Greeted and met Olivia.
Went and checked the previous notes.
Ran through routine.

Sachin grabbed tea entered room started puzzle, turned on the light, music and the diffuser.  Also applied the lavender oil and spray
Ran over “bpm”.
Sachin is very vocal today and in a good mood.

Feeling a bit "off"

Activities: Puzzle, went for a walk, watched a movie with mum.
 
Sachin was at table eating dinner when I arrived. I sat with him for a while, we had a slow start to the night. Saj had mentioned that Praveena feels Sachin may be a bit "off"/unwell possibly - Sachin does appear to not be feeling like his usual self tonight. He had his meds around 6:05pm - we organised tea + headed to the programs room + got setup and started on the puzzle.
Sachin seemed very distracted during puzzle time tonight and appeared to be "zoning out" quite a lot during puzzle time tonight - not like the way he would usually when stuck on his puzzle, requiring me to rub him on the shoulder and call his name to regain his attention.
 
He was very slow with finding pieces and sorting through boxes more than usual, only to go back to the first he had looked at.
Took Sachin for a short walk up the road + back @ 7:15pm, 438 steps - home 5 mins later. I encouraged him to sit on the couch - took runners off, placed slippers back on + encouraged Sach to move onto the couch next to Praveena. Gave Sach his iPad, he put YouTube on and say and relaxed next to mum for a little while, whilst she watched TV.
Saj toileted Sach @ 7:40pm - medium output and put a jumped on him and offered him a beanie to put on instead of his cap.
Once finished, Praveen had picked a movie to watch (Avatar), which Sachin actively watched with her.
Overall not a bad shift, despite how "off" Sach appeared. Be began to seem closer to his usual self more toward the end of the shift, more vocal out on the couch with mum.
Heartrate stayed below 70bpm for the majority of the shift, only going higher when evidently stressed or when moving around.

Holding Hands

Activities: Puzzle, walk and relaxing on couch
 
Sach was at kitchen bench when I arrived, but went back to relax in couch with Praveena shortly after. We got set-up and started in the programs room around 5:30pm. At 6pm Sach had his meds.
We completed the row of his puzzle that we got stuck on last night.
At 6:40pm Saj popped in and took Sach to the toilet (medium output).
Wr made decent progress with his puzzle tonight. Finished row and started a new one (third of way done). We finished with the programs room at roughly 7:25pm.
We went for a short walk up the street and back, Sachin initiated holding my hand on tonight's walk -
 
429steps - 5m41s duration.
Once we arrived home, Sach was encouraged to sit on loungeroom couch whilst I removed his runners, and put his slippers back on and encouraged him over to the other couch next to Praveena. Where I handed him his iPad, which he sat on the table.
The rest of shift was spent relaxing on couch with Praveena.
Overall another good shift with Sach.

Went for an eager stroll

Activities: Went for a walk and made progress with
puzzle

Sachin was eating dinner upon my arrival, I sat with
him whilst he ate. Once he had finished we put his
runners on and got ready for a walk - up the road
and back. At times Sach eagerly sped up his pace -
was quite vocal during this. Once home around
5:50pm we attempted to toilet with no output. So we
moved to the programs room + got set up and
started with the puzzle (at this time Sachs 6pm med
alarm went off), Saj brought Sachins meds to the
room to give Sachin, followed by cups of tea.
We got quite stuck on puzzle tonight (Saj offered

assistance), I redirected Sachin to the other side of
the row in order to try and de-stress him (during this
time, his heartbeat fluctuated 90+bpm). He also
became stuck on the side he was redirected to.
We finished at roughly 7:45pm.
We then headed out to the family area - At 7:50pm
Saj took Sach to the toilet - large output. Sach then
spent time with mum (Praveena) watching TV and
playing on his iPad.
Overall quite a good shift with Sach.
After moving out for relaxing time on couch his
heartrate came down below 70bpm.

Gone Fishin’

WEATHER:
20° partly cloudy day.
 
NOTES:
Sachin was inside his house at the house when I arrived, he was still eating his dinner. Once Sachin had finished I assisted him putting on his joggers and ankle strap, we then left the house and drove to Bowna reserve.
Sachin and I fished at Bowna boat ramp, Sachin was encouraged to participate and coached how to hold a rod and reel it in. I casted for sachin and he reeled it in each time. After approximately 45 minutes of flicking lures with no success of catching fish Sachin and I returned home. Sachin appeared to enjoy himself, he was reserved in body language and eager to reel in when I handed him the rod.
I assisted Sachin taking off his shoes and putting slippers on then sach was encouraged to use the toilet, he produced a large urine, after I assisted him brush his teeth.
Sachin and I made cups of tea and went to the programs room for a crack at his jigsaw.
Sachin put a few pieces in tonight, he did engage me to participate by sliding the right piece in the right spot a few times and did the rest himself.
At 1940 hours we packed up and went to the kitchen, Sachin and I made cups of tea, Sachin followed my instructions for the ingredients well and fast. We then went to the couch for movie time.
Sachin was well behaved and appeared in a good mood today.
Shift finished at 2100 hours.

Winter starts again

WEATHER:
15° cloudy cold day.
 
NOTES:
Sachin was inside his house at the house when I arrived, he was finishing off a pot off tea.
Sachin appeared cold, he was shaking but the temp inside was warm, His mother informed me it was a medication side affect some times.
After his pot of tea I assisted Sachin brushing his teeth then getting into shoes for a walk.
Sachin and I pit in a half hour walk along Tabletop road, Sachin walked well, he kept a good pace with me and appeared to enjoy himself, he giggled occasionally.
When we returned home Sachin and I made cups of tea, Sachin assisted me by getting the ingredients promptly when asked, we then went to the programs room for jigsaw puzzle time.
Sachin placed a few pieces in tonight, he didn’t appear to focused on it though.
At 1930 hours we headed to the lounge for rest and watched a movie with his mother.
Apart from the shivers at start of shift Sachin appeared to have a good night.
Shift finished at 2100 hours.

Being Cheeky

Arrived at 5pm, greeted by Saj. Sachin has already
had dinner prior to my arrival. Conversed with Saj
for a moment, he informed me Sach was already in
the programs room. Saj boiled kettle and made tea
whilst I went and got set-up with Sach.
He appeared to be in a relatively goof mood ,
despite being stuck on a piece of his puzzle. As
usual Sach was being quite cheeky with persistently
making me place puzzle pieces.
Meds given at 6pm.
We made slow but decent progress with the puzzle
tonight. 10 minutes spent with Saj looking for a
piece we got stuck on.
We finished with puzzle at 7:30pm. Saj took Sach to
toilet - medium output. Once finished toileting, I took

Sach to brush his teeth.
When finished with that, I brought Sach out to sit on
couch with Praveena and gave him his iPad to play
with.
Rest of shift was spent on couch relaxing with mum
(Praveena). During this time, Sachin was quite
happy and vocal.
Overall it was a good shift with Sachin - very vocal
throughout.
Bpm between 64-90 throughout duration of shift.
Higher bpm when stressed (struggling to find pieces
of puzzle)- or during walks to toilet.
Stayed reasonably low when moving smoothly with
puzzle and during time spent relaxing on couch.

Fantastic Mood

Arrived at 1700 was greeted by Saj mood today. Sachin was in a fantastic mood

Sachin was at the table waiting for one of his favourite meal being both vocal and smiling.

1705 Started sorting out puzzle outside

1715 - 1730
I sat with Sachin We and ate together.

1730 - 1745 / placed Sachin Shoes on his feet and went for a walk up Cambridge drive to
top Intersection. Sachin showed that he wanted to hold my hand.
Sachin showed this by grabbing my hand.

1805
We arrived home and Saj gave Sachin his medication.

1806
Saj took Sachin to the toilet, no output.  Next, I guided Sachin to the bathroom.
and brushed teeth.

1815
Sachin grabbed his tea and we headed to do the puzzle
Sachin turned on the light. While i turned on music and diffuser.

1820 I applied lavender oil and spray

at about 1840, we couldn’t find a bit and this made Sachin feel frustrated. Saj came in and helped us find the missing piece.

1910 I applied more lavender to try help calm him(86 bpm).
Sachin then encouraged me to do the puzzle with him he did this by grabbing my finger and sliding the puzzle into place waiting for the right clicking sound follwed by a fist pump, which was a great interaction

1930 I applied more oil bpm 76

1955 Sachin and I finished up in the puzzle room. Sachin turned out the light and I switched off music and diffuser. Sachin then put his tea bags in the bin Saj took Sachin to the toilet with a large output.

20:00 Sachin was handed his iPad and watched a movie while Praveena got Sachin a small serving of corn chips and sleep tea. I proceeded to complete notes.

2100 final bpm 68 was happy and vocal today.

Well Rested

I arrived before Sachin came home. I entered through the side gate and was greeted by Saj. Saj showed me how to sort the new puzzle for Sachin “11” “9” and colours
Sachin arrived home with Praveena at about 5:15. Praveena said Sachin would be tired after
an outing with mum, so Sachin and I didn’t go for a walk.
Sachin when he arrived home was giving his dinner and went to the toilet with Praveena.

At about 5.30 took Sachin to his teeth he appeared calm and happy while doing this praveena prepared tea. Once we finished brushing  sachin's teeth, he collected his tea we moved to the programs room. Sachin and I started working on his Puzzle.
I applied lavender oil to the inner part of Sachin nose and sprayed his clothing with lavender.

6pm
Saj handed Sachin his medication.

I Observed Sachin to be quite shaky today compared to my first shift with him. Sachins heart rate sat mostly in the high 90s with heavy breathing. (90-98)

6.30 i applied Some more oil to help being down Sachins heart rate.
Sachin showed he is visually pleased with how I
placed/clicked the puzzle piece in by smiling and not removing the pieces. Sachin is working at a fast pace resulting is the outer frame to pull apart. This doesn’t seem to bother Sachin as he calmly puts it back together and moved on.

At about 6:40 Saj asked Sachin if he needed to use the toilet using both verbal and sign communication. Sachin had min output.

7: 10 pm more lavender appled. (7.24 pm) (72 bpm)
7:45 we finished up in the programs room.

We moved to the kitchen and Sachin placed his tea bags in the bin then moved to the couch. Next, I handed Sachin his iPad and he rested next to Praveena on the couch.

20:00 Saj requested that I stay till 2100  and wrote notes

2100 When I left, Sachin was in a good mood.

WEATHER:
16° sunny day, Sunny evening.
 
NOTES:
Sachin was inside his house at the house when I arrived, he was finishing a cup of tea.
After he finished drinking his tea I encouraged Sachin to use the toilet, he produced a medium amount of urine and we moved on to brushing teeth in the bathroom. I assisted Sachin brush his teeth.
Sachin was then assisted into his ankle strap and shoes, we went for a walk along Cambridge drive to the end, approximately a 20 minute walk in the suburb.
We returned home and I assisted sachin out of his sneakers and into his house shoes. Sachin and I accompanied his father for a car ride to iga in lavington, Sachin appeared to enjoy the ride, he smiled a lot. When we got home Sachin and I made cups of tea and attended his programs room and commenced work on his jigsaw puzzle.
Sachin completed a little bit before we knocked off and exited the programs room.
Sachin and I took up seats in the lounge with his mother and watched a bit of TV.
Sachin was well behaved for the evening, no issues to report.
Shift finished at 2100 hours.
