’t use them. We sat at the park for approx. 10 minutes but Sachin wasn’t interested in using the swing. I guided Sachin back on to our walking track. We walked for 23 minutes and Sachin completed 1,874 steps. Upon home arrival Sachin went straight to the puzzle room after I room his shoes off. He seemed eager to get back into his puzzle. Sachin worked through his puzzle well this afternoon with great concentration. I noticed Sachin’s heart rate jumped when I tried to help him do some pieces so I stopped and let him go. 1830 meds given and take. He started approx. 7 different parts of the puzzle tonight to which he then pulled all of them apart. Sachin then began disengaged in the puzzle, swinging on his chair. He then started back again after 5 minutes but wouldn’t allow me to help. 1930 into family room to relax. Sachin went to the toilet and produced a medium urine output. Sachin sat and watched YouTube on his ipad. He then started to make his belly wobble so he could hear the liquid in it move around. I said goodbye. Shift ended at 2000.

To much noise!

ACTIVITIES.
Walk.
Jigsaw puzzle work.
 
WEATHER:
22 degrees, Sunny.
 
Mood:
Sachin appeared ok at start of shift, he smiled during our evening walk and made what appeared to me as happy sounds. Sachin had a moment of “out of the normal” behaviour when we returned from walking, he covered his ears with his hands and appeared to ignore myself and family.
 
NOTES:
Sachin was encouraged to use the toilet at the start of shift, he declined the use.
I then assisted Sachin brushing his teeth.
Sachin and I drove to Brooklyn fields today, we used the swing set there for 10 minutes, Sachin swung and propelled himself well.
We then walked along streets with large trees using the shade from the sun. Sachin done 3339 steps.
While driving back to the house at approximately 500 metres away he covered his ears and went quiet, the car radio was on gently playing will Smith, sachins mother later told me he appears to be not fond of will.
Sachin continued covering his ears in the house, he sat and ignored us for 20 minutes. I made Sachin a cuppa, this seemed to snap him out of it and we continued on with our routine.
We took our teas and went to the programs room for jigsaw puzzle time.
Sachin had his medication and a panadol (for a possible headache) at 1830, administered by his father.
At 1930 hours we packed up and went to the lounge where we watched TV with mum and nan.
Shift ceased at 2000 hours.

Toilet attempted at 5.15pm but not interested

Activities:
Walk up Table top road to second bridge and back completing 3822 steps Brushed his teeth then into program room to work on his jigsaw puzzle

Progress notes:
Sachin was finishing up his dinner when I arrived tonight so waited till he was done toilet attempted at 5.15pm but wasn't interested so got ready for our walk Saj instructed me to use my own discretion on walk heading up Table top road to second bridge and back setting a pretty good pace fair bit of heavy breathing but didn't appear to be too worried teeth brushed when we got back then grabbed our teas and headed into the activities room to start work on his jigsaw, Sachin extremely focussed to start with hr was a little high at first sitting in the high 70 range but quickly settled and sitting at 60 to 65 bpm ,Sachin was pretty focussed on jigsaw making good progress but slowly started getting disinterested normal medication was given at 6.30.pm and Sachin decided to call it quits at 7.10pm switched off his light and made his way into the lounge toilet attempted again but again not interested and was hovering around the kitchen ,several times Sachins hr spiking to 94 bpm but that was due to him looking for Preveena now Sachin is relaxing on the couch and his hr sitting at or just below 60bpm .

Activities: 
Walked up and down Cambridge drive for 10 mins. 866 steps complete. Short walk due to lack of sleep and it was hot. 


Progress notes: 
Greeted Sachin’s family at 1700. Sachin was hanging around the kitchen when I arrived. Toilet before shower, so I assisted him with brushing his teeth. Sachin appeared to be disinterested in our walk this afternoon, it took a lot of prompting to get Sachin going. We set off for our walk and only made it down Cambridge and back once due to lack of sleep and the weather being hot and Sachin was wearing his jumper and track pants. 866 steps complete in 10 minutes. We arrived home and headed into the puzzle room. Extra Epilim 200mg received at 1730 and taken. Sach’s heart rate was very high tonight. Reaching the mid to high 90’s. Sachin was working on different pieces of the puzzle through out the time and kept pulling them apart. All and some extra precautions taken today due to lack of sleep and risk level being high. Once Sachin got going with his puzzle his heart rate started to go down to mid 70’s. 1830 meds given and taken. Sachin completed a nice chunk of his puzzle tonight after he was focused. Into the family room at 1930. Another extra Epilim 200mg given at 1930 and taken. Sachin was quite calm sitting and watching tv. Saj toileted Sachin at approx. 2000 with some extra promoting and a medium urine output. Sleep tea made at 2010 and Sachin sat on the couch and drank the tea. Sachin ate some corn chips his favourite snack. I said goodbye. Shift ended at 2100.

Tired but call and a little bit cheeky at times.

1709, Walk up Cambridge dr. and back, 1009 steps in 13 minutes at a very relaxed pace due to hear and lack of sleep.
1727, Organised weekly medication with Sachin's assistance.
1734, Extra medication given to Sachin and he self administered.
1758, Took Sachin to the toilet but he did not go.
1802, Assisted in brushing Sachin's teeth, whitening agent used at the request of Sajit.
1804, Moved to activities room to work on Melbourne skyline puzzle. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every half an hour to help relaxation and reduce heart rate.
1830, Gave regular medication to Sachin and he self administered.
1935, Moved to family room for sleep tea and videos on the iPad.
1940, Extra medication given to Sachin and he self administered.
1952, Head massage from Grandmother.
2040, Took Sachin to the toilet, medium urine.

ACTIVITIES.
Walk.
Jigsaw puzzle work.
 
WEATHER:
23degrees, Sunny.
 
Mood:
Sachin displayed very much the same mood he had yesterday, he appeared easy and went with the flow until the last hour of shift, during that hour he lost interest in our puzzle and wanted to just chill on the couch.
 
NOTES:
Sachin was encouraged to use the toilet at the start of shift, he declined.
I then assisted Sachin brushing his teeth.
Sachin and I drove to Brooklyn fields today, 8nsted of walking the streets there we walked back to Tabletop road and walked back to the second bridge from the other direction.
When we got back to the house We made up cups of tea we took them and went to the programs room for jigsaw puzzle time.
Sachin had his medication at 1830, administered by his father.
Before Sachin went into the programs room we swapped his busted jigsaw for another exactly the same, Sachin did not appear interested in the puzzle, he didnot want to start again.
He took lots of encouragement to get started, he put 20 or so edge pieces together then pulled them all apart at 1930 hours.
At 1930 hours we packed up and went to the lounge where we watched TV with mum and nan.
Shift ceased at 2000 hours.

Appointment with OT

ACTIVITIES.
Walk.
Jigsaw puzzle work.
 
WEATHER:
20 degrees, partly cloudy.
 
Mood:
Sachin appeared calm and normal at start of shift, as the time went on he started displaying agitation, I believe triggered by his jigsaw puzzle having broken pieces.
 
NOTES:
Sachin used the toilet at start of shift, Large urine and then i assisted him brushing his teeth.
Sachin and I walked down to the second bridge on Tabletop road we did a 41 minute walk completing 4323 steps.
We made up cups of tea, we took them and went to the programs room for jigsaw puzzle time.
Sachin had his medication at 1830, administered by his father.
Sachin did not  achieved much tonight with his puzzle, there was many torn and bent pieces and this caused Sachin to lose interest.
At 1930 hours we packed up and went to the lounge where we watched TV with mum and nan.
Shift ceased at 2000 hours.

Emotional but calm new puzzle

Progress notes
Sachin was sitting in the lounge room when I arrived for shift tonight Saj stated that he had a Seizure during the night and just to keep an eye on him on the walk and use my own judgement, toileted Sachin at 5.10pm medium output then got ready for our walk and headed up Cambridge drive to park and had to  ut it short as I felt Sachin jerking as he was holding my arm and as I stopped and looked at Sachin he had tears rolling down his face and his bottom lip was quivering so we stopped for a minute to give him a chance to settle and was headed straight back to the house Sachin was quite unsteady on his feet so I got him to sit down on the couch for a minute until he settled then took him into the bathroom to brush his teeth then grabbed our teas and headed into activities room to start work on a new puzzle Soft music put on Lavender difuzer put on along with Lavender oil on his top lip and got to work on the edges whilst Saj and myself sorted out the rest of the pieces so not a whole lot of progress made Sachins hr was pretty goodsitting between60 and 65 bpm ,Sachin headed out of Activities room just after 7pm but came back in at 7.15pm for another go and required several promts to finish up at 7.30pm now just relaxing in the lounge room on the couch with Preveena til end of shift.
