NOTES:
I arrived at 1700 hours for shift with Sachin, he was inside his house in the kitchen with mum sorting his next days pot of tea. Sachin appeared to daze off a lot and stare into nothing, staff paying attention to this tonight, quite often he needed prompting to come back into reality by a hand or arm rub.
Once he had put his tea bags in the pot and placed it on the stove we went for a toilet break, Sachin needed two prompts to use the toilet, eventually he did pass a medium amount of urine.
I then assisted Sachin brush his teeth, I brushed his teeth first then let him finish off. Sachin and I worked together to make cups of tea then headed to the program room for jigsaw time. Sachins bpm was around 95 to 105 for the start of puzzle time, he appears relaxed but the high bpm is concerning. Sachins BPM did settle a bit to 90/95 for most of the puzzle time, Sachin maintained a consistent pace at his puzzle, despite the dazing off.
Fishing and or an evening walk was called off because of his dazing and high bpm. At 1930 hours Sachin and I packed up and left the programs room, he was encouraged to use the toilet, he didn’t go. Sachin and I rested on the couch with his mother watching TV for remainder of shift, during this time his bpm dropped to mid 70’s. At 2000 hours I made up cups of tea for Sachin and myself, his mother prepared him a bowl of chip snacks.
Sachin needed a lot of prompting with his dazing off tonight, staff had to be extra vigilant with observing Sachin so he didn’t stay dazed.
Shift ceased at 2100 hours.
