Focused on puzzle

Sleep 7hours risk level medium
Weather 22 degrees- mostly cloudy
Toileted before shower. Toileted after program room. Extra large urine output. 
Activities: 
Walked up and down Cambridge drive. 1,815 steps completed in 23 minutes. 


Progress notes: 
Greeted Sachin’s family at 1700. Sachin was in the bedroom fixing Praveena’s clothes after her shower. I had to get Sachin from the program room as he had already started puzzle time. He appeared to be very keen to get stuck into it. Walk up and down Cambridge drive. Sachin became very heavy on my arm, I checked his Fitbit and his heart rate was sitting in the 90’s. I looked at Sachin and he had tears streaming down his face. We continued our walk and I held Sachin’s hand to comfort him. We walked for approx. 23 minutes and completed 1,815 steps in this time. We returned home, brushed Sachin’s teeth then headed into the program room with our cups of tea. Meds given to Sachin and taken at 1830pm. Sachin kept good focus tonight, and made good progress. Heart rate remaining in the 60’s. Sachin began to lose focus at approx 7:10pm. He appeared to be quite content, smiling and laughing at me. He skulled his tea and made his belly wobble to hear the liquid in it. Saj came in and gave Sachin the guitar. Sachin strummed it a few times and then handed it back. Headed into the family room at approx. 1930 where extra 200mg of Epilim was administered and taken. Toileted Sachin and he produced an extra large urine output. He came from the toilet laughed and jumping around. Sachin sat with us on the couch while we watched a movie. His heart rate began to reach the 70’s and he seemed very irritated sitting on the couch not being able to sit still. Sachin then began to become relaxed and his heart rate dropped back down to the mid 50’s-60’s, he was still vocalising a lot looking at Praveena. Sachin was holding Praveena’s hand and his heart rate spiked right up to 97 but soon came back down. Saj made Sachin another cup of tea and Doritos for a snack. Sachin’s heart rate remained in the 60’s whilst he ate. Sachins heart rate jumped to 100 but quickly came back down. His heart rate was sitting at 76 when I left. I said goodbye. Shift ended at 2100.
