1754, Continued new puzzle and has made good progress. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every half an hour to help relaxation and reduce heart rate. 
1830, Medication administered by Sajit.
1918, Walk up and back Cambridge dr, 2347 steps in 29 minutes. 
1952, Took Sachin to the toilet.
1956, Back in to activities room for more puzzle time.

Well rested, happy and cooperative.

1730, Assisted in brushing Sachin's teeth.
1742, Music session for 15 minutes, Sachin on the piano and myself on the guitar.
1757, Transitioned to new puzzle and has made good progress. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every half an hour to help relaxation and reduce heart rate. 
1830, Medication administered by Sajit.
1933, Walk up and back Cambridge dr, 2237 steps in 28 minutes. 
2002, Sachin took himself to the toilet.
2014, Back in to activities room for about 15 minutes for more puzzle time.

WEATHER:
30 degrees, Sunny.
 
Mood:
Sachin was in a good mood tonight, he displayed moments of head strong behaviour, especially with wanting a refill of tea. He appeared bored while doing lego at first but we persevered and he started to show interest.
 
NOTES:
Sachin and I got straight into making a painters canvas stand tonight, it was prepack so all pieces were cut to size and pre drilled. I assisted Sachin by holding the legs in place and indicating where bolts go which he put in and tightened by hand.
legos tonight, he started off a bit slow and not interested but given time he came around and started really concentrating on matching the supplied pieces with the pictures I indicated to him.
Sachin had his 1830 medication, dispensed by his mother.
We did not go for a drive tonight, we stayed local and walked the streets, Sachin did 3515 steps, 33 minutes.
Sachin and I paused half way through the walk for a play on the parks swings for 10 minutes.
Sachin received a head massage from nan tonight and after that had some doritos while watching Indian drama shows.
Shift ceased at 2130 hours.

Progress notes
Sachin was already in the program room working away on his puzzle when I arrived and took several promts to get Sachin to come to the toilet as he's was super focussed on his puzzle, Sachin passed a moderate amount of urine when he did use the toilet then grabbed our teas and headed back into the program room music put on and his difuzer was put on as well as some oil on his top lip and happily got stuck into it making quite abit of progress,Sahins heart rate was higher than normal sitting between 80 and 90 bpm probably due to it being a new puzzle we finished up at 7.40pm and then got Sachin ready for his walk heading out at 7.49pm and getting back at 8.28pm at the start Sachin was very heavy on my arm but eventually quickened up the pace I didn't push him to hard as it was still warm ,Sachin When we got back was quite fidgety and wouldn't relax straight away his hr was still a little high sitting in the 80bpm range but started to settle coming down to low 70 bpm and appeared fairly relaxed at time of completion of shift .

Started new Solar system puzzle. Handed out Christmas hampers to the neighbours. Head massage.

• Mood:
Tired but happy and cooperative.

• Weather:
31c Sunny.

• Notes:
1736, Took Sachin to the toilet.
1739, Assisted in brushing Sachin's teeth.
1744, Moved to activities room and started new Solar system puzzle and has made good progress. Camomile and green tea, relaxing music in the background with lavender oil diffuser on. Monitored heart rate on a regular basis with iPad. Lavender oil above upper lip every half an hour to help relaxation and reduce heart rate. 68-76bpm whilst in the activities room.
1830, Medication administered by Sajit as well as extra medication due to high seizure risk day.
1930, Sachin and I handed out some Christmas hampers to the nearby neighbours, Sachin seemed to really enjoy the process. 
1950, Back in to activities room for about more puzzle time.
2014, Moved to the family room for sleep tea and videos on the iPad.
2016, Took Sachin to the toilet.
2022, Head massage from Grandmother.

Mood:
Sachin still had a few bouts of giggles tonight but overall he was much better than last night. Sachin appeared a lot calmer, his heart rate was better.
 
NOTES:
Sachin was much better tonight, for a change we did some legos, Sachin was quick, he followed the directions well, he made 4 objects.
It was impressive to watch Sachin work.
Sachin had his 1830 medication, dispensed by his father.
We went for a drive to Brooklyn fields and had a 29 minute walk in the neighbourhood, Sachin did 2869 steps.
Sachin had some doritos while we finished off the night watching mission impossible 3.
Shift ceased at 2100 hours.

Confused Sachin

WEATHER:
31 degrees, Sunny.
 
Mood:
Sachin had bouts of giggle fits constantly during the evening, these concerned me because he didn’t sound like his normal giggles, sometimes he’d start out with a happy expression and tone of laughter then mid way through convert to a confused expression and an almost sad tone. A very close eye was kept on Sachin tonight and a possible cause for his behaviour might be a recent change with his medication types.
 
NOTES:
Sachin was in his programs room already when I arrived for shift, having a little bit of a cheat, getting a head start on his Friends TV series Jigsaw puzzle.
Sachin immediately concerned me with his constant giggles, refer to MOOD section above.
A calm environment was maintained for sachins benefit tonight, lavender under nose every half hour, calming music, low voices, a treat while out driving, a head massage.
At 1825 Sachin was encouraged to use the toilet, he did go, we then got ready for a drive.
Sachin had his 1830 medication, dispensed by his father.
Sachin was had the giggles for a few minutes into the drive then settled down and was quiet.
Sachin got a treat from kfc on the way home, 3 wicked wings.
At 1930 hours Sachin and I got ready for a walk, he started up with his giggles again, but this lasted for the first 3 to 4 minutes of the walk, steps were forgotten tonight, I paid so much attention to Sachin I forgot to record them.
When we got back Sachin had a few minutes in the puzzle room then was ushered to the lounge for a head massage from his nanna.
Sachin had another bout of the giggles that lasted approximately 10 minutes then calmed and appeared to relax for a massage.
Shift ceased at 2030 hours.

Super focussed

Activities :
Good walk along table top road just short of the second bridge completing 2975 steps . Program room to work on his jigsaw puzzle, brushed his teeth.

Progress notes:
Sachin was already in the program room working on his puzzle when I arrived tonight and required several prompts to come out to use the toilet passing a small amount of urine then got ready for his walk heading up Table top road just shy of the second bridge as I didn't want to push him ,Sachin was quite heavy on the arm and breathing a bit heavy his avg hr was 111bpm taking 27 minutes doing 2975 steps ,brushed teeth when we got back then grabbed our teas and headed into the program room and Sachin very eager to get started his hr remained alot higher than normal sitting between 85 and 90 bpm throughout the evening but mainly put it down to his enthusiasm whilst working at the puzzle ,medication given at 6.30.pm, Sachin is taking a new medication which is Lamictal 25 mg and his tegretol is being slowly reduced by half a tablet first week we eventually finished up at 8.20pm and made our way into the lounge but several times Sachin getting up and going back into the program room and needed several prompts to come out now eating some Doritos on the lounge next to mum but his hr still a little high at 79 bpm and is being monitored closely until shift end at 9pm.

Dazed

Toileted before shower time.
Activities: 
Drove to Albury to drop mum out for dinner. Puzzle time. Watched Tv and ate snacks. 


Progress notes: 
Greeted Sachin’s family at 1730. Sachin appeared to be extremely excited to see me tonight. We fist bumped and then he began to jump and run around the house. We got Sachin’s shoes on and headed out to the car. We then drove to drop Praveena out for dinner with her friends. Sachin sat in the front and was extremely happy about it. We arrived home in time for 1830 meds to be administered. I assisted Sachin in brushing his teeth and then we headed into the puzzle room. Sachin’s heart rate was sitting in the 80’s for majority of the time spent in the puzzle room. Sachin was very concentrated on his puzzle tonight. We didn’t go for a walk due to the heat, and needing to sort out the week aheads medications. Sachin had a blackout at approx. 1925. This lasted for approx 4-5 minutes. Extra 200mg of Epilim administered at approx. 1938. Sachin took approx. 15 minutes to come back to himself. He began to work on his puzzle again. Saj and I made the decision to only organise 3 days worth of medications instead of a week due to the blackout. We headed into the family room to watch some tv at approx. 2010. Sachins heart rate remand between 60 and 80 for the remainder of the shift. Sometimes jumping into the high 90’s. Sachin enjoyed a bowl of corn chips and sleep tea. Extra 200mg of Epilim administered at approx 2130. Shift ended 2130

Woolies drive

ACTIVITIES.
Walk
Jigsaw puzzle work.
Drive to woolies.
 
WEATHER:
29 degrees, Overcast with wind.
 
Mood:
Sachin appeared happy and relaxed tonight, he was in a good mood.
He did not appear to enjoy his car ride, making worried noises every now and then.
 
NOTES:
Sachin joined his father and me for a drive to woolies for super glue, at times during the ride he made worried noises but calmed easily.
We returned home and started work on the jigsaw, half way through we had a break where Sachin used the toilet and was assisted brushing his teeth.
Sachin had his 1830 medication, dispensed by his father.
At 1930 hours Sachin and I packed up in the programs room and got ready for a walk.
We had a short walk tonight around the block and finished up with a play on the swings. Sachin did 849 steps in 10 minutes and 15 minutes on the swings.
 
Shift ceased at 2030 hours.
