NOTES:
Sachin was at his residence inside when I arrived for shift, he was seated on the lounge with his mother watching TV.
I encouraged sachin to use the loo, he produced a medium amount of urine, I then assisted him with brushing his teeth.
Sachin and I accompanied his father on a drive to Lavington chemist warehouse then returned home, Sachin appeared to like the drive, he saw some horses as we drove past a rodeo show on the way.
When we returned home we made cups of tea, Sachin assisted me by gathering the tea bags and milk, then putting it all away afterwards.
We moved to the programs room for jigsaw puzzle time, Sachin didn’t seem interested tonight, I attempted to get him interested a few times with no success,  he did however manage to put a few pieces in, approx 5 to 6.
When it was time to move to the lounge for quiet time Sachin didn’t argue, he was straight up and out.
We again made cups of tea and sat watching lord of the rings with his mother.
All up Sachin had a great night and no issues.
Shift ceased at 2100 hours.

1700 Greeted Sachin at dinner table where he was finishing a cup of tea. 1710 Sachin and I made our way to the bathroom. Sachin toileted (S-M output) whilst I prepared his toothbrush. Sachin finished in the toilet, together we washed his hands and brushed Sachins teeth. Sachin rinsed his mouth. We made our way out of the bathroom to kitchen where Praveena made our teas and we started in programs room. Sachin switched on the lamp and got started on his puzzle whilst I turned on fan, music, lights and diffuser. 1720 applied lavender oil to Sachins nostrils and spray to chest. BPM 82. Saj advised there would be no walk today due to Sachins lack of sleep last night. By 1730 Sachin already found 5 pieces and onto next row of puzzle. 1735 I showed Sachin the “music” cue card and asked if it’d be ok to play some guitar. Sachin responded by initiating a fist bump. 1800 Saj administered meds. 1810 82bpm. 1838 applied more lavender oil and lavender spray. BPM 87. 
1855 I refilled the diffuser with water and lavender. Sachin took the pourer from me and sipped out the drips. I asked Sachin if he was thirsty and he gave me a fist bump. I got Sachin a cup of water from the kitchen. We signed “Thankyou” to each other (I didn’t know he knew this one!). BPM 84. 1927 Sachin switched off the lamp and we shut down the programs room. 1930 Sachin toileted. Medium output. Washed hands and onto the lounge to play on iPad and watch TV. 2000 Sachin made a trip to the laundry to check out the washing and a little detour via Mum and dads bedroom before returning to the lounge for cup of tea and iPad until end of shift. BPM 62.

Greeted Sachin in kitchen around 17:00. Praveena assisted Sachin in finishing his drink, I signed for toilet and Sachin copied actions and moved to the toilet. Large urination. In bathroom I physically assisted in washing hands and brushing teeth, Sachin rinsed mouth out with water provided. Around 17:20 made it to puzzle, I prepped room with Sachin turning on lamp, he sat with his tea, it wasn’t long and Sachin had found his first piece. Lavender applied 17:30 and 18:00. Meds given with water 18:00. Lavender applied 18:30. Sachin doing well with puzzle so far putting 8 pieces in. Around 19:10 Sachin BPM up to 98, shaking fist got up holding his pants making his way to the toilet for bowel movements. Praveena said she will manage. Back to puzzle room BPM 110 but lowering .19:18. Around 19:30 we prepped for a walk assisting Sachin with putting his socks, brace and shoes on. Short walk went well no issues. Sachin sitting in lounge around 19:55 after being assisted with removing socks and shoes. Sachin watching iPad and I sat with for remainder of the shift.

FIRST CATCH !!!

WEATHER:
23° sunny day / slightly overcast arvo.
 
NOTES:
Sachin was at his residence inside when I arrived for shift, he was seated on the lounge with his mother watching TV.
I had a small handover / toolbox meeting with his father Saj and got brought up to speed with the happenings with Sachin over the past month of my absence. Saj asked if I could take Sachin fishing, today would be the best time as from now and the next 7 days will be in the 30° plus temp range. I encouraged  Sachin to use the loo ( large urine output ) then assisted him brush his teeth. I then assisted Sachin put his ankle support brace on and his walking shoes.
Sachin and I departed in my car and we went to Bowna reserve boat ramp, Sachin accompanied me on a short 10 minute walk to a desired fishing spot. I set up Sachin with a rod and lure, I cast for him and handed him the rod, he mimics me reel it in. We did this for 10 minutes with no luck, I changed his set up to a baited line and cast out for him. Sachin did think he was still luring and reeled it back in a few times until he followed my lead and did not reel in. Sachins Line went hard while he was reeling it in, It appeared to be snagged so I assisted Sachin break it free then while he was finishing reeling in a turtle surfaced and was on his hook! Sachin landed the turtle, I picked it up with lip grips and handed it to Sachin, he posed for a photo then I released it to the water.
Sachin and I continued fishing for approximately another 20 minutes, not so much as a single bite, then called it a day and packed up.
When we returned home I assisted Sachin by removing his ankle brace and shoes, he got back into his slippers. Sachin and I acquired cups of tea his father prepared for us and proceeded to the programs room for jigsaw time.
Sachin, in his own way asked for help tonight, he was happy to work with me, he found the right pieces and would wait for me to push them into the spots he indicated. Sachin was good, he displayed very minor ocd traits with his puzzle tonight, once the piece was in the right spot that was good enough for tonight, no re doing it again and again. At 2000 hours Sachin and I packed up and left the programs room. Sachin was encouraged to use the loo again, medium urine output. We then settled down on the lounge with his mother and watched final destination.
Shift ceased at 2100 hours.

1700 arrive to the house, say hi to Saj in garage and head inside to where Sachin and Praveena are on the couch watching TV. Sachin and I headed to the bathroom where Sachin used the toilet whilst I prepared his toothbrush. We washed hands and brushed Sachins teeth before heading out to grab our teas. Sachin had already prepared his tea and reminded Praveena by showing her the teabags on the counter and putting away the boxes. We headed to the programs room, Sachin turned on the lamp, I turned on the diffuser, music and fan and Sachin got to work on his jigsaw. 1745 applied lavender oil to Sachins nostrils. 1748 applied paw paw ointment to Sachins lips as they were looking dry. Making good progress on puzzle tonight. 5 pieces in 15 minutes. 1800 Sachin took meds, provided by Saj in med cup. 1825 applied more lavender oil to Sachins nostrils and paw paw ointment to Sachins lips. 68BPM. 1920 we left the programs room a little earlier, Sachin toileted, medium output. We then popped on Sachins sneakers ready for a walk. Walked to the end of Cambridge and back. Sachin was calm and appear to enjoy the walk. Home for cup of tea and settle on couch with iPad for remainder of shift.

Dogs on the walk!

1700 arrive and greeted by Praveena and Sachin in the kitchen. Sachin preparing his tea. 1720 Saj took Sachin to the toilet. We brushed Sachins teeth and headed to programs room with our teas. Turned on music, fan and diffuser. 1735 applied lavender oil to Sachins nostrils and spray across chest. Sachins lips are dry and look sore - so I applied some paw paw ointment at 1750. 1800 Sachin took meds from med cup provided by Praveena. Whilst Sachin was working on his puzzle in the programs room I played him some music on the guitar which he was enjoying with a big smile on his face. Sachin really fixated on the “pop” sound of the puzzle pieces tonight. 1840 80pm - applied more lavender oil to nostrils and across chest. More paw paw ointment applied to Sachins lips. Sachin was quite vocal during puzzle time tonight. 1905 refilled diffuser with water and lavender oil. Making good progress on row 10 of puzzle. 1914 75bpm. 1930 we finished in programs room, turned off diffuser, lamp, music and moved to sofa to put sneakers on for walk. Walked to end of Cambridge and back - short walk today. Sachin was happy on walk and noticed some dogs walking by. Walked home, Sachin toileted, small to mid output, washed hands and settled on lounge next to Praveena with iPad. 2000 76bpm. 2005 applied more paw paw ointment to Sachins lips. Praveena prepared Sachins tea. Back on couch with iPad. 2010 77bpm. Settled on couch for remainder of shift with snacks and tea.

Singing to Sachin

1700 I was greeted by Saj, Praveena and Praveenas colleague Ian in the courtyard. 
Went inside, said hi to Sachin sitting at the dinner table enjoying his dinner. Discussed Sachins sleep with Saj as Sachins Fitbit appears to be recording different data to what Praveena and Saj believe to be true. 1720 Sachin finished his dinner and tea, Sachin toileted with Saj (med - large output). 
1730 Sachin and I took our teas to programs room and got started on puzzle. 1738 heart rate got a little high (80bpm). Applied lavender oil to nostrils. 1740 I began to play guitar which Sachin obviously enjoyed. This was reflected in both Sachins heart rate and general demeanour, including his vocal cue “tictar” and smiling. 
1800 Sachin had medication, provided by Praveena in med cup. 
1810 Sachin heart rate 90bpm. Lavender sprayed across Sachins chest. 
1815 Sachin had trouble focussing and eyes rolling back. Regained focus with hand in front of face and checked in with Saj. 
Lots of interaction on the puzzle tonight and made good progress on the pieces. 
1900 Heart rate sitting at 80-88. More lavender applied to nostril. 
1930 Finished in program room, Sachin toileted with Saj. Medium output. 
Settled on couch with Praveena for remainder of shift watching swimming, comfortable steady heart rate at 60bpm.

Not a Fan Of Butter Chicken

Arrived @5pm, greeted by Saj - informed me that
fitbit had recorded not much sleep, Praveena
believes he had more than recorded.
Sach was having a shower when I arrived.
Sat down with Sachin at dinner table @5:15pm. He
was slow at eating (not a big fan of butter chicken
according to Saj and Praveena), once he was
finished we went and brushed teeth. Then collected
our tea Praveena had made and progressed into
programs room. Put lamp, infuser and music on and
applied lavender oil. Got started on puzzle, slow
progress again tonight, as on a section where
pieces are all very similar and Sach becomes quite
confused.

Sach had meds as usual at 6pm, put in med cup by
Saj as usual and given to Sach.
He had another lot of meds again at 6:30pm, again
put in med cup by Saj and given to Sach.
Sach wasn't overly vocal again tonight, only really
engaging with me through having me place puzzle
pieces. Saj popped in and assisted with finding a
few pieces we got stuck on.
We finished up in programs room @7:30pm, packed
up and turned everything off. Made our way out to
loungeroom, Sach put his teabag in bin and then
went and satin couch with mum. Gave Sach his iPad
Toileted before I arrived and @7:33pm (medium
output).
Praveena had a friend over.
Sach sat on couch and watched TV, whilst
Praveena chatted with her friend.
At one stage he decided to get up and walk around
the house a bit, seemingly agitated by the girls
chatting perhaps. Eventually settled down and went
back to sit on the couch, with kettle and his cup.
Rest of the night was good, Sach remained settled
on the couch with his mum whilst she chatted with
her friend.
Finished shift @9pm.
Overall a good shift with Sachin.

Shadow shift with Janine

Toilet,program room to work on puzzle, walk up Cambridge drive to Table top Rd and back, brushed teeth

Progress Notes:

Arrived at 5pm tonight to start work with Sachin and met shadow shift Janine and went in and introduced Janine to Saj Preveena and Sachin who was sitting at the dinner table having his dinner and his cups of tea ,Saj then got Sachin to use the toilet with a small output, we then grabbed our cuppas and made our way into the program room and got started on his puzzle, his hr was a little high at 93bpm but began to slowly come down,Sachin wasn't really showing alot of interest in the puzzle tonight and was doing quite abit of stoo starting through out but wasn't stressed at all as he was interacting very well with Janine and and myself. We finished up at 7.30pm and got ready for our walk and I showed Janine the routin effort getting him ready and headed out down Cambridge drive to the Table top intersection and back ,Sachin was sat times quite vocal and interacting with Janine very well and looked comfortable with her,when we got back Preveena took Sachin in for his shower then brought him into the lounge we also brushed his teeth and then happy to relax on couch until end of shift at 9pm.

17:00, arrived and was greeted by Saj. Sachin was eating at the dining room table. 
17:15- Brushed his teeth .
17:25- Got our tea and went to the programs room, turn on everything, then applied lavender spray and oil. 
18:00 medication and re-applied oil to nose. 
Sachin was making great progress on his puzzle.
18:35 applied more oil and spray 
At about 19:00 Sachin went to the toilet to mid output 
At approximately 19:40 Sachin and I finished up in the programs room. 
Sachin took his tea bags to the bin then sat down for me to put his shoes on and we went for walk just up to the corner of Cambridge “short walk” and back arriving home at about 20:00. 
20:05 Saj prepared Sachin's sleep tea and grabbed a light snack for Sach. 2100 clocked off .
