J Knock Tuesday the 25th feb 2025 8:17 hrs sleep 65 resting bpm 34° sunny day Sachin was wide awake and appeared happy when I arrived. He was drinking his pot of tea at the kitchen table. Sachins bpm was in the 90's from 4 till 6pm, afterwards he was low in the 70's for remainder of the night. Sachin used the toilet at 5:30 and 8 pm. I assisted him brush his teeth at 530. Sachin and I went to the supermarket market for a few groceries, done jigsaw puzzles at home and went for an exercise walk. Sachin was quiet tonight, he appeared happy but reserved. No issues to report tonight.

Seizure Risk Level: Risk level- low Previous Night Sleep: 10hr 11min of sleep Resting Heart Rate: 64 resting bpm Shift Time: 1600 to 2130 Date: Monday 24th February 2024 Sachin was still in bed upon arrival. Mum and I walked in and woke him up approx. 1645 and bean his morning routine. While in the shower, I helped with cooking his breakfast and preparing tea. Once out of the shower, he sat at dining table and ate and drank while I assisted with preparing dinner. He finished his tea and I helped him prepare the pot for tomorrow before we sat and ate dinner together. We then completed post dinner routine together before making teas and heading to the programmes room. Sach wasn’t very into the puzzle this evening and needed multiple reminders to focus. His bpm was in 80’s and 90’s. We finished up a bit early to go for a walk. We walked for 15 mins through the estate, he appeared to enjoy it and was smiling and vocalising. He then came outside with me to water garden - but did not want to help or come anywhere near the hose. We came back inside - toilet (medium output), to then relaxing in the couch. Sleep tea and snack at 2030. No issues for evening. A Very Fussy Boy
Support Worker: Katy Seizure Risk Level: Risk level- low Previous Night Sleep: 9hr 33min of sleep Resting Heart Rate: 64 resting bpm Shift Time: 1600 to 2130 Date: Tuesday 16th July 2024 Sachin was seated at the dining table when I greeted him today. He had just finished his pot of afternoon tea, so I prompted him up and we prepared the pot for tomorrow. He was focused, but did get very particular about one of the teabags. We then sat back down together and waited for dinner. Sach downed the main part of his food very quickly, but then apparently could not decide whether to lick his plate or not. It took approx. 15 minutes of prompting and encouragement before he finally licked his plate and brought it to the sink for me to clean. I then prompted him to the toilet, he tried to walk out before actually going, but I ushered him back in (large output). I then cleaned his teeth, we prepped teas, and then headed to the programme’s room. Sachin’s bpm was good and he was focused, albeit he was fussy about how pieces were being put in. We finished up at 1939, and sat on the couch. I got him his tea and a snack at 2000, even though the snack was different, he still didn’t want to eat (unless hand fed by his mum). Support Worker: Katy Seizure Risk Level: Risk level- low/med Previous Night Sleep: 8hr 10min of sleep Resting Heart Rate: 64 resting bpm Shift Time: 1600 to 2130 Date: Monday 15th July 2024 Sachin was sitting at the dining table drinking us pot of tea when I arrived. He seemed to be a little bit distant, but otherwise calm and relaxed. Once he finished, I helped him prepare the pot for tomorrow. He forgot to put one of the teabags in, and was initially confused when being prompted to correct this. We then ate dinner together, I cleaned up and then prompted him to toilet (no go), and then cleaned his teeth. We then prepped teas, which he did with no issues, and then headed to the programme’s room at approx. 1715. Sachin’s heart rate was initially in the high 80’s and, for the first hour, was needing quite a few reminders to focus. He eventually settled, but was still not getting many pieces of the puzzle in. We finished up at 1930 and went to couch with mum. We watched Charlie’s Angels, Sach has a tea and snack at 2000. Toilet again at 2045 (medium output). Despite the beginning of the evening, cat was relaxed and had a bpm mostly under 70 for the rest of the evening. Unsettled Sachin
Support Worker: Katy Seizure Risk Level: Med/High Previous Night Sleep: 6hr 37min Resting Heart Rate: 64 resting bpm Shift Time: 1600 to 2130 Date: Tuesday 9th July 2024 Sachin was drinking his arvo tea at the dining table when I greeted him today. He appeared to be in a silly mood and was laughing and smiling a lot, but didn’t seem to be very focused. I sat with him while he finished the tea, and then assisted him in preparing tomorrow’s pot. We sat on the couch for a short while, before I prompted Sach back to the dining table - which took multiple prompts as he was unfocused and wandering. West ate dinner together, Sach took his time again and needed multiple reminders to focus. I cleaned up our dishes and prompted him to the toilet (small output - took a long time to go) and cleaned his teeth. We made our teas and headed to the programme’s room. He needed some assistance to get started, but after approx. 10 minutes, he was focused and finding and placing the edge pieces by himself. His heart rate varied between 65-80 bpm, but was mostly on the low end of the scale. We finished up at 1930 and went to lounge room to relax with mum, only Sachin did not relax. His heart rate became elevated and did not calm down until approx. 2100. Toilet again - medium. No major issues. Support Worker: Katy
Seizure Risk Level: Risk level- low Previous Night Sleep: 8hr 26min of sleep Resting Heart Rate: 65 resting bpm Shift Time: 1600 to 2130 Date: Monday 8th July 2024 Zach was sitting at the dining table when I walked in. He appeared rested and was smiling and chatting. We played with the wooden blocks while waiting for dinner. Sach got up a couple times during this, and was running and skipping around the kitchen with a big grin on his face. His good mood could potentially be attributed to what was for dinner, which was chicken drumsticks in a curry sauce. Once dinner was finished, I helped Sach clean up and wash his hands, then prompted him to toilet (medium) and brushed his teeth. Sachin took his sweet time when prepping his tea, and we then headed to the programme’s room at approx. 1800. We finished the current puzzle and then moved onto the next. He needed nil prompting or assistance with starting the new puzzle, and his bpm was between 68-75 the whole time. We finished up at 1930 and sat on the couch with Praveena. Sach took approx. 45 mins to properly calm and settle. He refused to eat his corn chips. Toilet again at 2045 (medium). His bpm was excellent after approx. 2015 (below 75). Nil issues. Movie time. Walk Puzzle room. Drive and supermarket. WEATHER: 17° Sunny day. NOTES: Sachin was with his mother in the kitchen when I arrived, I immediately organised Sachin to go exercise before the evening chill set in. We went for a walk around the block twice, we kept it simple because Sachin was a little unsteady on his feet. Sachin and I put in a little work on his jigsaw while waiting for dinner, Sachin worked slowly and appeared happy to take it easy. Sachin had his dinner then we went for a drive with his father. We went to aldi, Sachin pushed the trolley and I steered for him. We returned home and watched TV show die hart 2. Sachin had 11:47 hours of sleep, his resting bpm is 61, his current bpm was low at 60 -80, occasionally he smiled and giggled, all these things indicated to me Sachin is calm and relaxed. Movie time. Walk Puzzle room. WEATHER: 15° Sunny day. NOTES: Once again Sachin had a good night, similar to last night, his resting BPM was lower though, which is good. Sachin had finished his pot of tea before I arrived, we cleaned it up in the sink and prepared it for tomorrow. Sachin and I headed off for a walk before the temperature and sun got too low. Sachin and I walked Tabletop road for 15 minutes then turned around and headed home. Sachin had no issues walking, he appeared happy. Sachin had his dinner of fish curry then we cleaned plates. Sachin used the toilet ( medium urine output ). I assisted Sachin brush his teeth then we made cups of tea, Sachin acquired his tea bags from the pantry and got them ready into his cup, once ready he pit his tea bags back in pantry. We moved into the programs room and got to work on his jigsaw. Sachin didn’t have any troubles tonight finding his pieces, he was fast and accurate, no mistakes. At 730 we moved out to the lounge and watched Taronga zoo TV show. 8 o’clock we made another cup of tea and returned to the lounge for Nine sports TV show. Sachin was relaxed and comfortable tonight, his bpm was low, in 60’s to 70’s, calm. No issues to report. Shift ceased at 2130 hours. Support worker:- James

Movie time. Walk Puzzle room. WEATHER: 15° Sunny day. NOTES: Sachin was happy, cheeky and appeared to have slept well. Sachin was drinking his pot of tea at the kitchen table when I arrived. Once he was done we went for a walk. Sachin was (at times) wobbly on his feet, he got puffed a little to quickly so we had a short walk. Sachin had his dinner and he appeared happy with his serving. I encouraged sachin to use the toilet but did not use it. We made cups of tea and put in some time in the jigsaw puzzle, Sachin was good and even forgiving with me, when I would click the pieces in wrong. At 730 I encouraged him to use the toilet again, this time he did go( large urine produced). 8 o’clock came and we made fresh cups of tea. We sat in the lounge with his mother and watched Wimbledon tennis on TV. Sachin was all smiles tonight, his bpm was good and low also his behaviour and attitude was excellent. Movie time. Walk Supermarket, drive. WEATHER: 17° Sunny day. NOTES: Sachin was his tea as I arrived, I assisted his father with a job in the garden then went inside. Sachin hung around the kitchen watching his mother prepare dinner. Sachin appeared to focus intently on everything she did, it looked remarkably like KFC, Sachin favourite food. Sachin got very excited before dinner, his heart rate flew up to 89-92 bpms in excitement when he saw it cooked and looked just like store bought. Sachin ate his dinner then he used the toilet (large urine). I assisted Sachin brush his teeth then into joggers. We went with his father to woolworths. Sachin and I walked around while he shopped. We returned home to a toilet break and cup of tea, we watched a film with his mother called “my sisters keeper”, Sachin also had an ice cream. Sachin has been spoiled tonight, and had a good night.
