Seizure risk level:- Risk level- Minimal. Previous night Sleep:- 8:37 hrs sleep. Resting heart rate:- 61 resting bpm. Shift Times, Date :- 1500 hrs to 2130 hrs. Friday the 18th August 2023. ACTIVITIES. Walk. Movies. Jigsaw. WEATHER: 11° Rainy day. NOTES: When I arrived for shift, Sachin, his father and his mother were in the programs room with sachins OT. I was requested to demonstrate the level of support Sachin requires, I demonstrated how I support Sachin with jigsaw work, brushing teeth, toileting assistance and going out for exercise walks, the support OT made her notes during the demonstrations and ceased her duties at approximately 1630 hours. Sachin and I took it easy until dinner time by playing with jenga blocks. I have used these blocks a few times with Sachin now, I am encouraging him to follow my lead and build the same things I am, Sachin is getting much better with following exactly what I Doo but struggles with delicate precise movement, his shaking gets the better of him but he is persistent and always finishes. Sachin had his dinner then a toilet break, I assisted him brushing his teeth again then we made cups of tea. We took these to the programs room for jigsaw time. Sachin appeared not interested , already being in here earlier, so he found a few pieces and just worked slow and steady. At 1930 we packed up and used the toilet again, we settled in the couch and watched a bit of TV with his mother. At 2000 hours we made cups of chamomile tea and returned to the couch. Sachin has had an excellent bpm all night, his behaviour and attitude was acceptable, he did slow slight interest and agitation towards to ‘New Person’ the OT, but all up he was fine. Shift ceased at 2130.
