Seizure risk level:- Risk level 4, Max.
Previous night Sleep:-  5.23 hrs.
Resting heart rate:- 62 bpm.
 
Activities:
Jigsaw puzzles in programs.
Walking.
 Exercise wobble board.
Weather:
21 degrees, Sunny.
 
Mood:
Sachin appeared more focused tonight on sights and sounds around him, during our evening walk with all the cars driving past and kids riding bikes and scooters around, Sachin did make a lot of vocal noises and giggles. Sachin appeared to calm once we were in the programs room where it was nice and quiet. He calmed down when there was no extra stimulating sights and sounds. Unfortunately in hindsight the damage might have already been done.
Notes:
Saj informed me Sachin had 5.23 hours sleep and will be treated as level 4 risk tonight.
Sachin was encouraged to use the toilet it took 3 attempts before he produced a medium sized urine.
I assisted Sachin brush his teeth, Sachin had a turn brushing tonight.
Sachin and I went for a short 10 minute walk back and forth in the street just out front of his home, Sachin completed 1077 steps at a very relaxed pace.
Sachin did 6 minutes on his exercise wobble board tonight, heart rate during was between 85 and 89.
Sachin and I then made our cups of tea and went into the programs room.
Sachin took his seat and calmly went about his Jigsaw puzzle, Bpm between 75 and 79.
At 1745 hours I heard Sachin making throat gurgling noises ( much like the sound a pig would make ) I observed Sachin Stare at the wall, his eyes were not moving, he was unresponsive to my voice, I observed Sachin for approximately another 5 seconds then alerted Praveena to come and assist me.
Below are time recordings, Bpm's and events.
 
1745 hours, Bpm 111, Sachin making gurgles, staring and unresponsive.
1748 hours, Bpm 117, Sachin is supported on his chair by Praveena and I, still staring and unresponsive.
1751 hours, Bpm 123, Praveena and I have lent Sachin back in his chair.
1752 hours, Bpm 125, Sachin begins to convulse and go rigid, Saj is called for to attend.
1752 to 1754 hours, Sachin is lowered to the ground by Saj and myself and placed in recovery position, I supported Sachins neck and kept his throat straight and airway open.
1755 hours Sachin begins to come out of his convulsions and his Bpm drops to 62.
1800 hours, Bpm 102, Sachin is still in recovery position, resting under a blanket.
1804 hours, Bpm 99, resting on ground.
1814 hours, Bpm 91, resting on ground.
1820 hours, Bpm 90, Sachin is assisted to his feet and into a chair by Saj and myself.
With Sachin back in his chair and focusing on his puzzle we resumed routine.
Medications given at 1830 hours.
Sachin continued on with his puzzle, his Bpm continued to slowly drop and by 1930 his Bpm was around 69.
Medication dispensed at 1930 hours, Lavender oil rubbed on Sachin’s top lip every 30 minutes since entering programs room.
At 1940 hours we finished in programs room and went to the kitchen.
I assisted Sachin make 3 cups of tea, one each and one for Nan.
Sachin then took his seat on the couch with a bowl of dorito’s as a snack.
Shift ceased at 2100 hours.
