Sleep 12hours 3mins risk level min Weather 30 degrees- sunny Toileted before shower. Large urine output. Toileted after puzzle time. Large urine output. 63BPM RHR. Activities: Sorted weekly medications, 35minutes. Puzzle time. Ate fresh butter chicken. Progress notes: Greeted Sachin’s family at 1700. Sachin was eating his dinner and drinking tea when I arrived. We got stuck straight into our weekly medications sittings. I noticed that Sachin’s heart rate was in the 100’s while we were doing the meds and a few times he stared off but I was able to get his attention. Teeth brushed and then we headed into the puzzle room. Sachin’s heart rate then came down to the mid 80’s once we were in a calmer environment and stayed there for the majority of the shift. 1830 meds administered. Sachin and I then went out to the kitchen and enjoyed some fresh butter chicken that Saj made for their visitors. Sachin licked the plate clean. We then went back into the puzzle room with our cups of tea. Sachin was very concentrated tonight and required next to no assistance with the puzzle. He was flowing well and completed 6 rows in the shift duration. Sachins heart rate remained in the mid 70’s to 80’s for the rest of the shift. Sachin went to the toilet after puzzle time and produced a large urine output. Sachin settled with a cup of tea when I left. Shift ended at 2100.
