NOTES:
Sachin was on his last few sips of his tea pot when I arrived, he was seated at the dining table. Sachin appeared rested and wide awake, alert and aware of his surroundings. Sachin greeted me with eye contact and a fist bump.
Shortly after Sachin had finished his tea pot we accompanied his father to local woolworths. Sachin pushed the trolley and I steered for him, Sachin was chill and calm during his outing.
At home Sachin had his dinner, he appeared unimpressed with dinner. I assisted Sachin with brushing his teeth, Sachin used the toilet (large urine output), we made cups of tea.
We occupied ourselves doing jigsaw from 1800 hours to 1920 hours, Sachin had no troubles finding pieces tonight. He appeared to enjoy his time on jigsaws.
At 1920 hours we sat with his father in his study for 20 minutes then Sachin settled into the loungeroom couch.
At 2000 hours Sachin and I made cups of tea and a bowl of doritos, for tv time.
Sachin was happy today he made good eye contact and his vocal tones were happy / cheerful sounding. Sachins BPM was very good all night, no spikes above 90 bpm.

Shift Times, Date :- 1600 to 2130 hours, Friday the 30th May 2025.
ACTIVITIES.
Drive.
 Indian movies, TV time.
Jigsaw.
WEATHER:
17° sunny day.
 
NOTES:
Sachin was chilling out on the couch when I arrived, he had finished his pot of tea earlier. After checking sachins fitbit stat’s and being satisfied he had enough sleep and a good bpm we set off for a walk.
Sachin and I went for a 30 minute walk, he went very well, his attitude was cheerful.
Sachin used the toilet (very large urine), he then had dinner. Sachin had chicken curry, he ate heartedly and appeared to like it very much. After dinner sachin and i done jigsaw for and hour then went for a drive to fill up his mother’s car with fuel, Sachin made some happy noises while on the drive.
Sachin used the toilet once more at home (medium urine output) we then made cups of tea and a dorito snack for tv time.
Sachin appeared to be in a very good mood, easy going, he was open to me and engaged me well.

Focused sach

ACTIVITIES.

 State of Origin, womens.
Jigsaw.
WEATHER:
17° sunny day.
 
NOTES:
Sachin had a good night Sleep and it showed with his resting bpm. A small set back was his hyper focus behaviour tonight, Sachin would focus deeply on minor details in his surroundings instead of focusing on the task at hand. Example is, Sachin during toilet time would concentrate, focus on the toilet paper for 5 minutes instead of using the toilet to urinate. Making cups of tea he would look around and focus on anything else in the kitchen besides making his tea. This is just two examples but the list goes on, sach needed constant redirecting to stay on target with almost everything we needed to do.
We attempted to maintain a normal routine, Sachin appeared to enjoy his dinner of rice. We accompanied his father on a drive to woolies for groceries, Sachin pushed the trolley while I steered for him. We watched the womens State Of Origin football, Sachin never looked away, he appeared happy watching.
Sachins heart rate was very good, he maintained a mid 60’s bpm most of the night.
Sachin had a good night stat wise, his behaviour was a little off though. Nil issues to report.

Shift Times, Date :- 1600 to 2130 hours, Wednesday the 28th May 2025.
ACTIVITIES.
Drive.

Walking.

17° partly cloudy day.
 
NOTES:
Sachin was finishing his pot of tea when I arrived. Sachin appeared happy but a bit slow responding to my fist bump. Once he was done we cleaned and prepared his tea pot for tomorrow. We set off for a evening walk. Sachin was walking fine, occasionally making vocal noises. Approximately 10 minutes into the walk Sachin began to slow down, I noticed sachins eyes were unfocused, he was staring off left and up. Sachin took 2 steps backwards and tilted his head towards the sky. I immediately hugged sachins chest and lowered him to the footpath, there he sat. Sachins eyes still looked upwards and he wanted to lay down, I kept Sachin in a upright seated position and maintained eye contact, I kept trying to get sachins attention, after approx 15 seconds sachins eyes focussed on me. Sachin was coming around and i then made a phone call to his mother requesting she come collect us in her car. We stayed seated while we waited, during that time sachins eyes scanned his surroundings and made eye contact with me as well. Sachin was unsteady on his feet when I assisted him to the car, he sat himself down and did up his seat belt.
We returned home to find KFC for dinner, sachins favourite, it appeared to cheer him up. After dinner sachin was assisted with brushing teeth, he used the toilet for a large urine output. We made cups of tea and done some jigsaw till 6pm. We accompanied his father on a drive to woolies, we waited in the car. When we returned home Sachin took his seat in the lounge with a fresh cup of tea, sachins bpm was in the 55 to 65 range, good. Sachin was going well so far, 8pm came and Sachin was encouraged to use the toilet, he didnt go so we made cups of tea and a bowl of chips to watch State Of Origin, once it started sachins bpm rose to 85. Sachins bpm did settle down after watching for a little while. After 8pm Sachin looked tired bit was still feverishly watching TV footy. Nil issues after returning home from walk incident.

Date: Tuesday 27th May 2025

Sachin was drinking his tea pot on the couch upon arrival. He was quiet and appeared relaxed. I sat with him while he drank, and then assisted with preparing the pot for tomorrow. Sachin then appeared very excited for dinner and was lingering in the kitchen while I assisted with preparing dinner. We then sat and ate together - he was very precise with his food and ate all of the peas in his dish one by one. We then cleaned up, washed hands, he refused to go to the toilet, and then cleaned his teeth. We then prepped teas and headed to the programmes room. He settled into the puzzle and his bpm was in the 60’s for our whole time in the programmes room. We finished up at 1930 and I prompted Sach to the toilet (large output). He was very methodical and took a while to go. We sat back down and relaxed on the couch. Snack and sleep tea at 2000. Sachs bom was great - he appeared happy and relaxed for the rest of the evening.

Date: Monday 26th May 2025

Sachin was at the dining table upon arrival. He was finishing up his breakfast and drinking his tea, he appeared to be calm and relaxed. I sat with him while he drank. He had an early dinner, which he ate very quickly. I cleaned up while he finished off his tea, and once he was finished, I supported him through making and preparing the pot for tomorrow. I then prompted him to the toilet (medium output), and cleaned his teeth. We then prepped teas and headed to the programmes room. Sach quickly settled in and his bpm lowered to 50s and 60s, indicating he was calm and relaxed. Finished up at 1930 and went to relax on the couch. Semolina a snack and sleep tea at 2000. Toilet again at 2020 (medium output). His BPM was steady for the rest of the evening. No issues.

Date: Tuesday 18th March 2024


Sach was still asleep upon arrival. I assisted praveena around the house until he had woken up, and then supported them through Sachin’s shower routine. Sach then came out and hid his pot of tea and breakfast. When he was done, I supported him through preparing the pot for tomorrow. We then sat and ate dinner together. After cleaning up, I prompted Sach to the toilet (small output), then assisted with brushing teeth. We then went to the kitchen to prepare teas together. We then headed to the programmes room for puzzle time. Sachin was very focused and needed no reminders to stay on track. We finished up at 1900 and went for a walk along tabletop rd. Once back, Sachin quickly got settled into the couch. Snack and sleep tea at 2000. Toilet again at 2045 (very large output). No issues throughout evening.

Sachin was at the dining table upon arrival. He appeared calm and alert. He was just finishing his tea from the pot, so I encouraged him up and supported him through preparing his tea pot for tomorrow. He then sat back down and I assisted Praveena with preparing dinner. Sach and I then sat down and ate with together at the dining table. Once finished, I cleaned out dishes and prompted Sach to let me wash his hands. I then prompted him to the toilet (small output) and supported him through brushing teeth. We then headed back to hitched and prepared teas together before heading to the programmes room. Sach sat down and got stuck right into the puzzle. BPM initially in high 89’s, that eventually settled to high 70’s. We finished up at 1900 and went for a walk around the estate. Once back, he rushed to the toilet and emptied his bowels. Once cleaned up, he settled on the couch. Snack and sleep tea at 2000. BPM in 70’s. No issues for evening.

J knockSachin was dressed and ready to go when I arrived, Sachin had dinner and his teeth brushed first then we left.
We went to lavi mall where Sachin and I wandered around bigw, rivers, eb games, woolworths, etc. Sachin did not appear bothered by people, he was looking around alot and occasionally smiling. We returned home to do a little jigsaw puzzle time then at 8pm Sachin took his seat in the lounge for TV time. Sachin had a very good steadily low bpm all evening he also had no behaviours indicative towards stress or anxiety, total relaxed.

12:06 hrs sleep, risk Minimal.

Sachin was in a very good mood today, when I arrived he finished what he was doing and went straight to programs room for jigsaw time. At 5pm Sachin had his dinner, he appeared to like it. After dinner sachin used the toilet and was assisted with brushing his teeth.
We made cups of tea and returned to the programs room for more jigsaw.
At 6:50pm Sachin and I exited the programs room and went to the rear yard and watered the vegetable garden, we also went for a 10 minute walk around the block.
Sachin used the toilet at 8pm, urine output medium. Together we made cups of tea and watched TV.
Sachin appears to have had a good amount of sleep, he was playful and friendly.
