Support worker Maddy
Sleep 8hours 36mins risk level 1 Weather 34 degrees- partly cloudy & humid Toileted after medication sorting. Large urine output. Teeth brushed before puzzle time. 56 BPM RHR. Activities: Sorted weekly medications, 35minutes. Puzzle time. Short walk. Watched Tv. Progress notes: Greeted Sachin’s family at 1730. Sachin was in the puzzle room this evening when I arrived. I organised the medications ready for Sachin to put in the next weeks days containers. Sachin had to make sure everything was perfect tonight while sorting the meds into their containers for the week. The lids were perfectly lined up and all tablets in the right spots. This took approx. 35 minutes. Sachin was very vocal tonight. Sachin was walking around the house after we did the meds. Sorted out his mothers clothes after her shower and then wandered into his own bathroom. Turns out he needed to use the toilet. Large urine output at this time. We got our tea and headed into the puzzle room. Sachin was extremely focused on organising the boxes again and stacking them right. Sachins heart rate went up to the 100’s and eventually went down to the 80’s and that’s where it stayed for the duration of shift. Sachin kept blocking his ears while we were doing the puzzle but he would fist bump me to ensure me he was okay. 1830 meds administered and taken. Saj and I were trying to help Sachin by putting some pieces in but Sach would just pull them back out and then go back to stacking the boxes perfectly. 1930 rolled around and we headed out to go for a walk. We went all the way down around Cambridge and got half way back when Saj picked us up due to the rain that was about to come. Sachin was very excited and smiling the entire time while we were walking. He was also very vocal. He set the pace fast at times and then slowed right back down. 1,544 steps complete. Upon arrival home I assisted Sachin with brushing his teeth. Sachin wouldn’t sit still in the family room. His heart rate went right back up to the high 90’s. Sachin returned from the toilet and sat down with his sleep tea. His heart rate dropped back to the 70’s again. Praveena gave Sachin a bowl of corn chips and he was very content. Sachin took his bowl up to the counter but then returned back to the couch with the rest of the corn chip packet asking for another serve. His HR went to 90 while he was eating his second serving of corn chips. I said goodbye. Shift ended at 2100.
