Shift Times, Date :- 1600 to 2130 hours, Wednesday the 28th May 2025.

17° partly cloudy day.
 
NOTES:
Sachin was finishing his pot of tea when I arrived. Sachin appeared happy but a bit slow responding to my fist bump. Once he was done we cleaned and prepared his tea pot for tomorrow. We set off for a evening walk. Sachin was walking fine, occasionally making vocal noises. Approximately 10 minutes into the walk Sachin began to slow down, I noticed sachins eyes were unfocused, he was staring off left and up. Sachin took 2 steps backwards and tilted his head towards the sky. I immediately hugged sachins chest and lowered him to the footpath, there he sat. Sachins eyes still looked upwards and he wanted to lay down, I kept Sachin in a upright seated position and maintained eye contact, I kept trying to get sachins attention, after approx 15 seconds sachins eyes focussed on me. Sachin was coming around and i then made a phone call to his mother requesting she come collect us in her car. We stayed seated while we waited, during that time sachins eyes scanned his surroundings and made eye contact with me as well. Sachin was unsteady on his feet when I assisted him to the car, he sat himself down and did up his seat belt.
We returned home to find KFC for dinner, sachins favourite, it appeared to cheer him up. After dinner sachin was assisted with brushing teeth, he used the toilet for a large urine output. We made cups of tea and done some jigsaw till 6pm. We accompanied his father on a drive to woolies, we waited in the car. When we returned home Sachin took his seat in the lounge with a fresh cup of tea, sachins bpm was in the 55 to 65 range, good. Sachin was going well so far, 8pm came and Sachin was encouraged to use the toilet, he didnt go so we made cups of tea and a bowl of chips to watch State Of Origin, once it started sachins bpm rose to 85. Sachins bpm did settle down after watching for a little while. After 8pm Sachin looked tired bit was still feverishly watching TV footy. Nil issues after returning home from walk incident.
 
Shift ceased at 2130 hours.
